MODE OF SELECTION:1. HW(F) (Regular/Contractual) (H & FW Deptt and SC & ST Development Deptt).- The selection will be made strictly on the basis of merit on marks secured by the candidates in the qualifying examination (i.e 50% in HSC & 50% in ANMTC) 2. Additional ANM (NRHM) (Contractual)- The selection will be made strictly on the basis of merit on marks secured by the candidates in the qualifying examination (i.e Purely based upon marks secured in HSC) 3. Pharmacist (Contractual) ( H & FW Deptt) The selection will be made strictly on the basis of merit on marks secured by the candidates in the qualifying examination (i.e Purely based upon marks secured in DPharma subject to number of attempts to clear the DPharma) GENERAL CONDITION:1. The engagement for the post of Sl.No.2,3 & 4 is purely contractual basis and may be terminated at any time without notice and without assigning any reason thereof. 2. Candidate must have registered their names in the Employment Exchange and have valid registration card for Sl No-1,2 & 4. 3. No. of posts under different category may vary as per norm/ vacancy position at the time of selection. 4. The Post of Sl.No. 2,3 & 4, candidates cannot claim any regular appointment in future as a matter of right and also will not claim any finical benefit other than the monthly remuneration and other allowances as per Govt. rules. 5. Preference will be given to the candidates of the Sundargarh district (having nativity/ residence certificate for service only). In case sufficient candidates will not be available in Sundargarh district, applicants belonging to other district will be considered. 6. ORV Act will be applicable as per Govt. rule prevailing at the time of selection. 7. The undersigned reserves the right for cancellation/ modification of the advertisement and mode of selection (as will be decided by the selection committee and as will be instructed by the Govt.) at any time without assigning any reason thereof. 8. They will apply to the Chief District Medical officer, Sundargarh by Regd. Post/ Speed Post. Only. The Office will not be responsible for any postal delay. 9. AGE:- As per state Govt. rules for Sl No-1,2& 4. But for Sl No-3, maximum age limit will be 65 Years.

Attested copies of caste certificate in case of SC/ST/SEBC obtained from the competent authority. 3. Attested copies of Nativity/ Residential certificate issued from the competent authority for the purpose of service only within six month from the date of advertisement. 4. Two recent passport size photograph duly attested by Gazetted officer to be affixed with the application form. 5. Attested copy of registration certificate from concerned council. 6. Handicap Certificate If any should also be submitted. 7. One self addressed Regd. Post envelop duly stamped to be enclosed with the application. 8. Incomplete applications and without all required documents will be summarily rejected. 9. Candidates interested for HW(F) (Regular) under H & FW Deptt/ HW(F) (Contractual) under SC & ST Development Deptt/ Additional ANM NRHM has to apply separately. 10. If any candidate was selected in any category of posts out of above three HW(F)/ ANM category then she will be forfeited for subsequent cases if selected. The envelope containing in the application form should be super-scribed as APPLICATION FOR THE POST OF ___________(NAME OF THE POST ) and (NAME OF THE PROJECT) in block capital letters in separate applications and candidate should mention their respective categories on the envelope ( i.e GEN/SC/ST/ SEBC ) otherwise rejected.
