Exhausted Illness Leave

Please remind your SAA to contact their Placement Specialist immediately when they stop reporting time for an
employee who has exhausted their illness balance. This will assist with avoiding an overpayment for the teacher.

Extended Substitute Requests

Requests for extended substitutes will be processed only through April 3 in single-track schools to prevent overpayments
during Spring Recess. It will be necessary for SAAs to submit another request beginning April 13 for substitute teachers
that will continue in the same position.

CTEL Online Test Prep Classes

Please advise teachers who do not hold an English learner (EL) authorization, preparing for the “California Teacher of
English Learner” (CTEL) Examination on June 13, 2009, of online test preparation classes. Teachers can access
information about this alternative to the LACOE Test Prep classes, by visiting http://www.kdsi.org/LAUSD-CTEL.

Leave of Absence Forms

A Request for Leave of Absence (Form 1065) accompanied by appropriate documentation is required when teachers are
absent from school for more than 20 consecutive days. A Request to Return from Leave (Form 1038) is needed upon
return. Forms that are not received by our office in a timely manner may result in payroll problems. These forms are
available at teachinla.com, link to Policies and Forms, then Placement and Assignments. For questions please contact
your Personnel Specialist at the number listed above.

To further assist in getting this information in a timely manner, principals should remember that after 5 consecutive days
of absence, an employee should submit a medical certificate for absence due to illness. Per Article XII, 12.9, of the
Collective Bargaining Agreement “An employee absent from duty for illness, injury or other disability for more than 5
consecutive days shall be required to submit either the Certification/Request of Absence for Illness, Family Illness, New
Child (Form 60.ILL) completed by the attending physician or statement from the attending physician on letterhead
attached to Form 60.ILL.
