THESTATEMEDICALBOARDOFOHIO

ThisConsentAgreementis entered into by and betweenRichard S. Skoblar,M.D., [Dr. Skoblar],
and the StateMedicalBoardofOhio [Board],astateagencycharged withenforcingChapter4731.,
OhioRevised Code.
Dr. SkoblarentersintothisConsentAgreementbeing fully informedofhis rightsunderChapter
119.,OhioRevisedCode, includingthe rightto representation by counselandthe rightto a formal
adjudicativehearingon the issuesconsidered herein.
BASISFORACTION
ThisConsentAgreement isentered intoon the basisofthefollowing stipulations, admissionsand
understandings:
A. TheBoard isempowered by Section4731.22(B), OhioRevised Code,to limit, revoke,
suspendacertificate, refuseto registerorreinstatean applicant, orreprimand orplaceon
probationthe holderof acertificatefor violationofSection4731.22(B)(26), Ohio
RevisedCode, for "impairmentof abilityto practiceaccordingto acceptableand
prevailingstandardsof carebecauseofhabitualorexcessiveuseorabuseof drugs,
alcohol,orothersubstancesthatimpairabilityto practice,"and for aviolationofSection
4731.22(B)(15), OhioRevised Codefor a"violationoftheconditionsoflimitation
placed by the Board uponacertificateto practice."
B. The Board enters into this Consent Agreement in lieu offurther formal proceedings
based upon the allegations set forth in the Notice of Summary Suspension and
Opportunity for Hearing issued on September 8, 2010, attached hereto as Exhibit A and
incorporated herein bythis reference. TheBoard expresslyreservesthe rightto institute
additional formal proceedings based upon any other violations ofR.C. Chapter 4731.,
whetheroccurringbeforeoraftertheeffectivedateofthisConsentAgreement.
C. Dr. Skoblaris licensed to practicemedicineandsurgery in the Stateof Ohio,License
number35.038099.
O. Dr.Skoblarstatesthathe is not licensed to practicein anyotherstateorjurisdiction.

AGREED CONDITIONS
Wherefore, in considerationoftheforegoing and mutual promiseshereinaftersetforth, and in lieu
ofany formal proceedingsatthistime, Dr. Skoblarknowinglyand voluntarilyagreeswiththe
Boardto thefollowingterms, conditionsand limitations:
SUSPENSION OF CERTIFICATE
1. ThecertificateofDr. Skoblarto practicemedicineand surgeryin the StateofOhiowas
summarilySUSPENDEDeffectiveSeptember8, 2010, pursuantto theNoticeof
SummarySuspensionandOpportunityforHearingattached hereto asExhibitA. The
summarysuspensionofDr. Skoblar'scertificatetopracticemedicineand surgery in the
StateofOhioeffective September8, 20Ishall be terminatedandsuperseded uponthe
instantConsentAgreementbecomingeffective. Further,Dr. Skoblar'scertificateto
practicemedicineandsurgery in the StateofOhioshall be SUSPENDEDforan
indefiniteperiod, butnotlessthanthreeyearsfrom theeffectivedateofthisConsent
Agreement.
Obeyall Laws
2. Dr. Skoblarshallobeyall federal, state,and local laws.
Sobriety
3. Dr. Skoblarshallabstaincompletelyfrom the personaluseorpersonal possessionof
drugs, exceptthoseprescribed,dispensed oradministered to himbyanotherso
authorized by law whohasfull knowledgeofDr. Skoblar'shistoryofchemical
dependency. Further, in theeventthatDr. Skoblaris so prescribed,dispensed or
administeredanycontrolled substance,carisoprodol, ortramadol, Dr. Skoblarshall
notifytheBoard in writingwithinsevendays, providingtheBoardwiththe identityof
the prescriber;thenameof thedrugDr. Skoblarreceived;themedical purposefor which
hereceived saiddrug; thedatesuchdrugwas initiallyreceived;and thedosage, amount,
numberofrefills, and directionsfor use. Further,within thirtydaysof thedate saiddrug
is so prescribed,dispensed,oradministered to him, Dr. Skoblarshall providethe Board
witheitheracopyof thewrittenprescriptionorotherwrittenverificationfrom the
prescriber, includingthedosage,amount, numberofrefills, anddirectionsfor use.
