Nonpesticidetechnologiessuchastrappingandmonitoringdevices. Coordinationamongallfacilitiesmanagementprogramsthathaveabearingonthepest controleffort. Asalastresort,pesticidecompounds,formulationsandapplicationmethodsthatpresent thelowestpotentialhazardtohumansandtheenvironment. B. IPMServiceRequirements :TheServiceProvidershallfurnishallsupervision,labor, materials,andequipmentnecessarytoaccomplishthemonitoring,trapping,pesticide application,pestremovalandpestpreventioncomponentsofthisIPMprogram.Any deviationsfromthisprogrammustbeapprovedbytheChiefOperatingOfficer(COO). 2. Indoorpopulationsandinvadingindividualsofrodents,insects,arachnids,andother arthropods. Outdoorpopulationsofpotentiallyindoorinfestingspeciesthatarewithintheproperty boundariesofthespecifiedbuildings. Nestsofstinginginsectswithinthepropertyboundariesofthespecifiedbuildings. Termitesandotherwooddestroyingorganisms. Birds,bats,smallmammals,andallothervertebrates. Mosquitoes.

3.PESTCONTROLPERSONNEL Throughoutthetermofthiscontract,allpersonnelprovidingonsitepestcontrolservicemust maintaincertificationascommercialpesticideapplicatorsintheappropriatecategoriesforthe facilitieslistedabove.Uncertifiedindividualsworkingunderthesupervisionofacertified applicatorwillnotbepermittedtoprovideserviceunderthiscontract. 4.SERVICEPROVIDERIPMPLAN TheServiceProvidershallsubmittotheCOOanIPMPlanatleastfive(5)workingdayspriorto thestartingdateofthecontract.IfaspectsofthePlanareincompleteordisapprovedbythe COO,theContractorshallhavetwo(2)workingdaystosubmitrevisions.TheIPMPlanshall consistofthreepartsasfollows: A. PesticideLabelsandMSDSheets :TheServiceProvidershallprovidecurrentLabelsand MaterialSafetyDataSheetsforallpesticidesthatwillpotentiallybeusedinthepestcontrol program. B.ServiceSchedule(s):TheServiceProvidershallprovideascheduleofroutinepestcontrol inspectionsforeachbuildingservicedunderthiscontract,includingfrequenciesof inspections,areasateachfacilitytobegivenspecialattention(e.g.,foodstorage,preparation andservingareas;washrooms;custodialclosets;mechanicalrooms;entryways)andspecific day(s)oftheweekonwhichtheinspectionswillbeperformed. C.CommercialPesticideApplicatorLicensesandCertificates:TheServiceProvidershall provideaphotocopyoftheStateissuedCommercialPesticideApplicatorLicenseforevery Contractorperformingonsitepestcontrolserviceunderthiscontract,andaphotocopyof theStateissuedCommercialPesticideApplicatorCertificateforeverypestmanagement professional(PMP)performingonsitepestcontrolservice. TheServiceProvidershallreceivetheapprovaloftheCOOpriortoimplementingany subsequentchangestotheapprovedServiceProviderIPMPlan,includingadditionalor replacementpestcontrolproducts.TheServiceProviderwillreviewandupdatetheService ProviderIPMPlanannually,includingupdatingMSDS/labelsasneeded. 4.RECORDKEEPING TheServiceProvidershallberesponsibleformaintaininganIPMlogbookorfileforeachbuilding specifiedinthiscontract.Theserecordsshallbekeptonsiteandmaintainedoneachvisitbythe

PMPperformingpestcontrolservice.Eachlogbookorfileshallcontainatleastthefollowing items: A.IPMPlan:AcopyoftheServiceProvidersapprovedIPMPlan,includingpesticideLabels andMSDSsheetsforallpesticidesthatwillbepotentiallyusedinthebuilding,service scheduleforroutinepestcontrolinspections,andphotocopiesoftherelevantCommercial PesticideApplicatorLicensesandCertificates. B. BuildingOccupantLogForm :TheseformswillbeusedtoadvisetheServiceProviderof routineservicerequestsandpestsightingsbybuildingoccupants. ServiceProvidersReportForms :CustomercopiesoftheServiceProviderssignedand datedServiceReportForm,documentingallinformationonservicesprovidedincluding pesticideapplicationsrequiredbyStateandlocalstatute.Thisformmustalsoindicateany recommendationsmadebytheServiceProviderforadditionalactionadvisablebythe customer,e.g.,structuralorplumbingrepairsrequiredtolimitpestaccesstothebuildingor tofoodandwaterresources;improvementsinsanitation,etc.Acopyofthisformmustalso beprovidedtotheCOOwithinoneweekoftheservice. ServiceProviderProductsandDevices :Allbaitstations,snaptrapsandglueboardsorother devicesleftbehindbytheServiceProvideraretobedated,numberedandlistedonthe ServiceProviderReportFormandcheckedoneachsubsequentvisituntilremoved.Allsuch devicesshallberemovedwhenfull,dirtyandnolongereffective,ornolongerneeded.

