Interior Stumpage Rate Request Form Procedures

Please complete all fields on the form. Refer to Chapter 6 of the Interior Appraisal Manual in effect on the Effective Date (Issue
Date in FTA or date rate is determined for advertising purposes) of the cutting authority for more information. The form can be
faxed (numbers below), emailed, or mailed to the Region however please ensure that the signatures and seal(s) are visible.

Licence/CP Enter Licence number and Cutting Authority (if applicable).
Forest District Enter the Forest District the Licence is within.
Forest Zone Enter the Forest Zone the Licence is within as per Section 6.1 of the IAM.
Timber Supply Area Enter the Timber Supply Area the Licence is within.
Timber Supply Block Enter the Timber Supply Block the Licence is within (must be alpha character).
Competitive Licence Check “Yes” if the Licence is competitively awarded, otherwise, check “No”. For competitive
Licences, once the Licence has been issued and FTA updated, please attach or fax a copy of the
application and tender documents (FS 574 or equivalent) to the Region. If the bonus bid is known at
the time the form is submitted, please enter in the comments field.
Timber Mark Enter the Timber Mark
Point of Appraisal Enter a valid Point of Appraisal from Table 4-1 of the IAM in effect on the effective date of the
Licence.
AAC (m3/year) Enter the AAC of the Licence (if applicable). Make note in the comment field if there are any
known changes to the AAC (eg uplift increase or decrease and the applicable dates).
Area (ha) Enter the area in hectares of the Licence (should match FTA).
Effective Date Enter the issue date of the Licence as entered in FTA or for competitive sales, the date the rate was
determined for advertising purposes.
Rate Calculation Method Select the appropriate rate calculation method. Please consult the IAM for the requirements of each
method to ensure the Licence meets the criteria. Where applicable, complete the Silviculture section
of the form. Where variable cost upset (VCU) was used, please attach the VCU form showing the
calculations. Where applicable, select which grades the bonus bid applies to. For competitively
awarded Licences, please attach or fax the application and tender, and enter the bonus bid in the
comments field. If the bonus bid applies to deciduous, indicate which grades of deciduous in the
comments section.

Eligible road amortization agreements for woodlots are those that were approved for cutting permits
issued for a woodlot licence with an effective date prior to December 1, 2008.

For rates under Section 6.5, please indicate if it has been greater than or less than 3 years since the
timber was decked or partially harvested.

Where a Licence has been issued for the removal of timber from an immature stand and where the
Licensee is the holder of a Ministry site preparation or site rehabilitation contract, please reference
the contract in the comments section.
Species Volume Estimates Enter volume estimate by species. Ensure the total net conifer and deciduous volumes are
completed (where applicable).
Silviculture Complete the Silviculture Responsibility section.
Crown: Where there is a requirement to establish a free growing crop of trees and the expense will
be incurred by the Crown (a Silviculture Levy, calculated by the DM, will be added)
Licensee: Where there is a requirement to establish a free growing crop of trees and the expense will
be incurred by the Licensee.
N/A: Where there is no requirement to establish a free growing crop of trees (a silviculture cost
estimate from the Interior Basic Silviculture Costs by Species Table - Interior Parameters (provided
quarterly by Revenue branch) will be added to the rate).

Indicate the grades the levy will apply to.

If the levy applies to deciduous, indicate which grades of deciduous in the comments section.

Indicate the Silviculture Levy determined by the DM (may be $0.00). Supporting documentation for
the levy should be kept on the District file.
Comments Enter any comments, particularly around any unusual aspects of the form. Any bonus bid must be
entered here.
Authority Except for Woodlots and Community Forest Agreements, ensure the application is signed and sealed
by a Forest Professional.

The signature of the District Review Officer is also required.
