Dear Sir/Madam: Please accept instant request, pursuant to the Freedom of Information Act, in re: Complaints filed by the undersigned, Dr Joseph Zernik, against Bank HaPoalim, BM, with Bank of IsraelBanking Regulation. Instant request pertains to following two complaints, filed by the undersigned with Bank of Israel-Banking Regulation: 1) Complaint dated August 28, 2012. [1] The undersigned never received written confirmation of registration of the complaint, but was recently informed by phone that it was registered under File #82304-12. 2) Complaint dated January 4, 2013. [2] Receipt of the complaint by fax was confirmed by Bank of Israel staff, but no File # was provided. Instant request pertains to all paper and electronic records, related to the two complaints, referenced above, including but not limited to: 1. Papers filed by the undersigned, the complainant. 2. Communications by Bank HaPoalim, BM. 3. Communications by Bank of Israel with Bank HaPoalim. 4. Communications by Bank of Israel with the undersigned. 5. Communications among Bank of Israel officers. The undersigned spoke today by phone with Deputy Spokesperson Yoav, who informed the undersigned that: - There is no fee associated with instant Freedom of Information request, and - Bank of Israel would provide to the undersigned written confirmation of receipt and registration of instant Freedom of Information request, if asked to do so. Therefore, the undersigned herein asks that Bank of Israel provide written confirmation of receipt and the request number, designated for instant Freedom of Information request.
