Toronto Police Check Program - Vulnerable Sector Screening

PSWs work with individuals who are considered vulnerable: anyone, who because of their age, disability or other circumstance, are in a position of dependence on others or are otherwise at a greater risk of being harmed by a person in a position of trust. (i.e. the elderly, children and people with disabilities). Therefore, all trainees are required to obtain a Police Reference Check including Vulnerable Sector Screening.

What is the Vulnerable Sector Screening?


The vulnerable sector screening is for individuals who intend to work or volunteer in the vulnerable sector. It will provide personal information pertaining to the applicants/individuals: Criminal Record Outstanding Charges Findings of not guilty by reason of mental disorder Probation, prohibition and other judicial orders which are in effect Convictions, pending charges under the Child and Family Services Act Apprehensions and/or contact under the Mental Health Act Suspect/Culprit information Criminal Investigations

Where to obtain a Police Reference Check?


Where to go to obtain a Police Reference Check depends on your current address: Students living within Toronto (Toronto, North York, Scarborough, East York and Etobicoke), may request a Police Check from the Toronto Police. Students living within York Region (Woodbridge, Vaughan, Markham, Richmond Hill, Newmarket, etc.) may request a Police Check from the York Region Police.

The Process (Toronto Police):


1. Consent to Disclosure Form
This form, when completed and signed, authorizes the Police to conduct an electronic search of various data banks for information outlined above. To obtain a copy of the Consent to Disclosure form, students must request a copy from the Metrocare Health Academy Ofce: Centrepoint Mall 6464 Yonge St, Suite 208 North York, ON M2M 3X4 Tel: (647) 352-8668

1. Complete the Consent to Disclosure Form


Please read the whole document to understand exactly what you are consenting to. After reading, please complete the following sections of the Consent to Disclosure Form: Surname (Last Name) GIven Names (First Name & Middle Name) Maiden Name or Other Names Used (if Applicable) Date of Birth Place of Birth Sex

Telephone (incl. Area Code) Drivers License Number Address City, Postal Code & Years at that address Previous Addresses (if current address is less than 5 years) Signature of Applicant Date Name and Address in the box at the bottom of the page

2. Payment
The service charge for students to obtain a Reference Check from the Toronto Police is $16.95 only. *** Important Note: NO PERSONAL CHEQUES ACCEPTED!!! Only certied cheques and money orders or bank drafts will be accepted. ! !

3. Submission to the Toronto Police


To submit the form to the Toronto Police, please place the completed Police Reference Check form inside an envelope along with your payment and mail to: Records Management Service Toronto Police Services 40 College St. Toronto, ON M5G 2J3 ! Note: Please do not forget to enclose a stamped self-addressed envelope.

4. Processing
The process usually takes 4 -6 weeks. After which, the waiver will be mailed back to you along with the results of the reference check. However, if there are any problems regarding your submission, the waiver will not be sent to you but to Metrocare Health Academy, along with instructions on what needs to be corrected to properly process it again.

5. Return to Metrocare Health Academy


Once you receive the waiver along with the results. Kindly bring the waiver back to the Metrocare Health Academy ofce for ling. Please note that the Police Check must be completed and returned prior to your Clinical Placement. Without it, the student can not commence clinical practice.

3. Requirements
Every applicant must submit their form along with 2 pieces of Government Issued Identication, one with a photo. Acceptable forms of photo ID and non-photo ID: Drivers License Foreign Drivers License Canadian Passport Canadian Citizenship Card Permanent Resident (PR) Card Certicate of Native Indian Status Student Identity card from a Canadian or Foreign institute Firearms Acquisition Certicate (FAC) Canadian National Institute for the blind (CNIB) identication card

Federal, Provincial or Municipal Identication card Military Family Identication Care (MFID)

8. Payment
The service fees: Volunteers: "" Students" " Employment" Extra Copies" $20.00 $20.00 with a valid student card $45.00 $5/per form

Payments must be cash, debit, VISA or Mastercard and must be paid in full at submission. *** Note: The fee is non-refundable

9. Processing
Processing takes approximately 4 to 6 weeks. The response will be mailed back to the applicant, unless the applicant requests to pick up the response from one of the York Region Polices 2 locations.

10. Return to Metrocare Health Academy


Once you receive the waiver along with the results, kindly bring the waiver back to the Metrocare Health Academy ofce for ling. Please note that the Police Check must be completed and returned prior to your Clinical Placement, without this the student can not commence clinical practice.
