Accountability form for furniture, Fixture and Equipment (FFE) will be forwarded to respective offices / employees for acknowledgement of receipt of all FFEs assigned to them.     StockRequisition 1. AllofficesmustusetheStockRequisitionForm(SRF)whenrequestingstock/supplies. 2. Allofficesmustsubmittheirrequestofstocks/suppliesusingtheSRFonceamonthonly. DeadlineofsubmissionofSRFofallofficesshallbeonthe5thworkingdayofthemonth.Any requestlaterthanthesaiddeadlinewillbeservedthefollowingmonth.Rushrequestsarehighly discouraged. 3. UponreceiptofSRFfromofficesbyPropertyManagementOffice(PMO),PMOshallimmediately processtherequest.ThePMOwillbegiventen(10)workingdaysfortheprocessingofallSRF. 4. AllofficeswillaccomplishaConfirmationofReceipt(COR)formuponreceiptofrequested stocks/supplies. AccountabilityFormandMonitoringSheetforFurniture,FixtureandEquipment(FFE) 1. AccountabilityformforFFEwillbeforwardedtorespectiveoffices/employeesfor acknowledgementofreceiptofallFFEsassignedtothem. 2. FFEMonitoringsheetwillbethenaccomplishedevery15thofthemonth. 3. Allnewlypurchased/GSOfabricatedFFEswillhavetobeforwardedtoPMOfordocumentation andproperassignment. 4. (Incaseof)damaged,brokenordefectiveparts,malfunctionofFFEs,theaccountableemployee willhavetoreportitimmediatelytothePMO.Failuretodosowillgivefullaccountabilityof employeeduetonegligence. Borrowing/returningforfurniture,fixtureandEquipment(FFE) 1. AllemployeesmustuseFFEBorrowing/ReturningformwhenrequestinganyFFEforan internal/externalactivity. 2. TheemployeewillsecureandaccomplishanFFEBorrowing/ReturningformatthePMOfive(5) workingdaysbeforethedateoftheactivity. 3. UponreceiptoftheformfromemployeebyPMO,PMOshallimmediatelyprocesstherequest. 4. TherequestingemployeewilltakefullresponsibilityofallborrowedFFEs.LostordamagedFFEs willbechargedtohim. 5. TherequestingemployeemustreturntheFFEstothePMOthedayaftertheactivity.
