Detailed instructions on what you need to do to take advantage of this program are set forth on the enclosed document entitled Complete Your Checklist. Generally, you will need to: Explain the financial hardship that makes it difficult for you to pay your mortgage loan using the Request for Mortgage Assistance (RMA, enclosed). Submit the required documentation of your income. Make timely monthly trial period payments.
If you meet the eligibility criteria, you will be offered a Trial Period Plan. The monthly trial period payments will be based on the income documentation that you provide. They will be an estimate of what your payment will be if we are able to modify your loan under the terms of the program. If you do not qualify for a loan modification, we will work with you to explore other options available to help you keep your home or ease your transition to a new home. Be sure you continue making your regular mortgage payments until you hear from us about your eligibility for this program. When we receive your completed documents and verify your information, well determine if you qualify for a Home Affordable Modification of your loan. During this review period, your home will not be referred to foreclosure or sold at a foreclosure sale.

1. The enclosed Request for Mortgage Assistance completed and signed by all borrowers (no notary required), Important note: You will need to provide information about other properties you own (if applicable), including property address and loan information. If you are requesting mortgage assistance for a property that is not your principal residence, you will also need to provide information about your taxes, insurance, homeowners association fees, and occupancy status. In addition, you will need to read and initial the Rental Property Certification. 2. A signed and dated copy of the IRS Form 4506-T (Request for Transcript of Tax Return) for each borrower (borrowers who filed their tax returns jointly may send in one IRS Form 4506-T signed and dated by both of the joint filers). 3. Proof of i ncome doc umentation to v erify a ll income of eac h bor rower. F or e asy r eference, t his i nformation is separated into five sections: A. Gross monthly income B. Self-employed borrower income/business income C. Retirement/pension/unemployment and disability income D. Income from tenants E. Other income Please note: Income must be documented as required. Include combined income and expenses from the borrower and co-borrower (if any). If you include income and expenses from a household member who is not a borrower, please specify using the back of this form if necessary. Additional income documentation may or may not be required. Gross monthly income For wage earners/salary: If homeowner(s) are wage earners, the following proof of income, including year-to-date information, must be provided, such as most recent paystubs: 30 days of wages: If paid weekly send four paystubs If paid biweekly send three paystubs If paid semi-monthly send two paystubs If paid monthly send one paystub If unable to provide paystubs, salary vouchers with the dates of payment must be dated within the last 30 days, including year-to-date information and the borrowers name and/or Social Security number. If unable to provide paystubs and salary vouchers (e.g. you are paid in cash), please send a letter from the employer with pay frequency and amount. This letter must be on company letterhead, signed and dated within the last 30 days and include year-to-date information, the borrowers name and/or Social Security number. The most recent tax returns along with a copy of filed IRS extension must be provided.
Other Earned Income such as bonuses, commissions, housing allowance, tips, or overtime: Reliable
third-party documentation describing the amount and nature of the income (e.g., employment contract or printouts documenting tip income).
IRA/401K/investment income:
Most recent two months account statement AND at least ONE of the following: Borrowers most recent year of tax returns (1040) including all schedules or most recent 1099 forms Retirement or pension: Award letter for current year unless it states lifetime and three months bank statements Unemployment income and short-term disability income: Award letter within 60 days AND three months recent bank statements or cancelled checks. Social Security, disability or death benefits, pension, public assistance, or adoption assistance: Documentation showing the amount and frequency of the benefits, such as letters, exhibits, disability policy or benefits statement from the provider, AND Documentation showing the receipt of payment, such as copies of the two most recent bank statements showing deposit amounts. D. Income from tenants
Rental/boarder income:
Copy of the most recent filed federal tax return with all schedules; including Schedule E Supplemental Income and Loss. Rental income for qualifying purposes will be 75% of the gross rent reduced by the monthly debt service on the property if applicable; OR If rental income is not reported on Schedule E Supplemental Income and Loss, provide a copy of the current lease agreement with either bank statements (demonstrating receipt of rent) or evidence of damage deposits. If boarder income is also included: Proof of occupancy (i.e.cell phone or utility bill, bank statements or other evidence of occupancy from boarder).
E. Other Income Alimony, child support, or separation maintenance payments as qualifying income:* Copy of divorce decree, separation agreement, or other written legal agreement filed with a court, or court decree that states the amount of the alimony, child support, or separation maintenance payments and the period of time over which the payments will be received, AND Copies of your two most recent bank statements or other third-party documents showing receipt of payment. *Notice: Alimony, child support, or separation maintenance income need not be revealed if you do not choose to have it considered for repaying this loan. Welfare: If welfare is listed as income, need a letter from the County.
