How to use the summary rate tables tool

Overview
This tool allows you to record your OSHA log summary numbers, and save and display your firms' incidence
rates and your benchmark incidence rates. Copy this file to your own computer and use it to keep track of your
OSHA log statistics.

The incidence rate calculations and an overview of the Burea of Labor Statistics Web tool, Incidence rate
calculator and comparison tool, are described in the Recordkeeping 101, Part 7 article published in the summer
2006 edition of Safety Lines. It is available on the Web at http://www.doli.state.mn.us/pdf/safetylin-sum06-
rcd7.pdf.

You need to complete the OSHA log summary (OSHA form 300A) before you begin.

Enter the number of hours worked by all employees and the number of cases from the OSHA log summary on
the Hours and Cases worksheet. Then use this information with BLS incidence rate calculator tool to obtain
incidence rates. Copy the results table from the incidence rate calculator to the Data from BLS tool worksheet.
Copy the incidence rates into the Data Tables worksheet. The rates will automatically be entered into the
incidence rate trend charts on the Charts worksheet.
