1. FOREWORD
SLC inform on barge arrival on 2nd august and Load out 12 august. SIKB alerted that this period of the year is extremely difficult due to the end of Ramadan and low attendance from personnel. SLC has confirmed those dates. Planning of load out issue to SLC and CPY. MDR Preparation for Jacket Scope under finalization (98% Completed). Documentation balance are: o Balance NCR27/29 for DC on Jacket and Deck legs superposition. Additional DC required on 4 legs. SIKB to implement o Submit RLMU issued to Perth SLC. o Final reports of IRC ( after quarantine) Piles design change: final clarification to be received. SLC to expedite final requirements for design of Pad eyes on Domgas piles. Pontoons and Piles Barge layout to be received from Project. SIKB request assistance from Project for the Pontoons water 2 valves impossible to source. SLC to expedite all SIKB Pending RFI ( test pressure of the pontoons, sequence, etc).

Highlight the latest incident to all workers on TBT. Keep continue socialization and monitored implemented of JSA for working on jacket

legs to all workers involved. Ensure all equipments (blower, flexible hose, etc.) and additional PPE in place and acceptable. Ensure condition of man-basket has been check by competent person and filled the checklist. Ensure and keep monitored the blasting and painting activities in accordance with the procedure. Especially for blasting and painting inside confine space (pontoon). Ensure only fire proof light used for activities painting inside confine space. Socialization JSA and procedure Painting and Blasting for pontoon to every worker involved. Monitored PFP for deck activities comply with the SIKB procedure requirement. Ensure all hazardous waste and toxic waste from PFP activities send to hazardous waste building daily based. Keep monitoring and improve the housekeeping/management, coordinated with team leader for arrange cable and hoses properly. Ensure proper scaffold structures erected for deck and pontoons. o The pest control Contractor monitoring the rat box, trigonal glue traps, cat/dog traps and Mobe
Moat on daily basis, and reported to the Quarantine Officer if any quarantine risk material trapped on the trap, and they will be replace the bait every two weeks.
Number of traps implemented at Gorgon Project area (updated 03 July 2013)
o Maintain salt barrier surround quarantine fences at Jacket and Load Out area.
o Replace moat at Final Storage & Erection Site.
o Covering the opened holes at connector for quarantine fences with adhesive tape. This activity has been done at Final Storage & Erection Site.

Residual insecticide spraying Regular insecticide spraying has been performed at toilet facilities, waste bins, Raw Material Storage, Prefabrication Shop, Assembly Hall, Final Storage & Erection Site and Load out Area. These activities are done at early morning (05:00hrs - 06:30hrs) by Pest Control contractor.
Insecticides spraying inside Leg Jacket and top cover plates at CD East Row C2, C3, D1, D2, AB East Row A2, A3 and B3 have been done after shim plates removed and cleaning activities finish.
Note: Chemical used for insecticide spraying is Bifenthrin, product name is BISTAR 25 g/L EC INSECTICIDE / MITICIDE. o Weed killer Weeds eradicated by two treatments, the first is manually pull out the weeds; and the second is prevent weeds growing by perform herbicide spraying. The active ingredient of chemical used for herbicide spraying is Glyphosate isopropyl amine salt, name of product is Roundup 486 SL. This activity has been performed at Final Storage & Erection Site.
o Fumigation Thermal fumigation has been performed around Raw Material Storage, Prefabrication Shop,

Note: Chemical used for fumigation is Permethrin, product name is PERMANETTM 100 EC INSECTICIDE.