5.MANNERANDTIMETOCONDUCTSERVICE A.TimeFrameofServiceVisits:FrequentandcompletecommunicationbetweentheService Providerandthefacilitymanageriscriticalforasuccessfuloutcome.Routinepestcontrol servicesthatdonotadverselyaffectstafforpatienthealthorproductivityshallbeperformed duringtheregularbuildinghoursofoperation.Whenitisnecessarytoperformworkoutside oftheregularlyscheduledservicetimesetforthintheServiceProviderIPMPlan,the ContractorshallnotifytheCOOand/orfacilitymanageratleastonedayinadvance. B.SafetyandHealth:Allpestcontrolworkshallbeinstrictaccordancewithallapplicable Federal,State,andlocalsafetyandhealthrequirements.Wherethereisaconflictbetween applicableregulations,themoststringentwillapply. C.SpecialEntrance:Certainareaswithinsomebuildingsmayrequirespecialinstructionsfor personsenteringthem.Anyrestrictionsassociatedwiththesespecialareaswillbeexplained bytheCOO.TheServiceProvidershalladheretotheserestrictionsandincorporatethem intotheServiceProviderIPMPlan. E. Uniforms :AllServiceProviderrepresentativesworkinginoraroundthebuildingsspecified inthiscontractshallweardistinctiveuniformsidentifyingthenameoftheiremployer.

Vehicles :VehiclesusedbytheServiceProvidershallbeidentifiedinaccordancewithState andlocalregulations.

6.SPECIALREQUESTSANDEMERGENCYSERVICE Onoccasion,theCOOmayrequestthattheServiceProviderperformcorrective,specialor emergencyservice(s)thatarebeyondroutineservicerequestssuchasremovalofastinginginsect nest.TheServiceProvidershallrespondtotheseexceptionalcircumstancesandcompletethe necessaryworkwithintwentyfour(24)hoursafterreceiptoftherequest. 7.INSECTCONTROL A. EmphasisonNonPesticideMethods :Nonpesticidemethodsofcontrolshallbeused whereverpossible.Forexample: 1. Portablevacuumsratherthanpesticidespraysshallbethestandardmethodforinitial cleanoutsofcockroachinfestations,forswarming(winged)antsandtermites,andfor controlofspidersinwebs. Trappingdevicesratherthanpesticidespraysshallbethestandardmethodforindoor flycontrol.

ApplicationofInsecticidestoCracksandCrevices :Asageneralrule,allinsecticidesshall beappliedascrackandcrevicetreatmentsonly,definedinthiscontractastreatmentsin whichtheformulatedinsecticideisnotvisibletoabystanderoraccessibletochildren duringoraftertheapplicationprocess. ApplicationofInsecticidestoExposedSurfacesorasSpaceSprays :Applicationof insecticidestoexposedsurfacesorasspacesprays(fogging)shallberestrictedto exceptionalcircumstanceswherenoalternativemeasuresarepractical.TheServiceProvider shallobtainapprovaloftheCOOpriortoanyapplicationofinsecticidetoanexposed surfaceoranyspacespraytreatment.Nosurfaceapplicationorspacesprayshallbemade whilestaff,patientsorvisitorsarepresent.TheServiceProvidershalltakeallnecessary precautionstoensurestaff,patientandvisitorsafety,andallnecessarystepstoensurethe containmentofthepesticidetothesiteofapplication. InsecticideBaitFormulations :Baitformulationsshallbethestandardpesticidetechnology forcockroachandantcontrol,withalternateformulationsrestrictedtouniquesituations wherebaitsarenotpractical.

