This has reference to your mail giving the details of the employees and their families to be covered under the Tailor-made Floater Group Mediclaim policy for the year 2013-2014. Policy coverage

Dependents covered: Self+Spouse+2 dependent Children+2 Depedents parents. (1+5). As per our standard new Group Tailor-made Mediclaim Policy 2007 with Waiver of pre-existing diseases. Waiver of 1st yr/2nd /4th year exclusions. Waiver of 30 days waiting period. Maternity cover: 1. Maternity cover restricted to 10% of sum insured or Rs. 50,000/- for normal and 20% of sum insured or Rs. 50,000/- for C-Section. Waiver of 09 months waiting for maternity benefit with baby day 1 coverage within full sum insured. 2. Pre and Post hospitalization coverage of 30 and 60 days respectively. 3. All treatments like Age Related Mascular Degenertion (ARMD) as per new rules. 4. Ambulance Charges: Rs. 2000/- in emergency admission into ICU. 5. Room rent restricted to 1% of sum insured for normal admission and 2% of sum insured for I.C.U.

6. Other conditions, clauses, limits sublimits etc as per new Mediclaim 2007. 7. The amount payable for any cataract surgery will be limited to actual or Rs. 24,000/Sum Insured. All treatments like Age Related macular Degeneration (ARMD) and/or Chorodial Neo Vascular Membrance done by administration of Lucentis/ Avantis/ Macugen /Avastin and other related drugs as intravertal injection, Rotational Field Quantum Magnetic Resonance (RFQMR), External Counter Pulsation (ECP), and Hyperberic Oxygen Therapy are excluded under this policy.
