Team Monitoring Plan
2008-2009

Individual Monitoring and Schedule

• Team Monitoring – Team Monitoring will be conducted by the Monitoring Team.


Half of the sites, M&M, DWO, Beck, and YWCA Full Day will be monitored in
October and March. The other half of the sites, D-7, Smith Center/Home Base,
Littlefield /Combination, & Garden City will be monitored in November and April.
Each site will also be monitored as part of the SAVS process using the SAVS
monitoring tool. The Monitoring Team is made up of the Education and Early
Childhood Development Specialist, the Disabilities Coordinator, the Family
Community Partnership Specialist, the Literacy Coordinator, the Mental Health
Specialist, the Child Health and Development Specialist and Site Leaders,
Fatherhood Outreach Worker and Lead Family Service Worker.
• Parent Committee Meeting Monitoring – This monitoring is conducted two times
per year by the Family and Community Partnership Specialist.
• Parent Involvement Monitoring – This monitoring is conducted monthly by the
Family and Community Partnership Specialist.
• FSA/FPA Monitoring – This monitoring is conducted monthly by the Family and
Community Partnership Specialist.
• Mental Health Classroom Observations – This monitoring is conducted twice a
year by the Mental Health Specialist.
• Self-Assessment for Early Literacy Classroom Environment - This is a self-
monitoring conducted by each classroom’s teaching team in September. The
Literacy Coordinator will complete the Self-Assessment for Early Literacy
Classroom Environment checklist for each classroom during the second round of
team monitoring to ensure that the classroom is still in compliance and we are
providing a quality environment conducive to learning.
• Health and Safety Checklist – This monitoring is conducted monthly by the Lead
Teacher.
• Site Leader Monthly Monitoring & CQI- This is a self-monitoring conducted
monthly by the Site Leader.
• Lead Teacher Weekly Monitoring & CQI – This monitoring is conducted weekly by
the site’s Lead Teacher.
• Classroom Teacher Monthly Self Monitoring Checklist & CQI – This is a self-
monitoring conducted monthly by each classroom’s teaching team.
• CACFP Classroom Observation – This monitoring is conducted at each site three
times per year by the Nutrition Manager.
• Professional Development Tracking Tool- Education Specialist will log training
hours as trainings take place.

• ESI/Denver Screenings – monitored weekly by the Education and Early Childhood
Development Specialist.
• Disability Enrollment and Smart Forms – monitored monthly by the Disabilities
Coordinator.
• All Screenings – monitored weekly by the Child Health and Development
Specialist.
• FSA/FPA – monitored monthly by the Family and Community Partnership
Specialist.
• ERSEA – monitored weekly by the Lead FSW.
• Fatherhood – monitored quarterly by the FOW and/or Family Community
Partnership Specialist
