healthcare,” said Heidi King, deputy director, DoD Patient Safety Program.

The TEAM-UP checklist encourages beneficiaries to:

z Team together with providers and become an active member of their healthcare team.
PSAs z Educate themselves about the members of their healthcare team and know the
proposed treatment plan.
z Ask questions.
z Manage medications by writing them down, reading labels carefully and alerting the
healthcare team if a medication does not match what was prescribed.
z Understand changes in their care plan.
z Provide their perspective and any concerns with the healthcare team. Feel free to speak
up.

It all adds up to TEAM-UP!

“We’re partnering for a new level of care,” said Ms. King. Beneficiaries should talk with
their primary care manager about taking a more active role in their healthcare. Visit
