NAME OF CADET I, _____________________________, having completed the academic requirements for NAME OF SCHOOL BSMT/BSMARE from ___________________________, and an MSAP 2012 Cadet Passer,

hereby signify my intention to apply for the IMMAJ-PJMCC Cadetship Training Program (CTP) for MSAP Cadets Batch 6. In this regard, I understand that I have to comply with the following requirements: 1. Accomplish properly and honestly the Application Form to be submitted with the other required attachments. 2. Provide other documents as may be needed by the IMMAJ-PJMCC Training Committee in order to make a fairly accurate assessment of my application; 3. Become an IMMAJ-PJMCC MSAP Cadet when accepted, and as such, be willing to: Sign a Memorandum of Agreement (MOA) with PJMCC and my future Manning Company/Principal Undergo three (3) months training onboard the TS Kapitan Felix Oca and nine (9) months training onboard a Japanese-owned/operated international commercial vessel. I also recognize that my acceptance into the program is subject to the sole and absolute discretion of the IMMAJ-PJMCC Training Committee.
