Calling All Parents/Guardians!

The Hamilton-Wentworth District School Board wants your opinion. Your input is very important to us and for that reason we
have developed an Annual Parent Satisfaction Survey. Based upon previous feedback received, improvements have been
made in the following areas:

• our consultation process with parents

• our HWDSB website
• preparation of elementary homework guidelines
• modifications to special education delivery and support
• including parent involvement in school planning
• improved parent access to student information through our “Parent Assist” project

This year, we are seeking your views about our progress in the following areas:

● Communication with You

● Student Learning
● School Safety
● Student Assessment
● High Expectations for Student Achievement
● Learning Time
● Home-School Relationships

We are very interested in how you view the Hamilton-Wentworth District School Board (HWDSB) – what you think we do well,
and how we might improve our education services to you and your child(ren).

To complete the survey, please go to our website www.hwdsb.on.ca or complete a paper copy
(available at your school) and return it to us through your school or at the Education Centre, 100 Main
St. West, Hamilton, Switchboard.

We appreciate your voluntary participation in this survey. Please remember that you may skip questions or stop completing the
survey at any time. If you wish to stop filling out the survey at any time, simply close your web browser. If you complete the
survey, you are communicating your consent to contribute your opinions to the broader pool of data that will be used to inform
our district practices going forward.

We also want you to know that the information you are submitting to the HWDSB is protected under the Municipal Freedom of
Information and Protection of Privacy Act (MFIPPA). By law, neither the HWDSB, nor any agent thereof, can publish or
disseminate this information for any purpose other than that which it was intended and as outlined (i.e., for quality assurance
purposes; to receive feedback from parents about our strengths and areas for improvement, to enhance school district
programming and services).

The survey should take 10-15 minutes to complete. There are no right or wrong answers. The results from all parent surveys will
be grouped together and only combined findings will be shared (by type of school – elementary or secondary and as a board).
We are interested in hearing your opinions and suggestions. Please complete the survey by June 30, 2009.
