LOCAL CHAPTER MEMBERSHIP REQUIREMENTS

General Reminders: 1. All local chapters shall submit the followring to the Regional Council: For the first time applicants: Local Chapter Constitution and By-laws. For renewing members: Amendments or Revisions of the Local Chapter Constitution and By-Laws. Annual Report of previous federation. Local Chapter Requirements (See other sheets) *Local Chapter Information *Local Chapter Officers *Local Chapter Calendar of Activities *List of Members First Year Second Year Third Year Fourth Year Fifth Year 2. The filename to be used shall be <Local Chapter>-Requirements.xlsx. Use the given template for unifomity purposes. Example: University of San Jose-Recoletos University of San Jose-Recoletos-Requirements.xlsx 3. For the JPIA Number Code shall follow the format below: 2013-07-XX-YYY *2013 Federation Year *07 Region Code for Region 7 *XX Local Chapter Number *YYYY Member Code Example: University of San Jose-Recoletos 2013-07-02-0001 REGION COUNCIL 7 Local Chapters Codes University of San Carlos 1 University of San Jose-Recoletos 2 University of Southern Philippines Foundation 3 University of Bohol 4 Cebu Institute of Technology University 5 University of Cebu-Banilad 6 University of Cebu-Main 7 4. For the renewing and the new member local chapters, the Regional Vice President fo Membership shall remain the discretio in providing LC Codes for their respective local chapters in their regions. The Regiona Vice President for Membership will just inform the local chapters for their local codes. 5. Certification from the school official declaring that the school duly recognizes the organization. 6. Resume of Local Chapter Officers and all graduating students. (5th year students for 5-year program, while 4th year students for 4-year program). Please use the prescribed resume format. 7. All the requirements are soft copies and will be place in a folder. The filename of the folder will be Regional Council-Local Chapter Name. Example: University of San Jose-Recoletos Region 7-University of San Jose Recoletos REGION COUNCIL Region 1 & CAR Region 2 Region 3

Region 4 Region 5 Region 6 Region 7 Region 8 Region 9 Region 10 & CARAGA Region 11 Region 12 NCR Deadline of Submission and Payment 1. The deadline of submission of membership requirements including the payment of membership fee is on July 25, 2013 (Thursday). The Council deems it imperative to waive the rights of membership if no payment has been deposited on or before the given deadline. 2. All soft copy requirements shall be sent to the official e-mail address of your Regional Vice President for Membership. All hard copy requirements shall be placed inside a long brown envelope and shall be submitted to the Regional Vice President for Membership via mail. 3. For those Local Chapters who will be submitting the COMPLETE documentary requirement and the 100% payment of the membership fee on or before July 25, 2013 will have an additional points for the Most Outstanding Local Chapter. 4. Late compliance of the membership requirements will receive a penalty of 5% interest in their total national membership fee that will automatically go to the socio-civic activities of NFJPIA. Payment for membership fee Deposit in Bank On or before July 25, 2013, 75 % of the whole due amount should be submitted. There will be a grace period of 1 month after July 25, 2013 to pay the remaining 25 %. For queries and suggestions regarding membership matters, kindly contact Neil Martin E. Pilapil, National Vice President for Membership either through e-mail, nfjpia.vpmembership1314@gmail.com, or through his mobile number, 0922-688-7360. Confirmation email or message shall be sent to respective regional councils regarding the verification of the files sent and the Membership Committee, headed by the National Vice for Membership shall confirm the files and documents submitted. If the regional council did not receive any confirmation regarding the submission of files, it means that the National Federation did not receive any file from the respective regional council.

If the local chapter did not receive any confirmation regarding the submission of files, it means that the Regional Council did not receive any file from the respective local chapter.

All local chapters are required to follow the prescribed format of the Council. All regional council are required to follow the prescribed format of the National Federation. This is to keep a well-track record. Templates and prescribe formats shall be available in the social networking sites for reference. Additional Reminders for the use of Application Forms: The official font style to be used is Cambria and the official font size to be used is 12. Always wrap text any entries that may not fit in the boxes provided.
