Bills and Receipts of expenses incurred towards the treatment.

Certificate officials documents/ FIR Police Report 8. Death Certificate 9. Bills / Receipts of Expenses incurred for carrying out funeral

LOSS OF BAGGAGE (CHECKED IN BAGGAGE) Section C 1. Covering letter detailing the circumstances of the claim. 2. Duly filled and signed claim form from the insured. 3. Original policy schedule 4. Copy of airline ticket, Passport Copies.( Front , Back & Visa Copies) 5. PROPERTY IRREGULARITY REPORT issued by the airlines (compulsory).. 6. A letter from the airline stating that the baggage is permanently lost. New purchase not payable. Copy of cheque / advice of compensation received from the Airlines 7. List of items contained in baggage lost: - 1. Its price 2) Bills /receipts 3) Year of purchase. Bills and receipts for the item above USD 100( Per unit) 8 Baggage claim form submitted to airlines

DELAY OF BAGGAGE Section D 1. Covering letter detailing the circumstances of the claim. 2. Duly filled and signed claim form from the insured. 3. Original policy schedule 4. Copy of airline ticket, Passport Copies.( Front , Back & Visa Copies) 5. PROPERTY IRREGULARITY REPORT issued by the airlines (compulsory). 6. Original Bills / Receipts of Emergency items purchased (clothes, toiletries) during the period of delay (compulsory). LOSS OF PASSPORT Section E 1. Covering letter detailing the circumstances of the claim. 2. Duly filled and signed claim form from the insured. 3. Original policy schedule 4. Copy of airline ticket. 5. Copy of old & New passports. 6. Police report. (To be reported within 24 Hrs.) 7. Original Embassy Receipt 8. Original Bills / Receipts for expenses incurred in obtaining replacement passport or Emergency travel certificate. THIRD PARTY PROPERTY DAMAGE (TPPD) Section F 1. Covering letter detailing the circumstances of the claim. 2. Duly filled and signed claim form from the insured. 3. Original policy schedule 4. Copy of airline ticket, Passport Copies.( Front , Back & Visa Copies) 5. Receipts or estimates for repaired or damaged items. 6. Original policy schedule Copy of airline ticket, Passport Copies.( Front , Back & Visa Copies) Death certificate / medical report of hospitalization India Details of lost incurred for pre booked personal accommodation and travel charges along with supporting payment receipts and relevant documents.

Copy of airline ticket, Passport Copies.( Front , Back & Visa Copies) 5. Airlines letter stating the reason for flight delay / flight cancellation. 6. Detailed List of Expenses Incurred Due To Missed Connection of Flight or cancellation of flight along with Original Bills & Receipts (compulsory). 7. Complete Tour Itinerary along with Breaking Cost From Tour Operator For The Scheduled Tour.

Bounced Hotel Booking Section I 1. Covering letter detailing the circumstances of the claim. 2. Duly filled and signed claim form from the insured. 3. Original policy schedule 4. Copy of airline ticket, Passport Copies.( Front , Back & Visa Copies) 5. Letter from the Hotel stating the reason for service cancellation. 6. Bills and Payment receipts for pre-booked hotel services. 7. Bills and payment receipts for transportation and alternative hotel booking 8. Copy of cheque/statement for refunded amount.
