APPLICATION GUIDE

How to Apply to UPOU Graduate Programs


Step 1. Obtain an application for admission form from one of the following: 1. Downloadable Forms 2. UPOU Learning Center nearest you 3. The Office of the University Registrar Step 2. Fill-out correctly and completely the application form. If you are a former UP student, indicate your student number in the form. Step 3. Prepare the other application documents. See list below: a. Official Transcript of Records (1 original and 1 photocopy) Notes: i. PhD applicants should submit an original and official TOR for both bachelors and masters degrees. ii. The TOR should bear the school dry seal and imprint, and the Registrars signature in ink, and with acceptable remarks (please see Attachment A for the list of remarks in a TOR that will make it valid as a transfer credential) b. Two letters of recommendation from: Employer/supervisor Former professor Former program adviser Note: Use UPOU Form 1a for this purpose. The form is attached to the printed application form. Or you can download it from the UPOU website. c. Two identical 2 x 2 photos and two identical 1 x 1 photos, with your name and the program applied for printed at the back of the photos d. A 500-word essay (refer to item #30 of the application for admission form) e. Photocopy of PRC license (not expired)/board exam rating (if applying for admission to the Master of Nursing program) f. Photocopy of PBET/LET eligibility (for teacher applicants only)

g. Certificate of Teaching Load, UPOU Form 1b (for teacher applicants only) Note: You can download the form (i.e. UPOU Form 1b) from the UPOU website. Step 4. Pay the application fee of PhP500.00 for applicants living in the Philippines. See payment options and procedure for locally based applicants. Pay the application fee of USD 100.00 for applicants living outside the Philippines. See payment options and procedure for offshore applicants. Step 5. Submit application documents. Mail or bring your accomplished application form together with the other documents and proof of payment on or before the deadline to: The Admissions Section Office of the University Registrar UP Open University Los Baos, Laguna 4031 Philippines Step 6. Take the admission examination if you are applying to either the Master of Public Management (MPM) or PhD in Education program. The Office of the University Registrar will contact you regarding the details of the examination. Step 7. Wait for the results of your application. You will know the results of your application through the following:

How to Apply to the Professional Teaching Certification program

Step 1. Obtain an application for admission to the PTC form from one of the following:

Step 2. Fill-out correctly and completely the application form. If you are a former UP student, indicate your student number in the form. Step 3. Prepare the other application documents. See list below: a. Photocopy of valid Transcript of Records for all degrees earned Notes: i. The transcript of records should state the degree obtained the date of graduation and the S.O. Number (Special Order from the Bureau of Private Schools). ii. The TOR should bear the school dry seal and imprint, and the Registrars signature in ink, and with acceptable remarks (please see Attachment A for the list of remarks in a TOR that will make it valid as a transfer credential) b. Two recent identical 2 x 2 photos and two identical 1 x 1 photos, with your name and the program applied for printed at the back of the photos Step 4. Pay the application fee of PhP500.00 for applicants living in the Philippines and USD 100.00 for applicants living outside the Philippines. See payment options and procedure. Step 5. How to Apply to the Professional Teaching Certification program

Refunds A student who has paid his/her matriculation fee and who wishes to withdraw his/her registration shall be entitled to a refund of his/her matriculation fees, except entrance and registration fees, in accordance with the following schedule:

Withdrawal before the opening of classes100% Withdrawal before the First Study Session 80% (One month after the start of classes) Course materials are the property of students, hence, no refund will be made. There shall be no refunds for those who withdraw registration after the First Study Session.

Payment Options and Procedures

You may choose the option that is most convenient to you.