TrappingdevicesshallbecheckedonascheduleapprovedbytheCOO.TheService Providershallberesponsiblefordisposingofalltrappedrodentsandallrodentcarcassesin anappropriatemanner. B. UseofRodenticides :Inexceptionalcircumstances,whenrodenticidesaredeemedessential foradequaterodentcontrolinsidebuildings,theServiceProvidershallobtainapprovalof theCOOpriortomakinganyinteriorrodenticidetreatment.Allrodenticides,regardlessof packaging,shallbeplacedeitherinlocationsnotaccessibletochildren,pets,wildlifeand domesticanimals,orinEPAapprovedtamperresistantbaitboxes.Asageneralrule, rodenticideapplicationoutsidebuildingsshallemphasizethedirecttreatmentofrodent burrowswhereverfeasible. UseofBaitBoxes :AllbaitboxesshallbemaintainedinaccordancewithEPAregulations, withanemphasisonthesafetyofnontargetorganisms.TheServiceProvidershalladhere tothefollowingfivepoints: 1. Allbaitboxesshallbeplacedoutofthegeneralview,inlocationswheretheywillnotbe disturbedbyroutineoperations. Thelidsofallbaitboxesshallbesecurelylockedorfastenedshut. Allbaitboxesshallbesecurelyattachedoranchoredtofloor,ground,wall,orother immovablesurface,sothattheboxcannotbepickedupormoved. Baitshallalwaysbesecuredinthefeedingchamberoftheboxandneverplacedinthe runwayorentrywaysofthebox. AllbaitboxesshallbelabeledontheinsidewiththeServiceProvidersbusinessname andaddress,anddatedbytheServiceProvideratthetimeofinstallationandeach servicing.

10.USEOFPESTICIDES TheServiceProvidershallberesponsibleforapplicationofpesticidesaccordingtothelabeland allapplicableregulations.AllpesticidesmustberegisteredwiththeUAEEnvironmental ProtectionAgency(EPA),Stateand/orlocaljurisdictionunlesspriorapprovalisgivenbythe COO.Transport,handling,anduseofallpesticidesshallbeinstrictaccordancewiththe manufacturerslabelinstructionsandallapplicableFederal,state,andlocallawsandregulations. TheServiceProvidershalladheretothefollowingrulesforpesticideuse: A. RentryTime,PostingandNotification: Pesticidesmaynotbeappliedwherestaff,patientsor visitorswillbepresentwithinsevenhoursaftertheapplication.Atleastseventytwohours priortoapesticideapplication,theServiceProvidershallpostan8x11pestcontrol informationsignbothatthesiteoftheapplicationandnearthefacilityreceptionareawhere

itwillbeseenbyvisitorsenteringthefacility.Thispostingshallincludethedate,timeand locationoftheapplication,theproductapplied,potentialadverseeffectsfromtheMaterial SafetyDataSheet(MSDS)andthepesticidelabel,andincludetheServiceProvidername, addressandtelephone.ServiceProvidershallalsoprovidethisinformationtothefacility directorwhowillusethisinformationtonotifystaffandpatientswhohaverequested notification.Emergencyapplications,wherepestsposeanimmediatethreattothehealth andsafetyofpatients,visitorsoremployees,disinfectants,antimicrobialsandselfcontained orgeltypepesticidebaitsappliedininaccessibleareasareexemptfromposting,notification andthe7hourreentryrequirement. B. ApprovedProducts :Nopesticideproductshallbeappliedthathasnotbeenincludedinthe ServiceProviderIPMPlanorapprovedinwritingbytheCOO. PesticideStorage :TheServiceProvidershallnotstoreanypesticideproductinthebuildings specifiedinthiscontract. ApplicationbyNeed :Pesticideapplicationshallbeaccordingtoneedandnotbyschedule. Asageneralrule,applicationofpesticidesinanyinsideoroutsideareashallnotoccur unlessvisualinspectionormonitoringdevicesindicatethepresenceofpestsinthatspecific area,andonlyafterallnontoxicmeanshavebeenexhaustedandshowntobeunsuccessful. Requestsforpreventivepesticidetreatmentsinareaswheresurveillanceindicatesapotential insectorrodentinfestationwillbeevaluatedbytheCOOonacasebycasebasis.Written approvalmustbegrantedbytheCOOpriortoanypreventivepesticideapplication. MinimizationofRisk :Whenpesticideuseisnecessary,asalastresorttheServiceProvider shallemploytheleasthazardousmaterial,mostpreciseapplicationtechniqueandminimum quantityofpesticidenecessarytoachievecontrol.
