Raenell Curry Ciproflaxin (Cipro, Cipro IV, Cipro XR, Proquin XR, Ciloxan) Drug Card

Medication Name: Ciproflaxin (Cipro, Cipro IV, Cipro XR, Proquin XR, Ciloxan) Pharmacologic Class: Quinolone Antibiotic Mechanism of Action: Inhibits DNA-gyrase and some aspects of transcription, repair, recombination, and transcription. Therapeutic Action: Broad spectrum antibacterial agent effective against many gram-positive and aerobic gram-negative organisms 3 Reasons ordered for patients: 1. Treat infections of the urinary tract, lower respiratory tract, skin & skin structures, bone & joint, GI, infectious diarrhea, chronic bacterial prostatitis, nosocomial pneumonia, bacterial conjunctivitis, typhoid fever, acute cystitis, and acute sinusitis. 2. Treat inhalation anthrax 3. Treat corneal ulcers (ophthalmic: Ciloxan) 3 Side effects: 1. Local burning and discomfort 2. Seizures 3. Photosensitivity 1. Calcium/ dairy products: Decreases the levels of Cipro

3 Medication/food interactions: and delays drug absorption.

2. Antacids, didonosine, iron supplements, sucralfate, and multivitamins containing iron or zinc: Decreases absorption of Cipro. 3. Oral anticoagulants: Cipro enhances anticoagulant effects and increases prothrombin time (PT) for patients. 4. Caffeine: causes CNS stimulation. 3 Nursing actions/administration: 1. Assess patient frequently, every 2 hours, for diarrhea because this may indicate pseudomembranous colitis. If the patient develops diarrhea notify physician and expect to withhold Cipro and treat the diarrhea, especially if it has blood in it. 2. Culture and sensitivity tests should be done before initial dose because treatment may be changed depending on results. These should be done the morning before or immediately depending on severity. As the nurse we are to make sure these are done before administering the infusion. 3. Assess urine pH daily,(it should be less than 6.8) assessing urine pH values daily you reduce the risk of the patient developing crystalluria. If pH is higher than 6.8 notify physician and prepare to withhold Cipro.

4. Dilute in NS or D5W to a final concentration of 0.5-2 mg/mL, if unable to start another IV site temporarily discontinue other IV infusions while infusing Cipro or choose another peripheral infusion site because this drug is incompatible with many other infusions. 5. Assess patient for signs or evidence of tendon rupture which may occur during or after Cipro therapy. Notify Physician and have patient rest and refrain from exercise until tendon rupture is ruled out.

3 Client education needs 1. Take Cipro 2 hours before or 6 hours after an antacid. Also do not take drug with dairy products, or calcium fortified juices. The calcium will decrease the levels of Ciproflaxin and delay drug absorption. 2. Be sure to maintain a fluid intake of 2-3 L/day to prevent alkaline urine which could lead to crystalluria or nephrotoxicty and restrict caffeine because it causes nervousness, insomnia, anxiety, and tachycardia. Notify physician if any of these occur. 3. Use adequate sunscreen and avoid excessive sunlight or UV lights to prevent severe sunburn. If this occurs notify physician and Cipro may be stopped.
