DISCHARGE PLAN

Name of Patient (Initial): AH Age: 59 y/o Sex: Female

Date of Admission: July 24, 2013 Diagnosis: Coronary Artery Disease

The client has Coronary Artery Disease and she is instructed to take the following plan for discharge:

I. MEDICATIONS:
Omeprazole 40mg once a day for two weeks, Antiulcer agents

Amlodipine 5 mg/tab once a day, for increased blood pressure/ Hypertension Celecoxib 200 mg twice a day, for Decreasing pain
Isosorbide Mononitrate 60mg once a day, Sublingual, Relief and prevention of angina attacks.

II. DIET: Diet must be low in saturated fat, cholesterol, sodium and alcohol. III. TREATMENT: Have an oral fluid intake with in cardiac tolerance. Emphasized the importance of regular follow-up check-ups and as instructed by physician. Advise the patient and the family members to seek medical advice if any unusual things arise like unstable angina, blood pressure greater than 120/90. Advice the client to drink medication properly, even the signs and symptoms do not come up and teach major side effects of each medicine prescribed using language the patient can understand. IV. EXERCISE: Do physical exercises at least 30 minutes of moderate intensity exercise three to four times a day. Teach client for the importance of exercise programs.

V. ACTIVITY: Avoid strenuous activities. Should have assistant in doing some of Activities of Daily Living if needed. Advice to sit while doing activity. VI. FOLLOW UP CONSULTATION: Have a follow-up check up on her doctor 1week after the discharge from the hospital. VII. SIGNS AND SYMPTOMS TO SEEK IMMEDIATE EMERGENCY

CONSULTATION: Unrelieved angina by used of medication and rest. It may typically describe as severe pressure, heavy, squeezing, constrictive, suffocating, vise-like or like an elephant sitting on my chest. High blood pressure even with medications. Manifesting signs of Transient Schemic Attack. VIII. ADDITIONAL INFORMATION (DOS and DONTS): Avoid activities known to cause angina pain such as sudden exertion, walking against the wind, extremes of temp, high altitude. Instruct client to eat healthy foods and must comply with the given diet. Instruct client to take medications as prescribed, at the correct time, route, and dosage. Advice the client and the family to avoid second hand smoking since it lowers HDL levels and increase the risk for severity of disease. Lessen emotional stress as it precipitates angina.
