Guidelines for Financial Awards

The goals of The Karen M. Oldham Foundation in providing financial awards include: To support resource families by assisting in addressing financial needs of either the family or the foster/adopted child/children within the family To contribute to the well-being of resource families through interventions funded by the Foundation. (Please read carefully)
Financial assistance provided by The Karen M. Oldham Foundation is made possible because of generous donors and fundraising activities. It is important that these funds be available for families experiencing the greatest financial need. To apply for financial assistance, please complete the attached application. The Karen M. Oldham Foundation staff will verify receipt of your application by email.

Eligibility

Resource families or foster/adopted children living with the family in the State of Pennsylvania. Social service agencies requesting financing for special projects that promote the stability of the resource family. Nominations for Financial Awards

1. Nominations will be accepted from someone other than the nominee or a family member of the nominee. We encourage nominations from people who are wellacquainted with the individual/family and who can attest to the nature of the need being addressed. 2. Requests for funds from a social service/adoption/foster care agency must be submitted by the Director of the agency.

How to Apply for Financial Awards

Nominators must submit a completed application for the award for consideration by the Foundations voting body. The application must include: 1. Contact information of the nominee and the nominator. 2. A brief description of why the financial award is needed and how it will be used. 3. Itemized, verifiable documentation of the financial award requested. For agencies requesting an award to provide supplemental financial assistance to an existing program or to initiate a new program, a detailed outline of the goals for the program and how outcomes for the program will be identified and measured must be included in the narrative of the application. In addition,
a work plan and timeline for the project should be included.

4. Applications will be accepted at any time throughout the year. Once submitted, the Foundation will notify applicants in writing of selection or denial of the award within 90 days of receipt of the application. 5. ALL SECTIONS OF THE APPLICATION MUST BE COMPLETED THOROUGHLY AND ACCURATELY IN ORDER FOR THE FOUNDATION TO REVIEW THE REQUEST. FAILURE TO PROVIDE COMPLETE AND TRUTHFUL INFORMATION IS BASIS FOR DENIAL.

Selection Criteria and Award Disbursement: 1. Financial awards will be granted after consideration by the Officers and Board of Directors of the Foundation. Approval by 2/3 of the voting body of the Foundation will be required for the award to move forward. 2. An interview (via phone or in person) may be conducted as part of the award evaluation process in order to verify the financial need being addressed meets the mission of the Foundation. 3. Selection will be at the sole discretion of the voting body of the Foundation based on the availability of funding, the amount of the financial award being requested, the ability of the nominee/nominator to provide verification of how the award will be used, how the requested award meets the goals of the Foundation, and the perceived need of the nominee. 4. Awards will be limited to $1000 per calendar year by any individual, family or social service agency. Larger awards can be granted at the discretion of the voting body of the Foundation based on fund availability and need. 5. During the selection process, the voting body of the Foundation may request additional verification of the nature of the financial award requested. Failure to supply needed verification will result in denial of the application and the award. 6. Qualifications on the use of the award money will be set by the Foundation at the time the award is granted. Failure to comply with the qualifications will be considered fraudulent use of the funds and the Foundation may pursue legal action to recover the reward amount. These qualifications may include but are not limited to: a. Return of receipts to the Foundation validating use of the award money. b. Submission of outcome reports related to programs initiated with financial rewards from the Foundation. This relates to, but may not be limited to, awards made to agencies for the support of or initiation of projects or programs related to resource families. c. Any other request made by the Board and Officers of the Foundation in order to validate the appropriate use of financial awards.