I. Payment Through a PNB Branch, Bank Transfer, or Remittance Center


A. If you are living in the Philippines a. Go to the nearest Philippine National Bank (PNB) branch, and ask for a Bills Payment Slip.

b. Fill out three copies of the slip completely and legibly with the following information: Account name: UP Open University Reference number: your student number (if you are a former UP student) or YEAR -83000 (example: 200983000, if you have no UP student number yet) Subscriber name: your complete name c. Print your name (again), the purpose of payment (e.g. application fee), and the program you are applying for at the bottom part of the slip. d. Make the deposit and get two copies of the validated slip. e. Attach one copy of the validated slip to your documents. This will serve as your proof of payment B. If you are living outside the Philippines 1. Payment through a PNB Branch a. Go to a nearest PNB branch, and get a deposit slip. b. Fill out three (3) slips completely and legibly; use the following account information: For peso deposits, write UPOU Revolving Fund as the account name, and 399693100012 as the accountnumber. For dollar deposits, write UPOU Trust Fund as the account name, and 399692900011 as the account number. Indicate PNB-UP Los Baos Branch as the depository office. c. Print your name, purpose of payment (e.g. application fee), and the program you are applying for at the bottom part of the slip. d. Make the deposit and get two copies of the bank validated slip. e. Attach one copy of the validated slip to your documents. This will serve as your proof of payment

2. Payment through Bank Transfer or Remittance Center a. Go to any bank that accepts a bank transfer, or go to a nearby remittance center. b. Fill out three copies of the deposit/remittance slip completely and legibly with the following account information: For peso deposits, write UPOU Revolving Fund as the account name, and 399693100012 as the account number. For dollar deposits, use UPOU Trust Fund as the account name, and 399692900011 as the account number. Indicate PNB-UP Los Baos Branch as the depository office. For banks/remittance centers that require a bank swift code, indicate pnbmphmm as the code, and NY026003007 as the routing number. c. Print your name, purpose of payment (e.g. application fee), and the program you are applying for at the bottompart of the slip. d. Make the deposit and get two copies of the bank validated slip. e. Attach one copy of the validated slip to your documents. This will serve as your proof of payment.

II. Payment Through a Credit Card


a. Through all UPOU Learning Centers: Visit a UPOU Learning Center and make the transaction there. b. Through the Gateway:Email registration@upou.edu.ph and request the transaction link. Indicate your name, program, purpose of payment and c. Through the Mail-Order-Telephone-Order (MOTO) system (If payment gateway is unavailable): Download the credit card authorization form from this link, http://www.upou.edu.ph/downloads/credit_card_payment.pdf. Scan the duly accomplished authorization form and mail, or email it to the following: 1. If the student opts to pay through credit card or MOTO and ecommerce/payment gateway, he/she shall be charged an additional fee for the use of bank facilities and as administrative cost. The additional fee shall be a percentage of the total fees due and shall be indicated in the admission pack given to the student upon enrollment and also posted in the UPOU website.

III. Cash Payment at the Cash Office, UPOU Headquarter, Los Baos, Laguna
a. Pay at the Cash Office, located at the second floor of the UPOU Headquarter in Los Baos, Laguna, Philippines. b. Office time is from 8:00-11:30 AM and 1:30-5:00 PM, every Tuesday through Friday.

What is the UP Open University?


The UP Open University or UPOU is the fifth autonomous university of the UP System. It was established through in 1995 to provide wider access to quality higher education through distance education. The university envisions to develop in students the discipline and capability to become lifelong learners who are at home in today's knowledge society. The UPOU has been declared by the Commission on Higher Education (CHEd) as the National Center of Excellence in Open Learning and Distance Education in recognition of its achievements in this field and its vital role in pushing the frontiers of learning in service to the Filipino nation.

What is an open university?


An open university espouses a philosophy of open learning which differs in degree and dimension from institution to institution. For the UPOU, its philosophy of open learning is evolving. Its emphasis at present is on widening access to quality higher education.

What is distance education?


Distance education is any of the various forms of study which is not under the continuous, immediate supervision of teachers present with their students in lecture/classrooms. It favors an independent self-learning style which takes place outside a formal classroom. Despite the physical separation between the learner and the teacher, this does not mean that students are left on their own. Teaching and learning is mediated through the use of technology like print, audio, video, and the internet.

What are the advantages of distance education?


