Introduction Health care has become one of the most important service. Hospitals, physicians, insurers, and managed-care firms are networking, merging, and forming integrated organizations to finance and deliver health care. Hospitals, doctors, and other healthcare centers around the world require the ability to send and receive healthcare data, including patient information and various lab reports. Means that vast amounts of healthcare information are exchanged on a daily basis. However, medical data can be extremely complicated due to the abundance of clinical terminology, as well as the structural complexity in the formation of the presented information. Thus, this information must be presented in a standardized format in order to ensure that the data is universally understood and organized. All healthcare information must be sent in a specialized healthcare language. The language that has been developed to overcome these obstacles is HL7. This interface protocols define the rules by which two applications will exchange data in an orderly way.

PACS (Picture Archiving and Communication System) : PACS enables images such as x-rays and scans to be stored electronically and viewed on screens, creating a near filmless process and improved diagnosis methods. Doctors and other health professionals can access and compare images. PACS technology allows filmless process, with all of the flexibility of digital systems. It also removes all the costs associated with hard film and releases valuable space currently used for storage. PACS enables centralized storage of images, full interoperability and compatibility with other services.PACS are helpful in : Ensured access to images Future proofing Wide impact Guaranteed service, performance and support

Integration of images with patient records HIS (Hospital Information System or Health Information System) : HIS, also called clinical information system (CIS) is an integrated information system designed to manage the administrative, financial and clinical aspects of a hospital. This encompasses paperbased information processing as well as data processing machines.lt deals with the resources, devices and methods required to optimize the acquisition, storage, retrieval, and use of information in health and biomedicine. Health informatics tools include not only computers but also clinical guidelines, and information and communication systems. Main functions of HIS include: To support the strengthening of national health information systems based on an analysis of the needs of information. To contribute to the strengthening of monitoring and evaluation functions and their integration into the national managerial process through the implementation of effective information systems. To co-ordinate the efforts of partners and countries in the field of health indicators measurement and use, health data management, and evaluation of health information systems. EHR (Electronic Health Record) / EMR(Electronic Medical Record) : Both Electronic Health Record (EHR) and Electronic Medical Record (EMR) are used to keep the record of patient's medical information. The only difference is that EHR is a global concept and EMR is localized record. The terms EHR and EMR are used interchangeably. An EHR system is also often abbreviated as EHR or EMR. EHR is a electronic record of patient health information generated by one or more encounters in any care delivery setting. EHR includes the information of patient demographics, progress notes, and problems, medications, past medical history, immunizations, laboratory data and radiology reports. The EHR automates clinician's workflow. The EHR has the ability to generate a complete record of a clinical patient

encounter as well as supporting other care related activities directly or indirectly via interface, including evidence-based decision support, quality management, and outcomes reporting. EHR, technology represents a vast improvement over paper-based systems, and is changing the way healthcare is administered in medical practices. Accessing patient information and medical histories from a handheld device or desktop computer gives physicians, clinicians, and office personnel instant access to the information they need without searching through paper files. EHR help to streamline clinical workflow, leading to improved patient care, reduced practice operating costs, and increased billing opportunities.

DICOM (Digital Imaging and Communications in Medicine) : Digital Imaging and Communications in Medicine (DICOM) is a standard for handling, storing, printing, and transmitting information in medical imaging. It includes a file format definition and a network communications protocol. The communication protocol is an application protocol that uses TCP/IP to communicate between systems. DICOM files can be exchanged between two entities that are capable of receiving image and patient data in DICOM format. The DICOM standard is a communication definition for exchanging data between medical equipment. DICOM relies on industry standard network connections (TCP/IP) and is an efficient method for communicating digital images from diagnostic devices to display systems. DICOM is used for CT and MR, including nuclear medicine, ultrasound, computed radiography, digitized film, video, capture, HIS/RIS information, and connections between networked hardcopy output devices.

HL7 also support clinical practice and the management, delivery, and evaluation of health services.Specially to create flexible, cost effective standard, guidelines and methodologies to enable healthcare information system (HIS) interoperability and sharing of electronic health record (EHR). HL7 is a standard for exchanging information between medical applications. Level Seven refers to the seventh OSI layer protocol for the health environment. HL7 is a protocol for data exchange which defines the format and the content of the messages that applications must use when exchanging data with each another in various circumstances. HL7 affiliates in more than 30 countries are working to ensure that people everywhere benefit from healthcare technology standards. Health Level Seven (HL7) is American National Standards Institute (ANSI) accredited Standards Developing Organizations, operating in the healthcare area worldwide. To exchange the data between medical systems, there are several methods defined in HL7.Each method contains many events, each event consists of several messages, while messages are made up of a series of segment, field, component and subcomponent. Flow of messages between different systems is shown in the following fig.

HL" ctsmfci The basic idea of designing HL7/DICOM gateway is to link HIS and PACS system, which follow different standards. When events occur in one system, messages will be automatically sent to inform the receiving system. The messages are transmitted between HIS and PACS, and will be constructed or parsed through the gateway, and they will be turned into an admissive form used by the receiving system, The main function of HL7/DICOM gateway is to achieve information transmission, processing and exchange between HIS and PACS. Literature Survey
Jonathan E. Turner( 2007)[1] This guide was written with the aim of being a basic beginner's guide to PACS in the UK National Health Service, to explain some of the concepts involved in PACS, and to aid the process of purchasing and running a PACS. It includes how images are acquired onto the PACS, how these images are stored and managed, how they can be viewed, and the movement of data between the various components of a PACS. Boqiang LIU, Xiaomei LI, Zhongguo LIU, Qingwei YUAN and Xiaohong YIN (2008) [13] achieved the functions of send/receive messages and construct/parse messages by analyzing the principal of data exchange based on HL7 standard and designing HL7 messages. By doing experiments in one hospital, HL7/DICOM gateway designed in this paper can achieve most information exchange between HIS and PACS. The Electronic Health Record (EHR) is a longitudinal electronic record of patient health information generated by one or more encounters in any care delivery setting. Included in this information are patient demographics, progress notes, problems, medications, vital signs, past medical history, immunizations, laboratory data and radiology reports [10]. ZHU Sui-hui, RAO Ni-ni(2005)[6] In this, authors present an HL7 V3 gateway design based on the Web Services which can solve the problems of the HL7 V2.x. Web Services is an information exchange protocol regardless of the implementation platform, and is currently the main technology in enterprise application integration (EAI). Tianlong Shen, Jun Shi, Zhuangzhi Yan (2007)[12], developed the designing of EHR system based on PDA.EHR is an important method to realize "Early prevention, early diagnosis, and early therapy", which is the foundation of digital health management system for community medical service. For the reason above, our purpose is to design a demo EHR system on PDA for GPs. With the "Six in One" function, the EHR system can satisfy the individual requirement of CHS.
