Participation Agreement
By signing this document you will waive certain legal rights. PLEASE READ CARFULLY. 1. This is a binding legal agreement. As a participant in the programs, activities and events of the Oak Ridges Co-ed Slo-Pitch League, Richmond Hill Friday-Nite Mens Slo-Pitch League and Richmond Hill Sunday Mens Slo-Pitch League, the undersigned acknowledges and agrees to the following terms.

IN CONSIDERATION of the programs, activities, and events of the leagues mentioned above, I ASSURE TO YOU THAT: 2. 3. I am and/or believe that emotionally and mentally able to participate in the programs, activities and events of the ORCRSL, RHFMSL and RHSMSL leagues. I hereby acknowledge that I am aware of the risks and hazards associated with or related to Slo-Pitch. The risks and hazards include, but are not limited to injuries from: a. Executing strenuous and demanding physical techniques in Slo-Pitch. b. Grass, diamond surfaces and any other surfaces including bacterial infections and rashes; c. Falls to the ground due to uneven or irregular terrain or surfaces; d. Spinal Cord injuries which may render me permanently paralyzed; e. Mechanical failure or failure to properly use any piece of equipment ; f. Extreme weather conditions which may result in heatstroke, sunstroke or hypothermia; g. Contact, colliding or being struck by other participants, spectators, equipment or vehicles; h. Vigorous physical exertion and strenuous cardiovascular workouts and exerting various muscle groups; and i. Travel to and from competitive events and associated non-competitive events which are integral part of the organizations activities. Furthermore, I am aware that I may: j. Sustain injuries in Slo-Pitch that can be severe, cause spinal cord injuries and even be fatal; k. Experience anxiety while challenging myself during the activities, events and programs; l. Come into close contact with other participants, including the possibility of accidental and unexpected contact; m. Risk of injury is reduced if I follow all rules established for participation; and n. Risk of injury increases as I become fatigued.

I UNDERSTAND AND AGREE, on behalf of myself, my heirs, assigns, personal representatives and next of kin that my signing of this document constitutes: 5. I am registering myself willingly and participating voluntarily in these activities, events and programs. 6. I agree that there are risks in as described above and I will be exposed to these risks and hazards. 7. I agree to accept all these risks and hazards and be responsible for any injury or other loss which I receive while participating in these events, activities, and programs. 8. If something happens to me, I release the Organizers of responsibility for any claims, demands, actions and costs which might arise out of my participation. I understand Organizers to mean: ORCRSK, RHFMSL, and RHSMSL Executives, volunteers, associates, and representatives. 9. To release the League ORCRSL, RHFMSL, RHSMSL from liability for any and all claims, demands, and actions and costs that might arise out of me participating, even though such risks , injuries, loss, damage, claims, demands, actions or costs may have been caused by the negligence of ORCRSL, RHFMSL, RHSMSL leagues. 10. I give consent to have my name, e-mail, phone number to be used in any ORCRSL, RHFMSL, RHSMSL publication but not on our web site. Accident Insurance Executing this agreement will not preclude you from accident insurance coverage, subject to the terms and conditions of The NSA insurance Policy Discrimination/Harassment Policy All members have the right to participate in League programs and activities in an environment that is free from discrimination and/or harassment. Language or activities that defeat this objective shall not be tolerated. If such an offense then the Executives have the right to take away any or all member privileges of the ORCRSL, RHFMSL and RHSMSL leagues. Refund/Payment Policy A verbal or written request for a refund must be received to our Director before the following dates: 75% refund before season starts, 50% before June of participating year, 0% after June of participating year. I ACKNOWLEDGE MAKING THIS AGREEMENT Continuing with this registration indicates that I have read and understand the policies mentioned above. Continuing also indicates that you are the player registered and to be bound by this Legal Agreement even if you have not read the agreement.
