Gentlemen: This is to officially inform you that we have been requested by your Insurers, the PIONEER INSURANCE & SURETY CORPORATION to handle the investigation and adjustment of the above-captioned loss/claim. In order for us to facilitate the evaluation of your claim, we request for your submission of the following claim supporting documents, to wit: A. GENERAL REQUIREMENTS 1. Duly accomplished and signed Sworn Statement of Loss and NonWaiver Agreement, forms enclosed; 2. Business Licenses and Permits to engage in business; 3. Bureau of Internal Revenue (BIR) VAT or Non-VAT Registration Certificate; 4. Secretarys Certificate as to who will represent the company in connection with this claim; 5. Incident Report regarding the circumstances that leads to the loss; 6. Summary of Claim prepared by your goodselves; and 7. Photographs of the affected insured properties.

Page 2 Our File Nos.: 9198F/JGD Contd B. STOCKS CLAIM 1. Copy of the Actual Count Sheet of the last physical inventory taken prior to the loss; 2. Complete inventory of all the affected stocks contained inside the premises as of the date of loss with corresponding description, quantity, unit cost and totals; 3. Copy of Purchase Invoices, Official Receipts and/or any documents in support of the unit costs applied on the above 4. Price list of all stocks as of the date of loss; 5. Inventory of merchandise/stocks filed with the Bureau of Internal Revenue for the last three (3) years prior to the date of loss;; 6. Audited Financial Statements filed with the Bureau of Internal Revenue for the last three (3) years prior to the loss with accompanying Income Tax Return; 7. Purchase Invoices, Delivery Receipts and etc., in support of the deliveries made for the period January1 to date of loss; 8. Sales Invoices, Officials Receipts and etc., in support of the sales made for the period January 1 to date of loss; 9. Commercial Invoices, Bill of lading and/or Importation documents for imported stocks, if there is any; 10. A written statement as to the companys position whether to retain the affected stocks or surrender the same to Insurers for recovery, if there is any. If decision is to retain, please include your offer for Insurers consideration and approval. Our evaluation of your claim will immediately be undertaken upon receipt from you of the above-cited documents. In addition to the above requested documents, we wish to invite your attention to Policy Condition No. 13, to wit: The insured shall give immediate written notice to the Company of any loss, protect property from further damage, forthwith separate the damaged and undamaged personal property, put it in the best possible order, furnish a complete inventory of the destroyed, damaged and undamaged property, showing in detail quantities, costs, actual cash value and the amount of loss claimed; AND WITHIN SIXTY DAYS AFTER THE LOSS, UNLESS SUCH TIME IS EXTENDED IN WRITING BY THE COMPANY, THE INSURED SHALL RENDER TO THE COMPANY A PROOF OF LOSS, signed and sworn to by the insured. Stating the knowledge and belief of the insured as to the following: the time and origin of the loss, the interest of the insured and of all others in the property, the actual cash value of each item thereof and the amount of loss thereto, all encumbrances thereon,

Page 3 Our File Nos.: 9198F/JGD Contd all other contracts of insurance, whether valid or not covering any of said property, any changes in the title, use, occupation, location, possession or exposures of said property since the issuing of this policy, by whom and for what purpose any buildings herein described and the several parts thereof were occupied at the time of loss and whether or not in then stood on leased ground. and shall furnish a copy of all the descriptions and schedules in all policies and, if required verified plans and specifications of any building, fixtures or machinery destroyed or damaged. The insured, as often as may be reasonably required, shall exhibit to any person designated by the Company all that remains of any property herein described, and submit to examination under oath by any person named by the Company, and subscribed the same; and, as often as may be reasonably required, shall produce for examination all books of account, bills, invoices, and other vouches, or certified copies thereof if originals be loss, at such reasonable time and place as many be designated by the Company or its representative, and shall permit extracts and copies thereof to be made. Should we, however, in the course of our evaluation of your claim find a need to request for additional documents and information to further substantiate the claim, your goodselves would be advised accordingly. This letter should not be construed as an admission of liability nor as a waiver of any rights and privileges your Insurers has under the policy and at this stage, we act without prejudice to Policy Liability.
