Introduction
This document provides a step-by-step guide for selecting a medical plan of your choice via the staffs health portal, My Health Wallet.

How to Login
Enter the following address in your browser: https://www.myhealthwallet.com The following page will appear.

Enter your Login ID and Password then click [Go]. If you had entered a wrong ID or Password, the following page will be shown. You will need to re-enter your ID and Password.

Please drop an email with your name, NRIC number & Company name to SingTelBenefits@parkwayshenton.com if you have forgotten your ID and/or password. Once you have successfully logged in, a welcome page will be shown. The various available options are shown on the navigation bar. This welcome page also contains relevant information related to your Flexible Medical Plan (e.g. Flexi Medical Plan Brochure etc).

How to Select Your Flexi-Medical Plan


Once you have successfully logged in, click on [Flexi-Medical Plan Enrolment] link on the navigation bar. The enrolment form which is divided into the following 5 sections will be displayed: 1. 2. 3. 4. 5. Selection of life(s) to be covered In this page you can choose which of your dependants (spouse and children) you want to cover. a) Select the dependants you want to cover by checking the relevant check boxes. Proceed to the next step by clicking on the [Continue] button. Please note the following:

Only dependants whose records are registered in Shentons database will be shown. It is compulsory for you as a SingTel staff to be covered. Therefore, the system does not allow you to remove yourself from the selection. If you have registered children in Shentons database who are between the age of 19 and 25 years (age next birthday as at 1 Apr 2013); you can select individually who you want to cover. Please note that you will have to bear the full cost via salary deduction. If you joined after 1 October 2006, you are required to bear the cost for your dependants should you choose to cover them. HSA and Flex points are not allocated to your dependants.

2. Selection of Plans The enrolment form for the selection of plan is divided into the following sections. About You This section displays information pertaining to your particulars, such as Name, NRIC, Staff Category, etc.

Please verify your details. If the details are incorrect, please login to SingTel intranet (PeopleConnect) to update or amend. To change / amend your email and contact number, enter the information in the Email and Contact Number textbox provided.

About Your Covered Dependant The details of the dependants for which you have selected to cover will be shown.

Points Allocated The amount of Health Spending Account (HSA) and Flexi points (if applicable) allocated to you and your selected dependant(s) will be shown.

Medical Outpatient Plans The list of medical outpatient plans available to you and their respective price tags are shown. For staff / dependant not certified with chronic conditions Staff (and staff with dependants who are not certified with chronic conditions) will have a choice of 4 outpatient plans. In the first year, the default outpatient plan is Plan 3. For the second year, it will be defaulted to your selected plan. You can select another plan that best suits your needs by clicking on the corresponding radio button.

Please note that for subsequent years, the default plan will be based on the previous years selection.

For staff / dependant with chronic conditions Staff and dependants who are certified with chronic conditions on or before 11 August 2006 will be automatically placed on Plan 5 while the non-chronic family member(s) will be defaulted to Plan 4.

Medical Inpatient Plans The list of medical inpatient plans available to you and their respective price tag per life are shown. The default plan is based on your job grade and will be automatically selected by the system. You can select another plan that best suits your needs by clicking on the corresponding radio button to make your selection.

For subsequent Flexi-Medical Plan Years, the default plan will be based on the previous years selection.

Your Selections This section shows the selections you have made and the corresponding price tags to you and your dependants. The sub totals of flex points spent on each option are also shown.

Summary A summary of the following are shown in this section: a) Total Flex Points Allocated This is the total flex points allocated to you b) Flex Points spent on Medical Outpatient Plan This is the total flex points spent on your selected outpatient plans. c) Flex Points spent on Medical Inpatient Plan This is the total flex points spent your the inpatient plans. d) Flex Points balance It shows the balance of your allocated flex points resulting from your spending on inpatient and outpatient plans. e) Amount deductible from payroll This shows the amount that will be deductible from your monthly salary via payroll deductions. This will occur should your flex balance be negative (as a result of your spending on inpatient and outpatient plans). f) Total amount deductible from payroll This shows the total amount deductible from payroll resulting from (e). Note that the actual amount deducted from your payroll will include GST.

Important point to note: Please read the fine print instructions carefully before making your selection as changes will not be allowed after clicking the [Confirm] button. You can make amendments by clicking on the [Back] button to go back to the enrolment page and make the necessary changes. Should you be satisfied with your selection, please click the [Confirm] button. Please note the following:

Upon confirmation, your selection will be submitted and no change will be allowed. Based on your selections, you may be required to submit Health Declaration Form. You must acknowledge that you will print, complete and submit the Health Declaration (HD) Form by checking on the checkbox provided. The duly completed HD Form must be dropped off at the designated collection point by 1 March 2013, failing which your plan options will be set to default. Should you exceed your allocated Flex Points, you are required to acknowledge and authorize any deductions from your salary via the payroll when applicable.

4. Flexi-Medical Plan Enrolment Acknowledgment Page Once your confirmation has been processed successfully, the Enrolment Acknowledgement Page will be shown.

5. Print Health Declaration (HD) Form Depending on your selections, you may be required to submit Health Declaration (HD) Form. The conditions are: Existing Employee

How to View Your Flexible Medical Plan Summary


After you have submitted Flexi-Medical Plan Enrolment, you can check the status of the enrolment in Flexi-Medical Plan Summary page. Choose the link on the navigation bar to open the page.

Pending status: Enrolment is being processed. Final status: Enrolment is finalized. The final plan will be the one that you have enrolled, if approved; or the default plan if your selections were rejected.

Notice that once the enrolment status is Final, the actual amount deducted from your payroll, including GST will be shown.

New Hires
For new hires, the following processes apply: a) You would have to go for Pre-Employment Examination (PEE) prior to making your selection. Otherwise you will not be allowed to proceed with your enrolment. Submit your dependants data, including spouse and children if any, to your respective HR Managers. Your benefits selection can commence once (a) and (b) are completed Please take note of the following:

Your PEE results will be used for underwriting. Once your record has been activated, you will receive your User ID and Password for MyHealthWallet within a week by memo / email. You will have 2 weeks to submit your SingTel Flexi Enrolment (see Section 3 How to select your medical plan).

New Dependant Enrolment


You can cover your new dependants (i.e. your spouse if you are just married, and your new born child) under the current Flexi-Medical Plan you have enrolled earlier. Follow the steps below: 1. Update your dependants data at the SingTel intranet (PeopleConnect) and scan your marriage/child birth certificate to HR Benefit Unit or submit to HR (Rewards & Benefits), Comcentre #25-00. 2. Once the data is in the system, a reminder will be sent to you to make your decision whether or not to cover your dependant. 3. You can submit your decision by clicking Flexi-Medical Plan New Dependant Enrolment link on the menu bar. The page will be displayed for you to indicate your decision.

If you chose to cover your dependant, you are required to submit Health Declaration (HD) Form. You must acknowledge that you will print, complete and submit the Health Declaration (HD) Form by checking on the provided checkbox.

To continue, click on the [Confirm] button to proceed. Once your confirmation has been processed successfully, the Acknowledgement Page is shown.

Please observe the following conditions for the new dependant coverage: During the Plan Year New Spouse or New Born Can your dependant be covered? Yes.

Existing Spouse or Children, No. not covered in the previous They will only be eligible to be selection period covered in the next selection period.
