MEMORANDUM OF AGREEMENT
This Memorandum of Agreement made and executed between: Northeastern College a non-stock, non-profit educational institution, duly organized and existing under Philippine Laws with office/business address at National Highway Villasis, Santiago City 3311 represented herein by the President TOMAS C. BAUTISTA Ph.D. , hereinafter referred to as the SCHOOL. and __________________________________________, a domestic corporation duly organized and existing under Philippine Laws with office/business address at ______________________________________________________, represented herein by ________________________________________, hereinafter referred to as the COMPANY. Witnesses: The parties hereby bind themselves to undertake a Memorandum of Agreement for the purpose of supporting the COLLEGE on-the-job training for student-trainees under the following terms and conditions: 1. 2. 3. 4. The SCHOOL shall be responsible for briefing the on-the-job student-trainees who intend to conduct practicum exposure in the COMPANY as part of the COLLEGE curriculum; The SCHOOL shall provide the on-the-job student-trainee the basic orientation on work values, behavior, and discipline to ensure smooth cooperation with the COMPANY; The SCHOOL shall issue an official endorsement vouching for the well being of the on-the-job student-trainee which shall be used by the COMPANY for processing the application of the student-trainee; The SCHOOL shall voluntarily withdraw a student-trainee who is found to misbehave and/or act in defiance to existing standards, rules, and regulations of the COMPANY and impose necessary school sanctions to the said studenttrainee; The COMPANY may grant allowance and/or adequate insurance to studenttrainee in accordance with the COMPANY'S existing rules and regulations; The COMPANY, upon consultation with the SCHOOL, may require qualified students to submit themselves to examinations, interviews, and file pertinent documents to support their application; The COMPANY is not obliged to employ student-trainee upon completion of the training; The student-trainees shall be personally responsible for any and all liabilities arising from negligence in the performance of his/her duties and functions while under training; There is no employer-employee relationship between the COMPANY and the student-trainee; The duration of the program shall be equivalent to 486 working hours unless otherwise agreed upon by the COMPANY and the SCHOOL;

11. Any violation of the foregoing covenants will warrant the cancellation of the Memorandum of Agreement by the COMPANY within thirty (30) days upon notice to the SCHOOL. 12. This Memorandum of Agreement shall become effective upon signature of both parties and implementation will begin immediately and shall continue to be valid hereafter until written notice is given by either party thirty (30) days prior to the date of intended termination. Narrative Report for On the Job Training of Information Technology (IT)


Intership Site: LGU Santiago City, Isabela

Introduction
Being a student of Information Technology we are undertaken a subject which is the On the Job Training (OJT) and be fulfill at the end of the 2nd Semester of Year 2011-2012. As we observed a person who has been employed of being Information Technology (IT), we are working on a several areas of an IT student such as Software Development, Web Development, Multimedia and etc.

Brief History
The origin of Santiago City can be drawn from the first native settlement discovered by the early Spanish missionaries at the bank of the old Carig River (now Diadi River) from which its original name, Carig, was derived. The early inhabitants were the Gaddangs and the Ibanags. When theSpanish settled in, the city was named Pueblo of Santiago Apostol de Carig, with Santiago as the Spanish name of Saint James the Apostle. In the early 1950s, the Municipal President Vicente Carreon changed the name to simply Santiago. Santiago remained a municipality for 84 years. Santiago was originally a part of the province Cagayan (comprising the whole Cagayan Valley region), which was reorganized as a political subdivision in 1583 with Nueva Segovia as its capital. On May 1, 1856, when the Province of Isabela was carved out by a Royal Decree, Santiago was among the towns relinquished to the newly created province. The first five barrios after the Cadastral survey in 1927 were Patul, Batal, Nabbuan, Buenavista and Dubinan. It was said that there were only about three Filipino-owned sari-sari stores in Santiago in 1917. The settlers acquired most of their merchandise and other provisions from Chinese traders in Echague, the landing zone for products intended for Santiago and other towns, owing to its proximity to the Cagayan River. It was when the Villa-Verde Trail was opened when things were set in motion. It facilitated the entry of immigrants from various provinces in Luzonto the Cagayan Valley and Santiago absorbed a sizable share of these travelers. The new route served as an impetus for growth and introduced new technologies and business opportunities. Santiago survived through world wars, although badly damaged, and from then on developed to become the leading commercial and industrial city in Cagayan Valley. In 1942, occupied by the Japanese forces entered in the town of Santiago. In 1945, founded to the liberation in the town of Santiago province of Isabela beginning the Filipino soldiers of the 1st, 2nd, 11th, 12th, 13th and 15th Infantry Division and the USAFIP-NL 11th Infantry Regiment of the Philippine Commonwealth Army, the 1st Infantry Regiment of the Philippine Constabulary and the recognized guerrilla fighter unit against the Japanese Imperial forces under defeated during World War II.

On December 17, 1993, the bill converting Santiago into an independent component city was approved by the Lower House. On the following year, the Senate Committee on Local Government approved another public hearing dated February 23, 1994.

Ramos and House Speaker Jose De Venecia On May 5, 1994, by virtue or Republic Act 7720 signed by His Excellency, President Fidel V. Ramos, Santiago was pronounced as an independent component city, the first in the Cagayan Valley Region. This made Santiago, self-governing and autonomous city from the rule of the provincial governor. Republic Act 8528 repealed this statute transforming it back to a component city. It was not until December 29, 1999, when theSupreme Court contested the validity of the latter decision and favored Santiago to be once again an independent component city.

