Bringing Our Values to Life


It gives me enormous pride to launch our third Celebrating Excellence Awards. Every day in the NHS dedicated teams and individuals go to extraordinary lengths to provide exceptional care and fantastic services. If you know someone who truly represents these values, goes above and beyond the call of duty and demonstrates excellence on a daily basis then please nominate them for an award. This is your opportunity to give special individuals the recognition they deserve for their outstanding contribution. So if you are a patient, a carer, a relative, a partner organisation or a member of staff and want to acknowledge and celebrate some of the excellent practice that often goes unsung in the health service then this is your chance! I was delighted and impressed with the enthusiasm shown by staff last year in identifying worthy nominees. The awards were of an exceptionally high standard and I have no doubt that we will receive the same standard of entries this year. I particularly look forward to receiving the nominations from members of the public as I thoroughly enjoy reading all the thank you cards and compliments we receive and once again the difficult job we have to select the winners! We have a lot to be proud of as an organisation. This years awards offer twelve categories, five new awards and the new public vote. So dont waitnominate today to say thank you to someone for making a difference.

The Partnership Trust wants to continually improve and provide the best care and services and our great teams are the key to success. We are looking for two teams, one in support services and another delivering front line care, that consistently demonstrate working Better Together to successfully introduce changes and improvements for the benefit of patients and services users.

Dedication to Service Award*

The Partnership Trust believes in working more closely with partners to improve quality and access to services, personalised care or treatment, a more positive experience and improved outcomes. This award is presented to a team or individual that has exemplified partnership working and the values of the Trust and wider NHS by working across boundaries to improve services for our service users and/or their families.

Working in Partnership Award 

Our volunteers make a huge contribution in helping the Partnership Trust provide great care and services. Working in our hospitals, supporting cancer patients or helping people at home, this awards recognises an individual volunteer or a team who have a made an outstanding commitment and exceptional experience for service users and/or their families.

Bringing new ideas into practice can have a positive impact on the people who use our services, improving satisfaction and/or quality of life. Through service redesign or by improving efficiency our staff work in innovative ways every day. Nominate a colleague or team whose positive and innovative approach has seen their idea successfully implemented to make a positive difference.

Innovation Award 

Whether newly qualified or new to a post, a fresh perspective can bring great benefits to our Trust. Do you know someone who performs exceptionally and continually makes improvements by developing and learning? Tell us about a colleague who shows real drive, exceptional performance, enthusiasm, motivation and inspires those around them.

Do you work with someone who has developed and successfully implemented a new or improved service, implemented high quality clinical research which promotes recovery and wellbeing and puts quality improvement at the heart of what they do to transform patient care? Who do you know who has created an excellent service nominate them today!

 eveloping Excellent Services D Award*

This special award is presented to an individual or team who has made a real difference to the personal experiences of service users, carers or colleagues by living the Trusts values. Overall they enhance the reputation of their team and the organisation and their words and actions make a real and tangible difference.  Apprentice of the Year Award* This award recognises the apprentice who has gone beyond expectations in delivering measurable benefits in their department/service area; introduced a new idea or initiative that has a positive impact on the service, for example doing something that has improved procedures and systems; and demonstrated outstanding commitment, enthusiasm and determination to personal development and progression through their learning.

Top Tips For Completing Your Form


If you want your chosen individual or team to win an award, you have to convince the judges that they stand out from the others...read these top tips to help you in writing your nomination
G  et to know the Trusts values; dont forget the judges will be looking for evidence of these in all categories. Consider all the categories available and choose the most appropriate  one for your nomination. B  oast about your nominees achievementsstart your application with lots of positives. Your opening words are crucial in keeping the attention of the judges when theyre reading your nomination. M  ake sure your nomination is written clearly and concisely and keep it jargon free. Remember, the judges may not always have a full understanding of trust or clinical jargon. G  ive full details of the achievements, benefits, improvements etc your nominee has made to the Trust, its service users/carers or staff. R  ead your nomination out loud. This will help you identify any spelling and grammatical errors as well as giving a chance to check it for clarity. A  sk someone else outside your service/team to read the nomination for clarity, spelling and grammatical errors. Its very difficult to spot your own mistakes, especially when youve written several drafts. D  ont forget to ask for help.

Everything You Need To Know


Key Dates T  he closing date for nominations is 5pm on 20th December 2013 *Please note that nominations will not be accepted after this date so please ensure you submit on time! Judging will take place on 14th January 2014. A  ll shortlisted nominations will be contacted week commencing 27th January 2014 to arrange filming and photographs. Forms To access the online nomination form, award criteria and further information about the awards please visit: www.staffordshireandstokeontrent.nhs.uk H  ard copies will also be available at bases throughout the Trust and can be returned by email to celebratingexcellenceawards@ssotp.nhs.uk or by post to Samantha Quarmby at Longton Cottage Hospital. Nominations should reflect work or achievements within the last 12 months.   n your nomination forms please make sure that you use the official team name, O location etc and that individuals names are spelt correctly, as this information is used for promotional brochures and on the awards. Receipt of entries will be acknowledged and everyone who is nominated will  receive a letter advising them of their nomination. General Information The awards are open to individuals who work for Staffordshire and Stoke on Trent  Partnership NHS Trust.  he judging panel will include the Chief Executive, Chair, Non Executive Directors T and Staff Side Representative.  ll shortlisted staff or teams will receive a certificate of recognition after the A awards ceremony. Award winners will be announced at the evening ceremony on  Friday 14th March 2014 and will receive an award.

Winning the award was wonderful and has raised the profile of rehabilitation work for Deaf and Deafblind people.
