We have received your Application for Continued Temporary Housing Assistance (from now on known as the Application). We have reviewed your Application and any documents you provided. The following information is missing or incomplete, and is necessary to determine your eligibility for continued rental assistance. Please read carefully and complete each section below or submit the documents listed in each section::

INCOME (Section 9 of your Application) Your income information is incomplete. You must submit documentation to validate any amount you supplied on the form and you will also need to provide the frequency at which the individual is paid by his or her employer. In the pay cycle box, please shade in the circle that is applicable. To ensure proper calculation and evaluation of your Household's Income, please shade in the circle indicating how often you are paid. If you have more than one job, please list yourself separately for each job. If your Pay Cycle is not listed, please shade in the circle for "Other." Income means: 1) Wages and salaries, overtime pay, commissions, fees, tips and bonuses and other compensation for personal services; 2) Interest, dividends and other net income of any kind from real or personal property; 3) Full amount of periodic amounts received from Social Security, annuities, insurance policies, retirement funds, pensions, disability or death benefits and other similar types of periodic receipts, including lump-sum amount or prospective monthly amounts for the delayed start of a periodic amount; 4) Payments in lieu of earnings, such as unemployment and disability compensation, worker's compensation and severance pay; and 5) Welfare assistance.

CONT ASST - SUBMISSION OF DOCUMENTS (8 of your Application)-Current lease You did not provide a copy of your current lease agreement or your lease agreement was not signed. A completed lease is required to verify the amount for rent of the temporary housing unit that you included as part of your Housing Costs on your Application for Continued Temporary Housing Assistance. Please submit a copy of your signed lease agreement. If you do not submit the requested document, FEMA will not be able to verify your Housing Costs, and therefore will not be able to provide assistance for your temporary housing needs. Please submit the name and/or contact information for your landlord. If you do not submit the requested information, FEMA will not be able to provide assistance for your temporary housing needs

In order to receive your rent payment as soon as possible, please make sure you have completely answered the sections above and submitted any necessary documents. Mail or fax this letter and documents to the below address before you run out of funds provided to you by FEMA for rent. If you have applied for other disaster assistance, you will receive a separate letter from us.