Studying through the distance education mode offers the following benefits:

No need to take leave from ones job No need to be separated from ones family Can immediately apply what has been learned to ones work Can save time and money Access to institutionally developed learning modules

How is distance education different from the traditional mode of learning?


UPOU students are expected to perform the usual learning activities and course requirements performed by students in traditional classes. Unlike in face-to-face classes, the course materials are usually the starting point for study. Course materials may come in the form of print modules, online articles and exercises, videos, and audio recordings. Interaction between the teacher and students also takes place through the internet.

How do students and teachers interact with each other?


If traditional universities have classrooms and lecture halls, UPOU has course sites or virtual classrooms where students and teachers log in to discuss the lessons as well as do other learning activities.

Academic interactions, submission of course requirements, online quizzes, chats and other online activities can be done in the course site.

Can I enroll in UPOU even if I am based abroad?


Almost all courses are taught online, which means that you can study any course wherever you are. Around 15 per cent of UPOU students are based abroad. Some subjects require pre-scheduled face-to-face meetings (e.g., laboratory work or field practice).

Do the students have to take examinations?


Yes, students normally need to take examinations. The students sit for supervised examinations through either of these means:

at a UPOU Learning Center or Testing Center. at a Philippine embassy/consulate or any academic institution/credible organization abroad under the supervision of a qualified proctor in the area. Online, via the internet and using a webcam.

Scholarship / Study Grant


Email

A limited number of scholarship grants and other forms of financial assistance (e.g., student loan) is available to qualified students. Grantees are entitled to 100% tuition fee subsidy, as well as a waiver of the instructional materials fee, laboratory fees, registration fee, entrance fee and deposit (for new students). Some grants provide a minimal stipend for attendance in face-to-face study sessions. The assistance is made possible through the generosity of donors and institutions that have joined UPOU in providing better opportunities for education.

Criteria in Awarding Scholarship / Study Grant


All scholarship funds deposited with the UPOU shall be governed by the following rules: An applicant for scholarship / study grant must: 1. Have been admitted into a program that is stipulated in the grant. 2. Fulfill the specific requirements/conditions of a particular grant. 3. Meet a general weighted average (GWA) of 2.0 or better for a new student, or have passed all courses taken during the previous semester for a continuing student. 4. Have an annual income of not more than P300,000 (Gross, Personal) and not more than P600,000 (Gross, Family for independent) 5. Be willing to abide by the terms and conditions of the contract.

Duration of the scholarship / study grant


The grant is renewable every semester until grantee completes the program provided requirements for retention of the support are satisfied.

Benefits provided by the scholarship / study grant


All grants provide free actual matriculation fees. Some grants provide a minimal stipend for attendance to study sessions.

Any UPOU student may submit application for scholarship.

How to apply for scholarship?


Step 1. Filling out of application form Scholarship application forms are available at UPOU Learning Centers and downloadable at Online Forms. Step 2. Submission of documents Submit the following application requirements for scholarship on or before the deadline of submission of application for every semester: accomplished application form photocopy of recent income tax return Certification of employment and permit to study from your principal/ supervisor or head/president (Form 2) How to Submit: 1. Email scanned copies to: scholarships@upou.edu.ph 2. Mail to: Office of the Vice Chancellor for Academic Affairs (OVCAA) UP Open University Los Baos, Laguna 4031 3. Bring to: any UPOU Learning Center near you. Step 3. Evaluation of applications The Scholarship Committee evaluates all applications. An applicant should first be admitted to a degree program before the application for scholarship / study grant is evaluated.(see Criteria in Awarding Financial Assistance) Step 4. Results of application The applicants will be notified of the official result of their application through postal mail and email. A list of qualified applicants is also sent to the Learning Centers. Step 5. Signing of contract Qualified applicants are required to accomplish contracts for scholarship / study grant. The UPOU has 10 Learning Centers in strategic locations in the country. These Centers are the venues where students can conduct and seek advice on the following transactions, activities and procedures:
