VILLAGE OF ALSIP MINUTES OF BOARD OF TRUSTEES COMMITTEE MEETING April 11, 2011 Mayor Kitching called the meeting to order at 7:30 p.m. Clerk Venhuizen called the roll with the following in attendance: Trustees Shapiro, Godfrey, McGreal, Michaels, Daddona and Quinn. Mayor Kitching and Clerk Venhuizen and Attorney Stephanides. MAYORS REPORT: Mayor Kitching reported the following will be presented at the Board meeting: a) A list of appointments. b) A Proclamation to proclaim May, 2011 as Motorcycle Awareness Month in the Village of Alsip. CLERKS REPORT: Clerk Venhuizen presented: a) March FOIA Report. PUBLIC FORUM: Mayor Kitching reported the Public Forum will be held at the end of the meeting in order to allow everyone present an opportunity to speak. FINANCE REPORT: Trustee Godfrey presented: a) A list of bills. b) Proposal from Clifton & Gunderson totaling $66,500 to perform the audit for FY 5/1/2010-4/30/2011. FIRE DEPARTMENT REPORT: Trustee Michaels presented: a) Request to approve incentive pay in the amount of $26,345.04 for the period 5/1/2010 to 4/30/2011. b) Request to approve holiday pay in the amount of $21,321.60 for the period 5/1/2010 to 4/30/2011. c) Thank you letter from the Hill Family directed toward Paramedics that responded within minutes to help Mr. Hill with a heart problem. POLICE DEPARTMENT: Trustee Shapiro presented: a) March 2011 Police Department Activity Report. b) Request for authorization to hire a crossing guard to replace the vacancy created when Laura Braden returned to maintenance. c) Letter of thanks from Maureen Paulmeyer, Principal, Nicole Niemiec, Assistant Principal of Prairie Junior High directed toward Officer Josh Spencer for assisting with a situation involving several students using inappropriate gang affiliated postings on Facebook. PUBLIC WORKS DEPARTMENT REPORT: Trustee McGreal presented: a) February 2011 Public Works Activity Report. b) 2010-2011 Winter Report. BUILDING DEPARTMENT REPORT: Trustee Shapiro presented: a) March 2011 Building Department Activity Report. b) Request for refund of a building permit totaling $140 from Thomas Dwyer, 4429 West Jean Street for work not done. (#07-6515) HEALTH & POLLUTION: Trustee Daddona presented; a) Report the Health Inspector will be attending the Southwest Health Conference on April 12, 2011. SEWER & WATER: Trustee Godfrey presented: a) March 2011 Water Department Activity Report. b) Request for authorization to advertise for bid the third and final phase of the 3.2 million gallon reservoir renovation. Specifications are available at Mr. Behrens office. LICENSE REPORT: Trustee Quinn presented: a) Request for approval of a list of licenses dated April 11, 2011. PLANNING/ZONING REPORT: Trustee Michaels presented: a) Report a Hearing is scheduled for April 27th, 2011 in the Board room to discuss the 125th & Cicero PUD. b) Elements Cremation LLC presentation will be held later in the meeting. c) Request for approval of an Ordinance Amending the Special Uses in the I-2 Industrial-Medium District to Add Crematoriums as a Special Use. d) Request for approval of an Ordinance Granting a Special Use Permit for the Property Located at 5620 and 5622 West 120th Street in the Village of Alsip, Cook County, Illinois. REPORTS OF SPECIAL COMMITTEES VILLAGE PROPERTY: Trustee Michaels presented: a) Request from Rodger Early to approve a proposal from M&R Landscape totaling $6,703.94 to perform a cleanup of seven sites and mowing for the next four weeks. There is no change in the prices charged by M&R in 2009. -1-

Board of Trustees Committee meeting April 11, 2011 INSURANCE COMMITTEE: Trustee McGreal presented: a) Report that additional information has been provided regarding adult and child well care; correction was made to other network benefits and dental coverage was included. Action Item: Motion by Trustee McGreal to approve Blue Cross/Blue Shield as the Village of Alsip health insurance carrier effective May 1, 2011; seconded by Trustee Godfrey. Roll #1: Voting aye: Trustees Shapiro, Godfrey, McGreal, Michaels, Daddona, and Quinn. Motion carried. b) Request to place Insurance Renewal for the Villages Property Casualty insurance on the next agenda. Greg Palumbo, Finance Director is securing final costs. ORDINANCE & LEGISLATION: No report. LOCAL IMPROVEMENT REPORT: No report. BOAT LAUNCH: Trustee Daddona presented: a) Request for approval to rehire two former employees. PRESENTATIONS, PETITIONS, COMMUNICATIONS: None. UNFINISHED BUSINESS: Presentation by Elements Cremation LLC. Administrative Introduction: Ronette Leal McCarthy, 9948 S. Spaulding, Evergreen Park, an Illinois licensed attorney will be Chief Operating Officer and will serve as legal counsel, providing direction and guidance for business development and growth; office and employee management; and, oversee all necessary local, county and state permits and compliance issues. Daniel J. McCarthy, 9948 S. Spaulding, Evergreen Park, an Illinois licensed funeral director since 1995, has worked in all areas of the funeral industry for almost twenty years. Daniel McCarthy will be the