Department: Beatitudes at Home Job Code: Division: Support Services Grade: Reports To: Beatitudes at Home Director EEO Code: Service Worker Revision Date: February 2013 WC Code: 8824 ______________________________________________________________________ Position Summary This position is responsible for providing personal care and companionship in the home setting to residents of the Beatitudes Campus and off-Campus clients. _____________________________________________________________________ Essential Functions Adheres to the written Care Plan to: Assist the client with activities of daily living such as bathing, grooming, transferring, ambulation, range of motion exercises, and nutritional needs. Assist client with instrumental activities of daily living such as shopping, light housekeeping, pet care, meal prep and maintaining a clean, safe environment. Perform delegated tasks as instructed by supervisor. Observes and reports clients condition and safety hazards to supervisor and office team in a timely manner. Documents observations and activities in an accurate, complete and timely manner. Possesses organizational skills necessary to carry out visit/shift and to complete assigned tasks. Completes the Home Companion log book after every shift to keep client status and care plan information current. Provides documents to keep employee information and certifications current. Protects client heath information according to HIPPA and federal and state law and regulations. Maintains professional appearances, attitude and demeanor while on duty. Performs other miscellaneous tasks which would normally be required of a nonmedical caregiver. Escorts clients to and from appointments, errands and activities. May drive own vehicle or clients vehicle.

Participate in and supports quality assurance/continuous quality improvement programs and processes. Attend all mandatory inservices and mandatory quarterly meetings, as directed. Consistently maintains complete confidentiality of all medical, financial, employee, computer or other sensitive material which may jeopardize the privacy of others. Demonstrate orientation towards excellent customer service in all interactions by conducting oneself in a cooperative, courteous, positive, and professional manner. Display sensitivity and awareness of others. Follow through with established expectations and keeps clients informed of progress. Support, maintains, and promotes Beatitudes Mission Statement. Perform other duties as requested or assigned.

Position Qualifications ________________________________________________________________ Minimum Training and Experience Minimum Education: High School Diploma or GED Minimum Experience: No experience required, work experience with seniors preferred Minimum Age: 18 years of age Requirements: Current, valid drivers license Reliable form of transportation (own vehicle required) Current vehicle registration and insurance CPR and First Aid certification Fingerprint clearance card Caregiver Certification or CNA License preferred but not required Level of Supervision: Minimal Management Experience Required: None

______________________________________________________________________ Knowledge and Abilities Required to Perform Essential Functions Knowledge of: State and Federal rules, regulations, and laws as they pertain to the Resident Services Department Changes that occur and needs within the geriatric population. Safe operation of equipment Use of department specfic equipment Proper telephone answering procedures Terminology used within the department Basic medical terminology Practices, procedures, methods, and practice of the facility Records, graphs, reports, etc. used in the daily performance of duties Methods and procedures involved in handling emergency situations Medication administration Physical Demands: Ability to lift at least 25lbs Ability to clearly hear vehicle sounds, traffic sounds, emergency vehicle sirens, and resident voices Unlimited (full) range of physical motion Considerable standing, reaching, stooping, bending, kneeling, crouching Must be alert at all times Vision Requirements: Eyesight at or correctable to meet requirement for a drivers license Must possess sight and hearing senses, or use prosthetics that will enable these senses to function adequately, so that the requirements of this position can be fully met

May perform emergency care. Must present a professional appearance/manner as you represent the Beatitudes (both on and off Campus) Subject to interruptions and frequent change of schedule

Daily advocating for clients and families Assist with ambulation and light exercise Regular communication with office staff and supervisor regarding client condition Report significant changes in clients status Alzheimers, Parkinsons, chronic disease, diabetes, and end-of-life care Maintaining a safe living environment Provide medication reminders Personal care including peri care, adult brief, and incontinence care Pet care, including but not limited to feeding, giving fresh water, cleaning litter boxes Plan and prepare meals, followed by clean-up Monitor food expiration dates, make future meals Make beds and change linens, as needed Light housekeeping to include dusting and vacuuming Assistance with bathing, dressing and grooming Laundry and ironing Take out garbage Run errands (pickup prescriptions, dry cleaning) Escort to appointments (hair salon, physical therapy, etc.) Escort to religious services or events Engage in physical and mental exercises Engage in activities (games, memory books) Maintain calendar and organize mail Companionship and general conversation Record daily activities Completion of home companion log book after every shift Call hours worked hotline every day, especially the last day of the month Recording and reporting miles driven for client