Summary of Works
I choose the LGU Santiago City as Internship Site for my On the Job Training Subject. I and myself was assigned in Government Servicing Office (GSO) and Administration Office to make a Report Generating System and it takes a several week to accomplish it and we also assign to trouble shoot some computers of the City Hall. In making the system we are also encountered an errors and debug it and we are not able to overcome this problem without the use of the internet. We use the internet to search for the solution of it like How could we generate and print the report with filtering. And we had been undergoing in checking by a demonstration to the person that has been assigned to use it. And we had been assigned to an enhancement of the system and add a security for it. We are use the HIGH-ENCRYPTION for the security. Conclusion I there conclude that there are so many ways to be a successful IT Professional. And By experiencing that assignments and doing that assignments we are now able to say that we are improving our skills by experience it, applying it and on the fact that we are able to be success for brighter future by doing the thing that we learned.

Narrative Report for On the Job Training of Information Technology (IT)


Intership Site: Office of the Municipal Assesor of Claveria Cagayan. Claveria Grassroots Multi-Purpose Cooperative.

Introduction
Being a student of Information Technology we are undertaken a subject which is the On the Job Training (OJT) and be fulfill at the end of the 2nd Semester of Year 2011-2012. As we observed a person who has been employed of being Information Technology (IT), we are working on a several areas of an IT student such as Software Development, Web Development, Multimedia and etc.

Brief History
Originally, Claveria had 29 barangays (formerly called barrios). With the strong belief and perception of decentralization in the government administration, former Mayor Placido P. Leao, through his leadership, some of the barangays were divided if not merged to create two or three barangas, thus sometime in July of 1982, the 29 barangays became 41. The splendors of the beautiful and fast growing town of Claveria can be traced from its founding forefathers, most of them from the general directions of the towns of Pasuquin and Vintar in the Province of Ilocos Norte. They blazed their trail northeastward and crossed the Caraballo Mountain Ranges (popularly known to travelers as Calvario because of the dangers posed by its winding, precipitous and muddy roads) in quest for better life and greener pasture. Clearing a vast area of forest situated besides a river full of fishes and lilies then called bakungs, they first settled there and called their villa kabakungan which means, where the bakungs are, until for obvious reasons the name became Kabikungan.

There was a move to transfer their community near the seashore, considering that most of them are fishermen. However, the Spanish missionaries who then ruled them were against the move because of the stone church that was constructed as the center of their faith. The move had never been realize until the coming of the marauding pirates called Tirongs, who cruised the South Seas. They plundered the place, burned some dwellings and had some other forms of violent attacks in the area. With the situation, Spanish officials decided to put up a Spanish military fort along Cabicungan River that would provide escorts of defense against

attacks and/or springboard of conquest in the mountainous region of Apayao. Morever, the putting up of a fort was to help protect the travelers who journeyed both the land and by the sea, particularly those from Ilocos to Cagayan, passing by Cabicungan. On the other hand, a church was reconstructed at Nagsimbaanan, now called Pontoon as a place of worship for Christians.

Don Narciso Claveria, the Governor General of the Philippines made a trip to the whole of Cagayan Valley in 1846 in order to find out and solve problems besetting in the area. Through his leadership, he was able to conquer and stop violent attacks and raid south seas wherein he was conferred the Grand Cross of San Fernando by the Queen of Spain. His successful campaign sounded the death knell of the marauding pirates in 1848. One day, when his ship docked beside the Kabicungan river, in time also of with thanksgiving and celebration in the village, the residents petitioned the sympathetic governor that their village be turned into town or pueblo and be separated from the town Pamplona in order for travelers and/or settlers from Ilocos to Cagayan have a better resting place and considering also the huge distance to be traveled from Kabicungan to the town which is 40 kilometers.

After years of waiting, Cabicungan finally became a town on June 5, 1865 through a decree of the Governor of the Philippines and adopted the name CLAVERIA in honor of Don Francisco Claveria, the Governor General of the Philippines in recognition of his successful campaign against pirates, his unforgettable visit to the place and his humanitarian policies that have contributed to the development of the place

Claveria was originally a land of Ibanags. But when it came a parish in 1878, it was dominated by Ilocanos from Ilocos Region, particularly Vintar and Pasuquin.

Summary of Works
On the month of April 2012 I was assigned on the Office of Municipal Assessor of Claveria Cagayan to make a system for the Treasury Office and called CRPTAS which arrived from Claveria Real Property Tax Assessment System. but not all the feature we will do for it only for the computation of the taxes if it is discounted or not. I have been told to do that. And on the third week I have been assigned make a new Logo of Claveria and it takes a week to do that because of the comment of it and what the clips on the logo symbolizes. And on the next month the month of May 2012 I was been transfer in Claveria Grassroots Multi-Purpose Cooperative and have been assigned to make the Personal Management System. On the first week I gather information and the Chairman told me what should the system must able to do. And it takes a month to do that. I design User Interface for it and research what codes and plugins that will be used for it. On the last weeks of our duty I have been assigned to present it on the Board of Directors of the Cooperative and been demonstrate the features of the system. And with the Personal Officer of the Cooperative Ive Demonstrate how to use it. Here are the some Screen Shots of my Works:

Conclusion I there conclude that there are so many ways to be a successful IT Professional. And By experiencing that assignments and doing that assignments we are now able to say that we are improving our skills by experience it, applying it and on the fact that we are able to be success for brighter future by doing the thing that we learned.
