Date: Aug. 30, 2012 Findings: Part examined: Chest x-ray supine- Heart is normal in size. Hili are normal. Pulmonary vasculatures in both lung fields. There are infiltrates in both lungs fields. Lateral costophrenic sinuses are intact.
Date: Sept. 4, 2012 Finding: Part examined: Chest- A comparison with the film shows no significant interval change in the infiltrates in the lung fields.
PSYCHOSOCIAL PROFILE
A. Health care practices and beliefs Before the patient got admitted to the hospital and got ill. She had experienced some symptoms related to her condition at their home like persistent cough and being nauseated and because of that, she took over the counter drugs such as Cotrimoxazole and carbocisteine for her cough. After 3 days of taking self-medication, she decided to go to the hospital for consultation regarding her condition. B. Typical Day Mrs. V daily routines is quite simple. She usually wakes up at 5:30 am and prepares for her breakfast. She then, does the household chores on her own. After which, she collects vegetables from her back yard, then sells them to her neighbors. When she gets back to her house, she eats. Then when the night comes, she sleeps early.
C. Nutritional Pattern According to the patient, she usually eats twice a day. She says that she barely has time to eat, since she has a lot of things to do at home. Her meals consist of a vegetables like Kamote-tops, Kangkong and Tagabang.
D. Activity and Exercise At the patients house, she does all the chores on her own. Chores such as sweeping, washing the dishes and cleaning the dust off the shelves. She also gardens in her backyard. E. Recreation, pets, and hobbies She often spends her leisure time at her house, watching TV. And she sometimes visits her friends next door. And She also considers gardening as a hobby of hers she enjoys. F. Sleep Rest Pattern The patient wakes up at 5:30AM. At the afternoon, she usually takes a short nap shortly after eating. At night, she retires to her bed at 8:30PM G. Socioeconomic Status Her socioeconomic status is good. She manages to buy the things she needs at a daily basis. She mentions that, she does have savings she keeps safe. H. Environmental Health Problems She lives in Sto. Nino, Kidapawan City. Her house is near a road. The area she lives in is very much environmental friendly. And her neighors house is just next to hers.
I. Roles Relationship, Self concept Mrs. BA is satisfied for what her children have achieved. She also feels very proud because she guided her children very well and did her role as a good mother and wife.
