Project 2: Revised Patient Hand Off Report


Reviewed current literature regarding various report styles-One on One, Bedside, Unit Surveyed staff to determine satisfaction levels with current report style/report sheet & preferred report style

90% felt having a report sheet was helpful 95% felt we should revise the report sheets

Some of the responses regarding sheets:


shorten the report sheets, put more than 1 patient on a sheet (to save paper) I think it's important that there is a place where we can put important patient information that does not get lost between change of shifts (hence the report sheet is still useful for this). However, there needs to be a way to shorten the report sheet and save some paper. Stick to basics and updates. Way too much info on paper Need to know what is their treatment and goal for discharge etc. I like the idea of the MD handoff and place nursing issues in the nursing section. but this too generates more paper. We could start with one to one and still use the reports sheets until we can come up with a report that transitions to our other needs. But the one on one is the best way for now to give report. Review of systems Not in excel --too cumbersome. Follow the handoff notes of thayer team with things to be aware of for coming shift. Short narrative from MD note so we're on the same page. Educational needs.

Reviewing current literature regarding standardized acuity scales and the benefit they offer Will research various types of standardized acuity scale Will survey staff with regard to different aspects of standardization
