ASF need to do something about this. TET need to support by issue the designed smoking area at lay down area to IRPC for approval. For the next future the man power will be up to 500. The designed smoking area at EBSM plant is not enough for all contractors.

* K.Somchai- IRPC Project Manager will discuss about this celebration with all contractors site managements on Wednesday April 24,2013. 1.4 K.Pongsapak requested PTEC to provide project information board at Lay Down Area. 1.5 K.Pongsapak reminded about HSE booklet/handbook. - All to provide ( ABB, MACS, TCC, PTEC & Dialog) 1.6 TET informed that evacuation drill #04 will be conducted at lay down area. Today TET conducted evacuation drill at lay down area the detail & problem is in summary evacuation drill #04 that TET will submit later 1.7 IRPC(K.Pongsapak) requested Ensure to submit the official explanation letter of unplanned lifting method of crane 400 ton by lifted the reactor weight172 ton hang over common EBSM pipe rack about 2 hours (10.00 12.00 am) on Tuesday March 19,2013. This was very UNSAFE action & condition of this event and not mentioned in what if, lifting plan & brief before lifting. 1.8 TET requested all to submit safety & fire watch man document ( Appointment letter) 1.9 TET requested all to prepare scaffolding log book. 1.10 TET informed that about induction training TET will provide items to safety officer professional level and observe. 1.11 TET Leadership Site visit the result as below - K.Waritnan (BD Manager) Opening floor - K.Hua Ming Soo (Engineering Manger) * Lifting work need 2 taglines ENSURE * Barricade some area need to be replaced. April 19, 2013. 1.12 TET will set schedule for training safety personnel and fire watch warden soon. This issue will be a topic in HSE Monthly Action Plan for May 2013 1.13 Hot work must be protected properly or else the fire watch warden will be rejected from site. 1.14 HOUSEKEEPING TEAM will be scheduled every Tuesday and Friday starting 13.30 16.00 pm the number to provide as above. 1.15 TET waiting detail of lesson learn - IRPC about electrical shock on scaffolding so all need to be careful about avoid hanging electrical cable on scaffolding or else use S hook with the effective insulator. 1.16 TET launched EBSM Contractor Evaluation for 500,000 MH celebrations by using IRPC criteria. The detail will be informed later.

General / Information 3.1 K.Ponsapak informed about IRPC management will come on site (April 12,2013) 3.2 K.Pongsapak reminded about hot work preparation and no blue sheet on site. 3.3 TET reminded to all about communication on radio. It should be exactly information and not panic to all employees 3.4 TET reminded about electrical extension box should not be overload of capacity and electrical cable need to be safely hung. 3.5 TET reminded about safety latch of lifting tools should be in good condition. 3.6 MACS reminded to all to avoid MACS working area (Furnace) as of scaffolding work and risk of falling object however MACS already provide safety sign and barricade. 3.7 Ensure informed about additional resting area(Trim cooler area) in lay down area. 3.8 Ensure requested all to support about hearing conservation in EBSM plant as of all plant full of bolt tightening. 3.9 TET informs next week HSE meeting will be cancelled. 3.10 TET informed about security officer start working in lay down area.
