I had joined State Bank of Patiala as a Probationary Officer on 02/03/2009. I had resigned from the Banks service on 14/05/2009. Subsequently, there was a wage revision for all the public sector banks including SBOP. The wage revision was implemented with effect from November 2007. Therefore, I am also entitled to the applicable benefits for the duration of my service in State Bank of Patiala. But I have not been paid my dues till date.

PARTICULARS OF INFORMATION REQUIRED:-

1. Total amount due to me under wage revision

2. Attested calculation sheet of my dues 3. Why have my dues not been paid to me till date? 4. Names of the officials who were responsible for non-payment of my dues. 5. When will my dues be paid to me?

6. Was any PF deducted during the course of my service at SBOP?

7. If yes, what was the total amount of PF deducted?

I had applied for my passport at Suvidha Centre, Patiala on 17/05/2010 vide application no. CHDZ17515110. But neither have I received my passport till date nor any other correspondence from your office providing me the reasons for not doing so.

When did your office receive my application? What action was taken on the received application? If my application was accepted, why was the passport not issued to me? If my application was rejected, was any correspondence sent to me? Whom should I contact for further enquiries regarding my application? Please provide the contact details of the concerned person.