4. Dr. Skoblarshall abstaincompletelyfrom theuseofalcohol.

5. Dr. Skoblarshallobtainpermissionfrom theBoardfor departuresorabsencesfrom
Ohio. Suchperiodsofabsenceshall not reducethe probationaryterm, unlessotherwise
determinedbymotionoftheBoardfor absencesofthreemonthsorlonger, orby the
Secretaryorthe SupervisingMemberoftheBoardfor absencesoflessthan three
months, in instanceswheretheBoardcan beassured thatprobationarymonitoring is
otherwisebeingperformed. Further,the Secretaryand SupervisingMemberofthe
Board shall have thediscretionto grantawaiverofpartorallofthemonitoringtermsset
forth in thisConsentAgreementfor occasionalperiodsofabsenceoffourteen daysor
less. In theeventthatDr. Skoblarresidesand/oris employedata locationthat iswithin
fifty milesofthegeographic borderofOhioand anyofits contiguousstates,Dr. Skoblar
maytravel betweenOhioand thatcontiguousstatewithoutseekingpriorapproval ofthe
SecretaryorSupervisingMemberprovidedthatDr. Skoblaris ableto otherwise
maintainfull compliancewith all otherterms,conditionsand limitationssetforth in this
ConsentAgreement.
Releases; QuarterlyDeclarationsand Appearances
6. Dr. Skoblarshall provideauthorization, throughappropriatewrittenconsentforms, for
disclosureofevaluativereports, summaries, and records, ofwhatevernature, by anyand
all partiesthat providetreatmentorevaluationforDr. Skoblar'schemicaldependencyor
related conditions,orfor purposesofcomplyingwiththisConsentAgreement, whether
suchtreatmentorevaluationoccurred beforeoraftertheeffectivedateof thisConsent
Agreement. Totheextentpermitted by law, theabove-mentioned evaluativereports,
summaries,and recordsareconsidered medicaJ recordsforpurposesofSection 149.43
oftheOhioRevisedCodeand areconfidentialpursuantto statute. Dr. Skoblarfurther
agreesto providetheBoardwrittenconsentpermittingany treatmentproviderfrom
whom he obtainstreatmentto notifY theBoard in the eventhe fails to agreeto orcomply
withanytreatmentcontractoraftercarecontract. Failureto providesuchconsent, or
revocationofsuchconsent, shallconstituteaviolationofthisConsentAgreement.
7. Dr. Skoblarshall submitquarterlydeclarationsunderpenaltyof Boarddisciplinary
actionand/orcriminalprosecution, statingwhethertherehasbeen compliancewithall
theconditionsofthisConsentAgreement. Thefirst quarterlydeclaration must be
received in theBoard'sofficesonthefirstday ofthethird monthfollowingthemonth in
whichthis ConsentAgreementbecomeseffective, oras otherwiserequested by the
Board. Subsequentquarterlydeclarationsmustbe received in theBoard'sofficesonor
beforethefirst day ofevery third month.
8. Dr. Skoblarshallappearin personfor an interview beforethe full Boardorits
designated representativeduringthethird monthfollowing theeffectivedateof this
ConsentAgreement. Subsequentpersonalappearancesmustoccureverythreemonths
thereafter, and/orasotherwiserequested by theBoard. Ifan appearance is missed oris

rescheduledfor anyreason, ensuingappearancesshall bescheduled based on the
appearancedate asoriginallyscheduled.