Non-borrower household income: The income of a non-borrower who contributes to the mortgage loan payment and is included in the monthly gross income must be documented and verified using the same standards used for verifying a borrowers income, including a completed and signed 4506-T. Proof of occupancy (i.e., cell phone or utility bill, bank statements or other evidence of occupancy from contributor), Contribution letter stating contributors name, relationship, frequency, and amount of contribution, signed and dated by contributor. 4. The enclosed Non-Borrower Occupant Certification Form must be completed, signed and dated by each individual who lives at your property address whose income will be included in the review of your loan modification program, but is not on the loan as a borrower, so that we can proceed with your loan modification process. 5. We will also need the following information to complete this analysis: For homeowner insurance review and setup of Escrow account: Copy of homeowner insurance declaration page showing the annual premium amount and start and end dates of the coverage. Copy of flood insurance declaration page showing the annual premium amount and start and end dates of the coverage, if applicable. Proof of Homeowners Association Dues showing amount and frequency of dues, if applicable. If you have other types of income, cannot locate required documents, or have questions about the documentation required, please contact us at 1.888.565.1422. Please send in all required income documentation, tax returns, completed Form 4506-T, signed RMA, and the Non-Borrower Occupant Certification Form. Keep a copy of all documents for your records. Dont send original income documentation as copies are acceptable.
Please read this section carefully to understand what you can expect from this process. A. Once we receive all of your documentation and verify your information, we will determine whether you qualify for a Home Affordable Modification of your loan. If you do, we will send you a Home Affordable Modification Trial Period Plan Notice which will outline your next steps and obligations for the trial. B. Under the Trial Period Plan, you will be required to make trial period payments, instead of your regular mortgage loan payments. The trial period payments should be close to the amount you would pay under a modification. Near the end of the trial period, we will be able to calculate the final amount and the final terms of your modified loan. Then we will send you two copies of the Home Affordable Modification Agreement for your signature. C. In addition, to successfully completing the Trial Period Plan, you will need to sign and return both copies of the Modification Agreement to us promptly. The Modification Agreement will only become effective after we sign it and return it to you with all signatures. Once the Modification Agreement becomes effective, we will modify your loan. D. Please note that it may take up to 30 days for us to review your documents, after we receive all required documentation. During this review period, your home will not be referred to foreclosure or sold at a foreclosure sale. However, if you fail to comply with the terms of the Trial Period Plan and do not make other arrangements with us, your loan will be enforced according to its original terms. This could include foreclosure. What you need to know about foreclosure We will continue to work with you to help you avoid a foreclosure sale. However, please understand that if you have not yet been referred to foreclosure, that process could begin at any time. If your mortgage has been referred to foreclosure, as part of the foreclosure process, you may receive notices from a third-party attorney delivered by mail, or see steps being taken to proceed with a foreclosure sale of your home. During the HAMP trial period, a foreclosure sale will not be held as long as you comply with the terms of the trial period plan and make all your payments on time. E. I will contact you if you do not qualify for this program. If you do not qualify, I will want to discuss other alternatives with you that may help you keep your home or ease your transition to another home.
NO FEES. There are no fees under the Home Affordable Modification Program. TRIAL PERIOD PLAN/MODIFICATION PROCESS. Submitting all required documentation to be considered for the program is the first step. If you are eligible for the program, you will need to successfully complete a trial period by making trial period payments. Once we are able to finalize your modified loan terms near the end of this trial period, we will send you a loan modification agreement ("Modification Agreement"), which will reflect the terms of your modified loan. In addition to successfully completing the trial period, you will need to sign and promptly return to us both copies of the Modification Agreement or your loan can not be modified. NEW PRINCIPAL BALANCE. Any past due amounts as of the end of the trial period, including unpaid interest, real estate taxes, insurance premiums, and certain assessments paid on your behalf to a third party will be added to your mortgage loan balance (the "Past Due Arrearage Amount"). If you fulfill the terms of the trial period including, but not limited to, making the trial period payments, we will waive ALL unpaid late charges at the end of the trial period. ESTIMATED MONTHLY PAYMENT. At this time, we are not able to calculate precisely the Past Due Arrearage Amount or the amount of the modified loan payment that will be due after successful completion of the trial period. As we near the end of the trial period, we will calculate any past due amount to determine your new permanent monthly payment and other modified loan terms. ESCROW ACCOUNT. The terms of your Trial Payment Plan and your Modification Agreement will require us to set aside a portion of your new monthly payment in an escrow account for payment of your property taxes, insurance premiums and other required fees. Your current loan may also require escrows. If it does not, the previous waiver of escrows is cancelled under your Trial Period Plan. We will draw on this account to pay your real estate taxes and insurance premiums as they come due. Please note that your escrow payment amount will adjust if your taxes, insurance premiums and/or assessment amounts change, so the amount of your monthly payment that we must place in escrow will also adjust as permitted by law. This means that your monthly payment may change. BORROWER INCENTIVE. Based on Home Affordable Modification Program requirements, your loan may qualify for borrower incentives. CREDIT COUNSELING. If you have very high levels of debt, you will be required to obtain credit counseling under the Home Affordable Modification Program. CREDIT REPORTING. When you become past due on your mortgage payments, it's important to understand that the actions you take to correct the situation can affect your credit score. If you choose a loan modification, your credit score could be negatively impacted during the trial period plan, but it may not be as impacted as an ongoing series of late payments or foreclosure. During that process, we are required to continue to report the delinquency status of your loan to the credit reporting agencies as "paying under a partial or modified payment agreement". When you are approved for a permanent loan modification, the modification process is complete and is recorded on your credit report as "loan modified" as long you make your permanently modified payments on time. Keep in mind that the degree of impact to your credit score depends on your overall credit circumstances and is different for each individual. Dodd-Frank Certification
