SUSAN DENTZER
Susan Dentzer is Senior Policy Adviser at the Robert Wood Johnson Foundation, the nations largest health and health care philanthropy, based in Princeton, New Jersey. In this role, she works closely with foundation leaders to carry out the organizational mission of improving the health and health care of all Americans. One of the nations most respected health and health policy thought leaders and journalists, she is also an onair analyst on health issues on PBS NewsHour. Dentzer is an elected member of the Institute of Medicine and the Council on Foreign Relations, and a fellow of both the National Academy of Social Insurance, a nonprot, nonpartisan organization made up of the nations leading experts on social insurance, and the Hastings Center, an institution dedicated to bioethics and the public interest. She is also a public trustee of the American Board of Medical Specialties, the not-for-prot organization that oversees 24 approved medical specialty boards in setting standards for board certication and maintenance of certication for the nations physicians, and a member of the board of directors of Research!America, which advocates for increased US investment in biomedical research. Dentzer is a frequent guest and commentator on such National Public Radio shows as This American Life and The Diane Rehm Show. From May 2008 to April 2013, Dentzer was the Editor-in-Chief of Health Aairs, the nations leading journal of health policy, where she transformed the journal from a bimonthly academic publication to a highly readable and topical monthly journal and online publication with more than 120 million page views annually. Prior to joining the journal, Dentzer was an on-air correspondent on health and health policy for PBS NewsHour with Jim Lehrer,where she earned numerous awards for her work. (The unit was supported by a grant from the Robert Wood Johnson Foundation.) Before joining the NewsHour, Dentzer served as chief economics correspondent and economics columnist for US News & World Report and a senior writer for Newsweek. records (EHR) for the improvement of medicine. Our network accelerates discovery of critical correlations, trends, and outcomes across millions of patients, clinical records, and diverse topologies providing unique insight to enable providers and life sciences organization to deliver safer, more ecient, and more eective health care. Stephen has had a 25-year career in founding, investing in and building technology companies. He is recognized for his focus in data mining and business intelligence related ventures. Most recently he served as the Chairman and Chief Executive Ocer of Everstream, the company he founded in 1999. Everstream is the largest census level media measurement, data warehousing, and analytics platform in the world, measuring usage across 25 million households. Sold to Concurrent [NASDAQ: CCUR] in 2005, it continues to be the industry leader of enterprise software for broadband network usage measurement. In 1985, Stephen founded McHale and Associates, Incorporated, a USConnect regional oce where he served as Chairman and Chief Executive Ocer. Stephen was a founding investor and served on the board of directors and as CEO of USConnect, an international systems integrator. USConnect generated aggregate revenues of $150 million until its sale to IKON Oce Systems [NYSE: IKN] in 1997. Stephen is also Managing Director of 23Bell LLC Investments and an active Advisory Board Member to Genesis Inventions.

DAPHNE MILLER, MD
Daphne Miller, MD is a family physician, writer, and Associate Professor in the Department of Family Medicine at the University of California San Francisco. In 2000 she founded WholefamilyMD, San Franciscos rst integrative primary care practice, where she still cares for patients across the lifespan. Part clinician, part ecologist, and part anthropologist, Miller approaches medicine with the idea that opportunities for health and healing are found not only in the medical system but in such unexpected places as home kitchens, school gardens, community organizations, spiritual centers, farms, and nature trails. Throughout her career, her teaching, writing, and advocacy work has reected this perspective. In her latest book, Farmacology: What Innovative Family Farming Can Teach Us About Health and Healing (William Morrow, 2013), Miller discovers how learning from sustainable farmers can make her a better doctor. STEPHEN MCHALE, CEO


Stephen McHale is co-founder and Chief Chairman and Executive Ocer of Explorys. Formed in partnership with Cleveland Clinic, Explorys addresses the national imperative to leverage electronic health

Her rst book, the Jungle Eect (HarperCollins 2008), a Northern California Bestseller, chronicles her voyages to areas around the world that are still relatively free of modern chronic diseases. Miller is a regular contributor to the Health section of the Washington Post and has been interviewed and proled in a wide range of publications and media outlets, including the New York Times, O Magazine, Harvard Medical Magazine, Vogue, the Michael Krasny Show and the Dr. Oz Show. She has spoken in such venues as Google Headquarters, the Yale Ideas Festival, the Commonwealth Club, Chubu Hospital in Okinawa Japan, and PINC in the Netherlands. Miller is a nationally recognized leader in the Healthy Parks, Healthy People initiative, an eort spearheaded by the National Parks Service to build linkages between our medical system and our park system in order to reintegrate human, environmental, and ecological health. Her 2009 Washington Post article Take a Hike and Call Me in the Morning is widely credited with sparking nature prescriptions, a concept that is rapidly gaining traction across the United States.

A full refund of fees less a $25.00 administrative fee will be made if notice of cancellation is received, in writing, by Friday, October 18, 2013. A $50.00 administrative fee will be assessed for any cancellations received after October 18, 2013 and until Thursday, October 24, 2013. No refunds will be made after October 25, 2013.
