Symptomsusuallyresolvethemselvesspontaneouslywithinseveraldaystoseveralweeksbut mostpatientdeveloprecurrentepisodes.Treatmentwillusuallyaimatrelievingsymptoms, whileallowingspontaneousrecovery.Amoreaggressiveintervention,includingsurgery,is requiredincaseofseverepain,severebleeding(morearesultofafissure)orhemorrhoidsthat donotresolve. Thehemorrhoidalveinsprovideapathwayofcommunicationbetweenthesystemicvenous systemandtheportalvenoussystem.Theseveins,whichduetotheirlocationgounder tremendouspressuresandstrains,haveatendencytodilateanddevelopintoatwistedand turnedplexus.Theanalcushionsareapartofthenormalanatomyoftheanalcanal.These cushions,containingthehemorrhoidalveins,thearterialplexuses,smoothmusclesand connectivetissue,permitthepassageofvariablesizesofstools,withoutdisruptionoftheof therectalmucosa.Hemorrhoidaldiseaseisthoughttobearesultofthedisplacementofoneor moreofthreevascularanalcushions,mainly:therightanteriorcushion,therightposterior cushionandtheleftlateralportionoftheanalcanal.thereareexternalandinternal hemorrhoids: externalhemorrhoidsarisefromtheinferiorhemorrhoidalplexus,exteriortotheanalverge. Theyarecoveredbyalayerofskin,whichisverysensitivetopain.Thrombosisinthis structuremaycauseaseverepain.