President/Chief Executive Officer for both the crematory and funeral services portion of the business. Kevin Finnery, 15660 S. LaSalle Street, South Holland, President, Cremation Systems. Ms. McCarthy opened the presentation and explained Elements Cremation LLC, (Elements) is a company that provides cremation services and memorial service coordination. The McCarthy Family, owners, with over 20 years of experience in the death care industry; combined their knowledge with Ronette McCarthy, attorney who has worked with the funeral cremation industry and end of life care for nearly 15 years. Elements desires to own and operate a crematory within the space located at 5620 and 5622 W. 120th Street, Alsip, Illinois. Elements is a full service cremation company. In Cook County the cremation rate is 35% and expected to exceed 50% in the next 10-15 years. The front 3,000 sq. ft. of the building will be used for an office, and the back 3,000-4,000 sq. ft. will be used for the cremation equipment. An attraction to this particular building was the double garages in the rear of the building that allows inside parking and ingress and egress of vehicles delivering persons for cremation with no vehicles on the street. Families will not come to the building to make arrangements; Elements will go to the place selected by the families. Elements will adhere to all local, state and federal guidelines and specifications for operation of a cremation facility. Elements engaged Cremation Systems of South Holland to design, build and install the equipment which will be used in the cremation process. Ms .McCarthy reported only one chamber will be used and any additional will need Board approval. Kevin Finnery, Cremation Systems, explained the equipment, a CFS-2300 Cremation Chamber. The CFS-2300 Cremation Chamber offers a one-step automatic operation, high efficiency refractory lining, quick changes hearth tiles, automatic pollution monitoring, touch screen digital controls, remote access via the internet. the unit has an extra-wide door opening for easy loading and retrieval, low overall height, single, air-cooled viewing port, meets or exceeds all applicable standards (NFPA54, NFPA70, NFPA82, NFPA211, UL508 and UL791), Fire Marshall crematorium monitoring system. Board of Trustees Committee meeting April 11, 2011 Attorney Stephanides explained special use means that it would be subject to final Village Board approval. Anytime a crematorium wanted to locate within the Village they would have to come to the Village for permission. Elements Creation is a business that has applied for the text amendment and the special use, and the Village has to address the request. In Illinois crematoriums are allowed and there is nothing the in state statute prohibiting them throughout the state. PUBLIC FORUM: The following were some of the comments made from persons in attendance as well as Board members, and many were answered by Ms. McCarthy. Fred Gruenberg, 11638 S. Marshfield, raised concern about possible soot emissions and was against the crematorium. Lynn Dwyer, 11937 Lockwood, posed the following questions: Whether the McCarthys owned another crematory, and if so, this is contrary to their statement at Planning Hearing; What background checks were made on the applicants? Who would invest money in a business they dont have? Ms. Dwyer reported she was against this as well as neighbors. What happens to the building if the request is denied? Thomas Dwyer, 11655 Leamington, questioned proprietary ownership in the property and lease to own; ownership interest should be disclosed to the Board; anticipated company growth of 50%; possible mercury toxins put out into the atmosphere; radiation exposure from persons that die of cancer while undergoing radiation; what experience does Mr. McCarthy have in establishing maintaining and checking proposed equipment; what are the State standards governing emissions from crematories; how many other community rejections were received and for what reasons; what is the impact on property values; what monetary investment was made? Trustee McGreal questioned improvements to the building, equipment purchase, signature of Mr. McCarthy as authorized agent. Stan Marek, 3759 W. 120th St. spoke on possible affect on allergies, Blue Island incinerator; Patti Brenza, 5100 W. 121st Street, already has asthma and concerned about additional pollutions in the air as well as opening the door for more crematoriums; Rocky Golumb, 11604 Kolmar, questioned mercury emissions; Edward Bohn, 5240 W. 120th Street, recommended zero tolerance on any emissions, and questioned what happens if unit malfunctions? Wendy Morrissey, 12017 S. Lockwood, presented list of names and address of neighbors who could not attend this meeting. Terry Vrshek, 11738 Keeler, questioned total cost for cremation services and affect on allergies; Teresa DelToro, 12000 S. Lockwood, questioned loss of long term residents if this business is approved; Denis Tische, 11515 Lawler, questioned emission of body particulates, equipment feedback system. NEW BUSINESS: None.
