Concerns Expressed by Parents on Childs Current Behavior: Gelo undergone O.T for less than a year. III. MEDICAL HISTORY / REPORT (include the services your client receives from you like tutorial, occupational therapy, shadow, etc.) He has complete history of immunization. He undergone occupational therapy at health center located in Guadalupe for less than a year. IV. BEHAVIORAL REPORT (home, school, environment), (good & bad behavior, what triggered the behavior), (reinforcers / hobby) Home: He tends to room around go up and down to stairs. He knew where to find his plates, glass and spoons . School: He is very cooperative. He tends to be self injurious. Reinforcers/hobby: He love to pick something from floors and bounce them. ASSESSMENT Performance on the different scales are as follows:

Physical Health: Gelo is physically healthy. Gross Motor : He can jump. He can skip. He can crawl. He can clap his hands. Fine Motor Skills: He can hold pencils. He can crayon. He can pick mongo beans. Language Communication: He cans hymn. He can say e and a. Writing: He can do scribbling. Reading: N/A Self-help: Eating Dressing Grooming : can eat using spoon. : can put and remove his short pants. : can wash his hands using soap with minimal prompt.

SUMMARY & LATEST DEVELOPMENT Gelo is very cooperative and active in terms of the task we give to him. Even though he cried sometimes he still manages to finish his tasks. there is regression on his development due to frequent absences.

VII. RECOMMENDATIONS Gelo need to have undergo occupational and speech therapy. Parents should be involve in Gelos activities and should be aware for his needs and conditions.
