OVERVIEW
Welcome to the Camaligan Citizens Charter. This is a Product of collaborative undertaking among many entities and local offices operating in the local government of Camaligan. It is designed to serve as guidebook on key services of the municipal government. Further, it hopes to strengthen the social contract among our frontline service providers in the belief that there is always a better way. With the Camaligan Citizens Charter, the local government hopes to institutionalize new service orientation and consciousness among our service providers with improved service delivery performance at the same time advocating transparency in local governance. With this publication, the local government as frontline service providers of goods and services, institutionalize a mechanism for effectiveness and accountability in its transactions by laying more efficient systems and procedures to eliminate practices of red tape and graft and corruption, thereby creating client-friendly offices in the local government. The Citizens Charter: Making it Happen QUALITY SERVICE. This is what Mayor Emmanuel T. Prado envisioned the municipal government to provide its clientele. Prior to the issuance of Republic Act 9485(An Act to Improve Efficiency in the Delivery of Government Service to the Public by Reducing Bureaucratic Red Tape, Preventing Graft and Corruption, and Providing Penalties therefore), Mayor Emmanuel T. Prado has already issued a memorandum creating the Task Force Anti-Red Tape, now known as the Citizens Charter Team, in an effort to minimize red tape in its various external and internal transactions. Complying with the requirements of R.A. 9485, therefore, was no longer a very difficult task to do. All local government officials and workers have been clarified of the necessity to effect some changes in its operating procedures even before the issuance of this law in order to better serve our clientele. The crafting of Citizens Charter of Camaligan started with a series of workshops, meetings and consultations among the team, the local employees and the public through a consultative committee, largely participated in by the members of the Camaligan Peoples Council, a

network of non-government organizations, peoples organizations and the private sector. Camaligan as a municipality in the province of Camarines Sur to implement R.A. 9485, entailed technical assistance for this challenging task which the Department of Interior and Local Government and the Civil Service Commission ably provided. Conceptualizing the charter was one thing, but actually giving it shape and substance was an entirely different matter. Time and serious efforts were devoted by the people who have labored hard to make it happen. It has given the local administration an opportunity to look deeper and analyze existing systems and procedures in each office and undertake re-engineering of services. Certain policies had to be improved as support measures in order to sustain initial steps towards the elimination of red tape.

Among the landmark measures are the following: 1. Institutionalizing the Action Slip. All local operating offices are required to issue action slipsto its clientele for every transaction. The action slip provides information on the action taken by the receiving employee, regardless whether the documents submitted have been received or returned to the client. Further, it provides information on the date, time and name of the receiving officer which enhances accountability in each transaction. 2. No Lunch Break Policy. Through a memorandum issued by the Municipal Mayor, all heads of offices were required to submit the schedule of personnel to man their specific offices from 12:00 noon to 1:00 pm to ensure continuous service to our clientele. 3. Assignment of alternate signatory in the absence of the head of office. Except for offices who have personnel occupying the next-in-rank positions, such as assistants, the Municipal Mayor has appointed the most senior officer as alternate signatory upon recommendation of the head of office concerned, to ensure that transactions will not be disrupted in the absence of the head of office. 4. Strengthening the Citizens Complaints Desk or Public Assistance Desk. A specific area at the lobby of the municipal building has been assigned where complaints against erring employees of the local government shall be received. This is manned by a competent staff who shall assist the clientele and provide guidance on actions to be taken whenever necessary. The Mayor calls the attention of the office concerned and coordinates efforts to address the complaint or suggestion. The Suggestion Box is located at the lobby to receive feedbacks from the public. 5. Hotline Number. (054)4783365 / (054)4745090. To provide quality and appropriate emergency response to urgent situations, such as vehicular accidents, rescue, transporting victims requiring immediate medical attention, collection of garbage and

other related incidents, a hotline number has been provided where the public can get immediate action/response. 6. Client Feedback Form. The local administration acknowledges the need for public participation for the effective enforcement of the charter. This tool shall measure how well services have been delivered or how these can still be improved. Further, it shall also manifest client satisfaction towards performance of the local government insofar as service delivery is concerned.

SECURING BUSINESS PERMIT

ABOUT THE SERVICE


ALL businesses are required to secure a Business License & Mayors Permit and pay business taxes before the start of operation. The license and permit must be renewed from January 1-20 of every year. Business taxes for new enterprises are based on capitalization. For succeeding years, business taxes are computed as a percentage of gross receipts/sales. The capital investments or the gross sales/receipts are declared in a sworn declaration form to be subscribed by a notary public. Payments may be made annually, semi-annually or quarterly. If the last two modes of payments are preferred, succeeding payments can be done during the first 20 days of the 7 th month or succeeding quarters respectively. Innovations or re-engineering of the systems and procedures in this type of service have been introduced particularly in securing clearance from various offices such as Municipal Engineering Office, Bureau of Fire Protection, Zoning (MPDC) and Municipal Health Office. Said offices will prepare their certificates/clearance or report before January of the succeeding year.

SECURING ZONING CLEARANCE FOR BUSINESS PERMIT

ABOUT THE SERVICE ENTERPRISES are required to secure Zoning Clearance prior to application for Business Permit to ensure that the enterprise is allowed in the target location per the Comprehensive Land Use Plan (CLUP) and other relevant zoning and land use ordinance.

RETIRING A BUSINESS LICENSE

BUSINESS that ceased to operate or whose ownership has changed must file with the Municipal Treasurers Office an application for Retirement or Change of Ownership of Business, as the case maybe. This should be done to update the Municipal Governments record and to avoid accumulation of tax payments and penalties.

SECURING A CERTIFICATE OF SITE ZONING

ABOUT THE SERVICE SITE Zoning Classification is requested for record and reference purposes. The Municipalitys Comprehensive Land Use Plan (CLUP) was last approved by the SangguniangPanlalawigan of Camarines Sur in February 14, 2006 under SangguniangPanlalawigan Resolution No. 69 of 2006. SECURING SANITATION CLEARANCE FOR BUSINESS PERMIT


ABOUT THE SERVICE ALL enterprises are required to secure a Sanitation Clearance upon application for business permit to ensure that the enterprise complies with the Sanitation Code of the Philippines as adopted by the Municipality and other relevant health-related ordinances. ABOUT THE SERVICE ALL enterprises and private persons constructing a new building or applying for expansion / renovation are required to secure a zoning clearance before applying for a building permit. This should be done before the start of construction to ensure that the building/business is allowed in the target location as per Municipal Comprehensive Land Use Plan (CLUP). The permit becomes null and void if work does not commence within one year from the date of issuance of permit, or if the building or work is suspended or abandoned at any time after construction has started within 120 days.

SECURING AN OCCUPANCY PERMIT

ABOUT THE SERVICE An occupancy permit is required before any building/structure is used or occupied. This may be secured after the completion of a structure. If there is any change in the existing use or occupancy classification of a building, structure or any portion thereof, an occupancy permit is also required.

Second Endorsement Present duly accomplished forms and related documents to any member of the building staff for initial verification of the requirements. A 2nd endorsement will be given to the client for issuance of a Fire Safety Inspection Certificate from the Bureau of Fire Protection. 3. Submit requirements after clearance has been secured from BFP together with the duly accomplished forms and related documents; safety certificates as-built plans; and detailed sketch of location. PAYING REAL PROPERTY TAX

ABOUT THE SERVICE OWNERS of land, buildings, and machineries have to pay the real property taxes annually. Taxes are the percentage of the propertys taxable value. Taxable value is based on the Municipal Assessors Schedule of Fair Market Value. Real Property Tax payments are made at the Land Tax Division of the MTO. Taxpayers may choose to pay on an annual or quarterly basis. Discounts are given to those who pay the amount in advance.

ABOUT THE SERVICE A certificate of Real Property Tax Payment is required in certain transactions (e.g. securing a building permit), as proof that taxes on real property have been paid and updated. This can be secured from the Land Tax Division of the Municipal Treasurers Office. REQUIREMENT(S) Copy of Real Property Tax Declaration Community Tax Certificate Documentary Stamp Certification of Tax Payment (O.R.)

If the purpose of the requested certification is for transfer of property ownership, the following are further required: Duly notarized document supporting the transfer of ownership Transfer Tax Payment (Official Receipt)

ABOUT THE SERVICE THE couple must be a male and female whose ages must be at least 18 years old and above, are not related and has no legal impediment to marry. Contracting parties must be separate sworn applications for marriage licence with the proper Local Civil Registrar of the place where either one of the contracting parties habitually resides. RECONSTRUCTION OF MARRIAGE CONTRACT

ABOUT THE SERVICE Per OCRG Circular No. 96-1 dated January 31 1996, reconstruction of the Certificate of Marriage could be done at the Office of the Civil Registrar of the Municipality where marriage solemnization took place. If both the applicant and the solemnizing officer have no copy of the marriage contract, information may be obtained from the Registry of Marriage of the church or Office. However, if records can no longer be obtained, the contracting party shall prepare an affidavit declaring therein all information needed in accomplishing the Certificate of Marriage.

ABOUT THE SERVICE Supplemental report using the appropriate form of the Certificate of Live Birth, Marriage, Death and Fatal Death may be field to supply information inadvertently omitted when the document was registered. However, the Medical Certificate in the Certificate of Death and Fatal Deaths and all applicable certifications contained in the Certificate of Marriage should be accomplished correctly and completely before registration. Hence, no supplemental report having reference to the mentioned certificates is acceptable. Only one supplemental report for not more than two omitted information in any registered event will be accepted by the Local Civil Registrar. In cases where there are more than two omitted information, all papers related thereto must be forwarded to the Office of the Civil Registrar General.

ABOUT THE SERVICE Out of town reporting of birth is done when the birth of a child is reported to the Civil Registrar of a City or Municipality which is not the place of birth. The duty of Civil Registrar is to prepare the Certificate of Live Birth of the Child to be forwarded to the Office of Civil Registrar of the City or Municipality where the birth occurred for registration.

ABOUT THE SERVICE This service is provided to all pregnant women to prevent complication during pregnancy. Pre-natal care is given every Wednesday in all health facilities. Midwives also attend to and conduct normal deliveries to expectant mothers. Care of the baby and the mother is also done during home visits.
