Designer Hazard Register - Instructions to Design Teams

1. Please review the items on the Hazard Register especially those which you have been assigned ownership. To list only the items wher you have been assigned ownership, pick your intials from the 'Ownership' drop down box. See below for screen shot of this operation:

(Note: Please inform the CDM coordinator if you believe you have been wrongly assigned ownership) 2. If you wish to make amendments or additional comments please do this in red text so that the CDM coordinator can see which comments have been made. Once you are happy with your comments, please save the spreadsheet and e-mail it to the CDM Coordinator. If you wish to insert a new line in any particular section, right click on the grey number to the left of the blank line in that section and click insert' from the drop down menu. See below for a screen shot of this operation:

Note to the Project Manager - outstanding issues on the Hazard Register should be reviewed at each Design Team Meeting, led by the Project Manager (or CDM Coordinator if present). If a computer is not available at the meeting, please make handwritten comments/amendments at the meeting and issue to the CDM Coordinator Closing out Hazards - the obvious aim of the register is to ensure that all items are closed out. When an item is deemed to be closed out, some form of evidence is required e.g. Minutes of a meeting, Notes of Discussion, Designer Risk Assessment, Drawing Notes etc
