should provide the accused with the documents it intends to use in the hearing as well as other relevant documents requested by the employee. It is the trend of Weir to have a form that is used in an interview for the purposes of making sure that the right documents and or questions are asked during the interview - I request the 1st document for my very first interview conducted by Weir for the position of Shift Millwright / electrician and the 2nd interview whereby I was promoted to the position of ASSET CARE AND INSTRUMENTATION TECHNICIAN. 2. It is common practice that when Weir trains persons on In house company IT programmes etc as in SHOPWARE, BAAN ETC there is a training form signed and kept on record of the said training that has been provided by Weir and this is kept in the said requested file. Copies of the said training is requested. 3. Copies of any other grieviences and or warnings to be supplied. 4. Copies of all documentation on the fast track successor plan as initiated by GM Pieter Schoeman. 5. Copies of the accident recorded and investigated whereby Phillip Du Preez was involved in driving a crane and an IOD incident took place whereby Desmond the Boiler maker from CAP. These reports to made available as the person driving the crane was unlicensed at the time. 6. A copy of my PDPs as used in my performance at Weir. 7. Further more as in line with my right to a fair hearing witness's will again be supoenad to the said hearing, these will be delivered by the end of business on the 30th of July 2013. The said statutory fee as prescribed in the LRA shall be paid over before the said date of the hearing. Witness fees (1) The fee payable to a witness in terms of section 142(7) of the Act is the total of- (a) R200 for each day or part of a day during which the witness is required to be present at any proceedings; and (b) reasonable substantiated travel and subsistence expenses incurred by the witness in order to be present at those proceedings. (2) Despite sub-regulation (l), no witness fee may be paid to a person who, at the time of the relevant proceedings, is employed full-time by the state, or is a member of any legislature mentioned in the Constitution.

8. The above documents will be expected to be collected from your premises at Weir Minerals Africa 31 Isando Road Isando by close of business 7 days hereof calculated to be the 1st day of August 2013 15h30
