student survey questions:

Which school activities (non-athletic) have you participated in an enjoyed? Describe and indicate dates.

Did you have a signficant role or leadership position in those activities? If so, describe.

Other out-of-school activities (e.g. community service, church activities, scouts, performance arts, tutoring,e tc.)? Indicate where and when.

Which sports have you played (including out-of-school)? Indicate what, where, when, and any leadership position you may have held.

Work experience (paid and volunteer)? What, where, and when?

Special camps or programs? Describe and indicate when.

Have you earned any awards (e.g. Rachel Austin, departmental, athletic, community service, employee-of-the-month, scouting)? When?

What are your favorite subjects in school and why?

What are your least favorite subjects and why?

Do you have any hobbies, special interests, or special talents not mentioned above?

Do you have any career interests at this time? If so, what are they?

What are your entertainment favorites (e.g. music, books, movies, plays, etc.)?

List your three most distinguishing, most admirable qualities. Explain each with several sentences, including examples. Think about those aspects of yourself that you would like your recommendation writer to emphasize.

Describe the single academic accomplishment (e.g. major paper, science project or experiment, artistic project or accomplishment) of which you are most proud and tell why you take special pride in it.

What do you choose to learn when you can learn on your own?

List the books you have read on your own in the past two years or so.

Has any summer activity, work, experience, or study been of significance to you? If so, describe why.

Have you traveled or lived in different localities? Where? Comment on signficant experiences there?

Have you had an especially stimulating intellectual experience in recent years? What was it?

If you could spend a day doing whatever you wanted, whatever the cost, what would you do and why?

Is your high school academic record an accurate measure of your ability and potential? If not, what do you consider the best measure of your potential for success in college?

What circumstances, if any, have interfered with your academic performance (e.g. learning challenges)?

Is there any other information you would like to share so that your recommendation writer can make an accurate appraisal of you to colleges and universities? Here you may write of such things as cultural background, family crisises, etc. that have had a significant impact on your own interests and concerns.

Is it okay for your TA to share with colleges your answers to the previous 3 questions?

Will you take or have you taken any AP test(s) before the end of junior year? If yes, which and what was your score, if applicable.

What do you consider to be the outstanding accomplishments of your child during the past three or four years? Why did you select these as most important? If you had to describe your son or daughter in six adjectives, what would they be? Please elaborate on the previous question by telling a story of how your child demonstrated at least one, preferably, more of these adjectives In what areas has your child shown the most development and growth during the past three or four years? Are there any unusual circumstances that have affected your child's educational or personal experiences?
