PHYSICAL EXAMINATION Brown color, warm to touch. Presence of edema in both arms Black color hair with white streaks, thin and dry in texture. No scalp or flaking Head symmetrically rounded. Neck non tender with full ROM. Neck symmetric without masses, scars. Lymph nodes non palpable. Trachea is in midline. Thyroid non palpable. Eyes 2cm apart without protrusion. Eyebrow sparse with equal distribution. Lacrimal apparatus non edematous. Pinkish, palpable conjunctiva, anecteric sclera. Left and right auricle without deformity, lumps or lesions. Auricle and mastoid process non tender. Lips moist, no lesion or ulceration. Buccal mucusa pink and moist with patchy areas of dark pigment on ventral surface of tongue, gums, and floor of mouth. No ulcers or nodules. Gums pink and moist without inflammation, bleeding or discoloration. Tongue midline when protruded, no lesions or masses. Upon auscultation presence of Rales. Symmetrical expansion of posterior and anterior chest. Full, soft, slightly tender in hypogastric area. No joint deformities. Full ROM in hands, elbow and shulder, knees and ankles. Moves upper and lower extremities freely against gravity and resistance. Posture slightly stooped. Pleasant and friendly. Appropraitely dress for the weather. Facial expression symmetric and correlate with mood and topic
