It was a pleasure meeting with you, Ms. van Nordheim (Program Specialist) and Ms. Glick (Senior Administrator - School Health Services) on June 19, 2012. This email is to formalize my recommendations to MCPS on the three topics that were discussed at this meeting. 1- Pre-participation Sports Physical form: My recommendation is that MCPS use the "Pre-Participation Physical Evaluation" form recommended by both the Maryland State Department of Education (M.S.D.E.) and Maryland Public Secondary Schools Athletic Association. This form, created in 2010 by the American Academy of Family Physicians, American Academy of Pediatrics, American College of Sports Medicine, American Medical Society for Sports Medicine, American Orthopedic Society for Sports Medicine and American Osteopathy Academy of Sports Medicine,

is available in both English and Spanish. This form is available on-line at http: //www .mpssaa. orq/assets/publications/Physical%2OEvaluation%2OForm. pd f 2- Pre-Enrollment Physical Examination form: Physical examinations are required on first entry to school (COMAR 13A.05.07) . Maryland Students Record Manual 2011 currently state "The Department of Health and Mental Hygiene and the Department of Educations document, Record of Physical Examination" shall be used to meet this requirement. It is my recommendation that MCPS use the M.S.D.E. recommended form found on their website at: http: //www.mccshh.com/pdf/MarylandSchoolsRecordof_PhYSiCal_EXamiflatiOfl T_)df The above two recommendations require that the currently used MCPS Health Inventory form SR-6 (http://www.montgorneryschoolsmd.org/deoartments/fOrmS/Pdf/Sr6.Pdf) be fully revised, and it is my recommendation that the SR-6 solely address the pre-enrollment physical examination. It should not be combined with the pre-participation sports physical form. 3- MOPS Concussion Policy: I commend the MOPS Concussion policy, which has taken on the majority of recommendations written in the Maryland State Department of Education "Policies and Programs on Concussions for Public Schools and Youth Sports Programs". I would recommend that MOPS place their Concussion policy on the Athletics Division of the MOPS website, as well as each individual high school, for easy visibility and transparency.