Drug& Alcohol Screens; DrugTestingFacilityand CollectionSite
9. Dr. Skoblarshallsubmitto random urine screeningsfor drugsand alcohol at leastfour
timespermonth, orasotherwisedirected bytheBoard. Dr. Skoblarshallensurethatall
screeningreportsareforwarded directlyto theBoardonaquarterly basis. Thedrug
testingpanel utilized mustbeacceptableto theSecretaryof theBoard,andshall include
Dr. Skoblar'sdrug(s) ofchoice.
Dr. Skoblarshallabstain from theuseofanysubstanceandtheconsumptionofpoppy
seedsoranyotherfood orliquid thatmayproducea low level positiveresult in a
toxicologyscreen. Dr. Skoblaracknowledgesthat he understandsthattheconsumption
oruseofsuchsubstances, including butnot limitedtosubstancessuchasmouthwashor
hand cleaninggel, maycauseapositivedrugscreenthatmay notbeabletobe
differentiatedfrom intentional ingestion, and thereforesuchconsumptionoruse is
prohibitedunderthisConsentAgreement.
Al1 such urinescreeningsfor drugsand alcohol shall beconductedthroughaBoard-
approveddrugtesting facility and collectionsite pursuanttotheglobalcontractbetween
said facilityandtheBoard,thatprovidesfor theBoardto maintainultimatecontrolover
theurinescreeningprocessand to preservetheconfidentialityofall positivescreening
results in accordancewithSection473l.22(F)(5),OhioRevised Code, and thescreening
processshall requireadailycall-in procedure. Further, in theeventthattheBoard
exercises its discretion, asprovided in Paragraph lO below,to approveurinescreenings
tobe conductedatan alternativedrugtestingfacilityand/orcol1ection siteora
supervising physician, suchapprovalshall be expresslycontingentupontheBoard
retainingultimatecontrolovertheurinescreeningprocess in amannerthatpreservesthe
aforementionedconfidentialityofall positivescreeningresults.
Dr. Skoblarshall submit, athisexpenseand on the dayselected, urinespecimensfor
drugand/oralcohol analysis. All specimenssubmitted byDr. Skoblarshall be negative,
exceptfor thosesubstances prescribed, administered, ordispensedtohim in
conformancewiththeterms, conditionsand limitationssetforth in thisConsent
Agreement. Refusalto submitsuchspecimen,orfailure to submitsuchspecimenonthe
day he is selectedorin suchmannerastheBoardmay request, shallconstitutea
violationofthisConsentAgreement.
Further, withinthirty days ofthe effectivedateofthisConsentAgreement,Dr. Skoblar
shallenterintothe necessaryfinancial and/orcontractualarrangementswiththeBoard-
approved drugtesting facility and/orcollectionsite in orderto facilitate theurine
screeningprocess in themannerrequired by this ConsentAgreement. Further, Dr.
Skoblarshall promptly provideto theBoardwrittendocumentationofcompletionof
sucharrangements, includingacopyofanycontractentered into betweenDr. Skoblar

and theBoard-approveddrugtestingfacilityand/orcollectionsite. Dr. Skoblar'sfailure
to timelycompletesucharrangements,orfailure to timely providewritten
documentationto the Board of completionofsucharrangements,shallconstitutea
violationofthisConsentAgreement.
Dr. Skoblarshallensurethattheurinescreeningprocessperformed throughtheBoard-
approved drugtesting facility and/orcollectionsiterequiresadailycall-in procedure;
thattheurinespecimensareobtained onarandom basis; and thatthegivingofthe
specimenis witnessed by areliableperson. In addition, Dr. Skoblarand theBoard-
approveddrugtestingfacilityand collectionsiteshallassurethatappropriatecontrol
overthespecimen is maintained and shall immediatelyinform theBoardof anypositive
screeningresults.
Dr. SkoblarshallensurethattheBoard-approveddrugtestingfacilityand/orcollection
site providesquarterlyreportsto theBoard, in a formatacceptableto theBoard,
verifyingwhetherall urinescreenshavebeenconducted in compliancewiththis Consent
Agreement, and whetherall urinescreens have beennegative.
