The medium (CD/DVD/memory stick) contains the following folder structure: 1. Root directory It contains all files for running the patient data burned on the medium with eit her the iQ-LITE viewer or from a web browser, including the manual. It also cont ains the 'DICOMDIR', the main table of the database. 2. DICOM In this folder are saved the DICOM image data. A specific folder structure is us ed: Each study is filed in a separate folder which is comprised of individual fo lders for the different series that are included in the study. The series folder s then contain the individual images. 3. IHE_PDI This folder contains the 'BANNER.JPG', which is used as a label for both the iQLITE viewer and the top of the INDEX.HTM page. Further you will find here the ht ml sub-pages for study level and series level as well as the individual images a s JPEG. 4. REPORTS The DICOM structured reports in an html web view are stored here. iQ-LITE provides a convenient way for physicians and other medical professionals to copy and save patient studies/images processed by an iQ-VIEW / PRO Workstati on. When providing copies of these studies to referring physicians and/or patien ts, iQ-LITE provides an integrated, simple way to view the information stored on the CD/DVD/memory stick - without the use of any 3rd party software. iQ-LITE is only permitted for diagnostic use if all applicable legal requirement s in your country are met. Please consult your authorized local distributor befo re using the application for diagnostic purposes. CONCURRENT LICENSE You may: (i) install and use copies of the Software on an unlimited number of co mputers within your organization and local network; and (ii) you may only use th e Software with one user simultaneously. If you wish to use the Software with mo re than one user simultaneously or in different networks, you must obtain additi onal licenses from IMAGE. 2. LICENSE RESTRICTIONS a) All rights not expressly granted are reserved. b) You may not: (i) permit other individuals or entities to use or have access t o the Software except under the terms of this Agreement; (ii) modify, translate, reverse engineer, decompile, disassemble (except to the extent that this restri ction is expressly prohibited by law) or create derivative works based upon the Software or Documentation; (iii) copy the Software or Documentation (except for back-up or archival purposes); (iv) rent, lease, lend, assign, or otherwise tran sfer rights to the Software or Documentation; or (v) remove any proprietary noti ces or labels on the Software or Documentation. Any such forbidden use shall imm ediately terminate your license to the Software. c) You agree that you shall only use the Software and Documentation in a manner that complies with all applicable laws, regulations and the like in the jurisdic tions in which you use the Software and Documentation, including, but not limite d to, applicable restrictions concerning medical uses and copyright and other in tellectual property rights. d) The Software or the use of the Software may be subject to legal or regulatory provisions related to products used in the Health Care Industry. Prior to using the Software, it is your responsibility to ensure that your use of the Software will not violate any legal or regulatory provisions. Please contact IMAGE to de termine the most recent information regarding legal and regulatory approvals. e) The use of the Software is intended only for use with properly authorized con tent. Content files, including, but not limited to images, which are viewed usin g the Software, may be protected by copyright laws or other laws of any jurisdic tion and are used at your own risk. f) You may only use the Software for your internal purposes. You may not use the Software in any way to provide, or as part of, any application service, provide r service or other similar commercial service or application in which third part ies have access to the Software.

