Where did you hear about the CSA? ____________________________________________________ MARK YOUR SELECTION(S): Small Share $300 Designed for a couple, a couple friends, or as a sampler for a family, this size of share is great for those wanting to see what this seasonal eating is all about! You'll receive at least 4 items weekly, most of which will be familiar vegetables. Standard Share $450 Designed for a family of 2-4, depending on your vegetable consumption. This share will focus on basic vegetables, often with a twist. Expect staples such as potatoes and tomatoes, but look forward to new tastes, colors and textures of these old favorites. Veggie Lover Share $600 Enough for a veggie-lovin' family of 4 or more, or for two families to split. (You are responsible for splitting your own box.) The Veggie Lover Share will also have greater variety, with less common vegetables such as fennel, Asian greens, and a variety of fresh herbs. Egg Add-on $40 Receive a dozen local eggs every other week, for a total of 10 dozen. Fruit Add-on $60 New in 2013: Receive locally-grown fruit in addition to your veggies. We plan to have seven to ten weeks of fruit this season. Only 25 shares available. Share-a-Share Donation Circle amount $25 $50 $100 $200 $300 (entire small share) Other $_______ Provide a low-income family in our community with fresh, healthy produce all summer long. PICKUP DETAILS: Market-Style CSAPack Your Own We distribute the CSA shares at one central location every Thursday. In this market-style technique, members pack their own boxes or bags from our pre-determined list of produce. Pick up your vegetables between 3:306:30 pm at the parking lot of the Westminster Presbyterian Church (27th and South). CSA members receive shares for 20 weeks. Including two scheduled breaks, the CSA will run from May 23 to October 17. More CSA details can be found on our website: www.communitycrops.org/csa. PAYMENT OPTIONS: Option 1 Pay the full balance for your share and add-ons. Make checks payable to Community CROPS. Option 2 Submit the Scholarship Application with this Registration Form to be considered for our Share-aShare program. Scholarship availability is determined by donations from the community. We will contact you by May 1 to notify you of your approval and set up a payment plan. We accept SNAP! Work-Share Discounts are available through our work-share program. Contact us if you are interested in participating as a work-share volunteer for the 2013 CSA. Volunteers must commit to 3 hours/week, working on one of the following: member communication, distribution site assistance, harvesting, events, or organizational tasks. BRANCHED OAK CHEESE ADD-ON: Branched Oak Farm, located outside of Raymond, NE, offers a cheese share add-on to pick up with your vegetables. Krista Dittman, owner and operator, along with her husband Doug, will be handling the majority of communication with those interested in this add-on. Farmer-Member Agreement By joining the CROPS CSA, I understand I am sharing in both the rewards and risks of supporting beginning farmers. I understand that there are no refunds available in the case of crop failures or adverse weather, nor is there any guarantee of varieties or quantities of crops. I acknowledge that I am responsible for picking up my share each week, or sending someone else to do so,* and that if I miss my pickup, my share will be donated that week.
