Re: Request for Verification of Vital Records Pursuant to Hawaii. Rev. Stat. 338-14.3 and 338-1 8(g)(4) Dear Director Fuddy, I am acting pro per in a case that involves president Barack Obama and I am trying to confirm information about a vital record which was acquired and submitted in legal proceedings in the Alaska Superior Court. 1 The case number is 3AN-12-9961 CI. As pointed out in the complaint, a school record of president Barack Obama indicates that his name was Barry Soetoro and was listed as an Indonesian citizen. In order for this vital event to be verified, a request is being made to verify under the laws of Hawaii, the existence of an amended birth certificate that would support the name change or possible adoption that would support the documentation that the president as a young child was enrolled by his parents as an Indonesian citizen. Moreover, in a divorce decree issued by the State of Hawaii between Ann Dunham Soetoro and Lolo Soetoro, the decree lists the couple as having two children and were dependent on the parents for their education. 2 Under Haw. Rev. Stat. 338-1 8(g)(3) a governmental agency associated with the education of president Barack Obama would have the means to verify an adoption and the concern in this case is: was the president enrolled as an Indonesian citizen or foreign student at any point in his education both overseas and in the United States. Under Hawaii law, the Department of Health "shall," upon request by a qualified applicant, will furnish "a verification of the existence of a certificate and any other information that the applicant provides to be verified relating to the vital event that pertains to the certificate." See Haw. Rev. Stat. 338-14.3.

A "qualified applicant" includes a private . .. attorney who seeks to confirm information about a vital event relating to any such record that was acquired during the course of or for purposes of legal proceedings." See Haw. Rev. Stat. 338-1 8(g)(4). I am acting pro per in this case and as such, I am a qualified applicant. Therefore, pursuant to the above referenced statutes, I hereby request official verification from the Hawaii State Department of Health on the existence of an amended version of the original birth certificate (Certificate of Live Birth).3 Pursuant to Haw. Rev. Stat. 338-14.3 (d), the "fee for a verification in lieu of a certified copy shall be a maximum of one half of the fee established in section 338-1 4.5 for the first certified copy of a certificate issued." And according to Haw. Rev. Stat. 338-14.5, the "'fees for certified copies of birth . . . certificates issued by the department of health shall consist of $10.00 for the first copy issued .... " I have enclosed $5.00 to cover the cost of the requested verification. Thank you for your prompt attention to this matter.