3. UPDATES/ERRORS IMAGE reserves the right at any time to, but is not obligated to, correct errors , alter features, specifications, capabilities, functions, release dates, genera l availability or other characteristics of the Software ("Updates"). You agree t hat you may not have access to such Updates without separate payment or through a separate support agreement. If any such Updates are provided to you by IMAGE, such Updates will be considered a part of the Software and subject to the terms and conditions of this Agreement. You agree and understand that although IMAGE t akes steps to prevent errors, the Software may contain errors affecting proper o peration. 4. TITLE Title, ownership rights, and intellectual property rights in and to the Software and Documentation shall remain with IMAGE. The Software is protected by the cop yright laws of the United Kingdom, the European Union and by international copyr ight treaties. Title, ownership rights and intellectual property rights in and t o any content used with the Software shall be retained by the applicable content owner and may be protected by applicable copyright or other law. IMAGE also ret ains all rights, titles, and interests in and to the trademarks, trade names, lo gos, and icons (collectively "Marks") used in or identifying the Software or its features and you may not use such Marks without the prior written permission of IMAGE. Other trademarks are either registered trademarks or trademarks of their respective owners. 5. LIMITED WARRANTY AND LIMITATION OF LIABILITIES IMAGE warrants that the Software will perform in accordance with the Documentati on for a period of: (a) thirty (30) days for the demo and evaluation version and (b) ninety (90) days for the full version from the date of receipt. Subject to section 4 above, IMAGE will make commercially reasonable efforts to correct any failure to perform in accordance with the Documentation. DISCLAIMER OF WARRANTY & LIMIT OF LIABILITY. 7. TERMINATION This License Agreement will automatically terminate if you fail to comply with a ny term hereof. No notice shall be required from IMAGE to effect such terminatio n. You may also terminate this License Agreement at any time by notifying IMAGE in writing of termination. Without prejudice to any other rights, upon any termi nation of this License Agreement, you shall immediately discontinue use of the S oftware and Documentation and shall within three (3) days return to IMAGE, or ce rtify destruction of, all full or partial copies of the Software, Documentation and related materials. 8. REPLACEMENT/MIGRATION LICENSES IMAGE reserves the right to impose charges for replacement and migration license s. This applies in the event of the following circumstances: (i) You require a r eplacement license for a hardware fingerprint activated license without a valid Annual Service Package. The standard fee according to the valid MSR price list a pplies if you provide IMAGE with the software uninstall code before replacement or migration. (ii) You require a replacement license for a hardware fingerprint activated license without a valid Annual Service Package. If you cannot provide IMAGE with the uninstall code before replacement or migration, a fee of 4/10 (40 %) of the current license price applies. (iii) You require a replacement licens e for a hardware fingerprint activated license with a valid Annual Service Packa ge. You can provide us with the uninstall code before replacement or migration. One (1) replacement license will be free of charge per valid Annual Service Pack age. The standard fee according to the valid MSR price list applies for the seco nd and all following replacement requests. (iv) You require a replacement licens e for a hardware fingerprint activated license with a valid Annual Service Packa ge. If you cannot provide us with the uninstall code before replacement or migra tion, a fee of 4/10 (40 %) of the current license price applies. (v) Replacement s and migrations for file based activated licenses are available for the standar d fee according to the valid MSR price list. 9. MISCELLANEOUS This Agreement shall constitute the complete and exclusive agreement between the Parties, notwithstanding any variance with any other written instrument submitt ed by you, whether formally rejected by IMAGE or not. The terms and conditions c ontained in this Agreement may not be modified except in a writing duly signed b y you and an authorized representative of IMAGE. This Agreement is personal to y ou, and may not be assigned without IMAGE's express written consent. If any prov ision of this Agreement is held to be unenforceable for any reason, such provisi on shall be reformed only to the extent necessary to make it enforceable, and su ch decision shall not affect the enforceability of such provision under other ci rcumstances, or of the remaining provisions hereof under all circumstances. The parties specifically acknowledge and agree that this Agreement be drafted and fu lly enforceable as written in the English language. This Agreement shall be gove rned by the laws of the United Kingdom, without regard to conflicts of law provi sions, and you hereby consent to the exclusive jurisdiction of the courts sittin g in London, United Kingdom. To the extent that you have breached or have indica ted your intention to breach this Agreement in any manner which violates or may violate IMAGE's intellectual property rights, or may cause continuing or irrepar able harm to IMAGE (including, but not limited to, any breach that may impact IM AGE's intellectual property rights, or a breach by reverse engineering), IMAGE m ay seek injunctive relief, or any other appropriate relief, in any court of comp etent jurisdiction. This Agreement will not be governed by the United Nations Co nvention on Contracts for the International Sale of Goods, the application of wh ich is hereby expressly excluded.
