Congress Avenue Austin, Texas 78701-1494 pir@tea.state.tx.us debbie.ratcliffe@tea.state.tx.us Under the Texas Public Information Act, Chapter 552 of the Texas Government Code, which guarantees the public's access to information in the custody of governmental agencies, I request that you open to inspection and/or copying the following records: + The names of all teachers, administrators and those who serve any other role in the work of the school including contractors, who have been charged with any violation of state statute, federal or state law or any other code violation regarding the TAKS testing of high school students between the dates of January 1 2007 and January 1, 2012. This would include the names of all who have received revocations, voluntary surrender of certification, probated suspension, certificate suspended, inscribed reprimand and non-inscribed reprimand. + The disposition of each case, including case number if assigned, all documents, including statements, affidavits and other evidence filed regarding each case. This too would include the names of all who have received revocations, voluntary surrender of certification, probated suspension,

certificate suspended, inscribed reprimand and non-inscribed reprimand. If any of the requested records are maintained electronically, I seek access to them in an electronic format. If they exist in an electronic, searchable, database-readable format (such as .csv, .xls, ..hst, txt or .dbf files), I request access them to that in that searchable format. Also: I wish to come to your offices to inspect the nonelectronic records in question before any copies are made for me. If that is not possible for some reason, I ask that you perform no work on my behalf, copying or otherwise, until I can be given an estimate of the charges involved and agree to them. I also ask that I be allowed to access these records piecemeal, as they are gathered and can be made available, instead of waiting until all the requested records are gathered. As asserted by Texas law, in the case of an appeal for any part of this request does not justify the withholding of the other documents that are not part of that appeal.