In theeventthattheBoard-approveddrugtestingfacility and/orcollectionsite becomes
unableorunwillingto serveas required bythis ConsentAgreement, Dr. Skoblarmust
immediatelynotifytheBoard in writing, and makearrangementsacceptableto the
Board, pursuanttoParagraph 10 below, as soonas practicable. Dr. Skoblarshall further
ensurethattheBoard-approveddrugtestingfacilityand/orcollection sitealso notifies
the Board directlyofits inabilityto continueto serveandthereasonstherefore.
Dr. SkobJaracknowledgesthattheBoardexpresslyreservestherighttowithdraw its
approvalof anydrugtestingfacility and/orcollection sitein theeventthattheSecretary
and SupervisingMemberoftheBoarddeterminethatthedrugtesting facility and/or
collectionsitehasdemonstrateda lackof cooperation in providing informationto the
Boardorfor anyotherreason.
10. Dr. Skoblarand theBoard agreethatit is the intentofthisConsentAgreementthatDr.
Skoblarshall submithis urinespecimenstotheBoard-approveddrugtestingfacility and
collectionsitechosenbytheBoard. However, in theeventthatutilizingsaid Board
approveddrugtesting facility and/orcollectionsitecreatesan extraordinaryhardship
uponDr. SkobJar, as determined in thesolediscretionoftheBoard,thensubjectto the
following requirements,theBoard mayapprovean alternatedrugtestingfacility and/or
collectionsite, orasupervisingphysician,to facilitatethe urinescreeningprocessforDr.
Skoblar:
a. Withinthirtydaysofthedateupon whichDr. Skoblaris notified of the Board's
determinationthatutilizingtheBoard-approveddrugtestingfacilityand/or
collectionsiteconstitutesan extraordinaryhardship uponDr. Skoblar, he shall
submitto theBoard in writingfor its priorapprovalthe identityof eitheran
alternatedrugtestingfacility and collectionsite, orthe nameof aproposed

supervisingphysician,to whomDr. Skoblarshallsubmittherequired urine
specimens. In approvinga facility, entity,oran individual to serve in thiscapacity,
theBoard will givepreferenceto a facility located nearDr. Skoblar'sresidenceor
employmentlocation, ortoa physicianwho practices in thesamelocaleasDr.
Skoblar. Dr. Skoblarshallensurethattheurinescreeningprocess performed
throughthealternate drugtestingfacility and/orcollection site, orthroughthe
supervisingphysician,requiresadaily call-inprocedure;thatthe urine specimens
areobtained onarandom basis;andthatthegivingofthespecimen is witnessed by
a reliableperson. In addition,Dr. Skoblaracknowledgesthatthe alternatedrug
testingfacility and collection site, orthesupervisingphysician, shall assurethat
appropriatecontroloverthespecimen is maintained and shall immediately inform
theBoardofany positivescreeningresults.
b. Dr. Skoblarshallensurethatthealternatedrugtestingfacility and/orcollection
site, orthesupervisingphysician, providesquarterly reportsto theBoard, in a
formatacceptableto theBoard,verifyingwhetherall urinescreenshave been
conducted in compliancewiththisConsentAgreement,and whetherall urine
screenshavebeennegative.
c. Intheeventthatthedesignated alternatedrugtestingfacility and/orcollectionsite,
orthesupervisingphysician,becomesunableorunwilling to so serve,Dr. Skoblar
must immediatelynotifytheBoard in writing. Dr. Skoblarshallfurther ensurethat
the previouslydesignated alternatedrugtestingfacility andcollectionsite, orthe
supervisingphysician,also notifiestheBoarddirectly ofthe inability to continueto
serveand thereasonstherefore. Further, in orderto ensurethattherewill be no
interruption in hisurinescreeningprocess,uponthepreviouslyapproved alternate
drugtestingfacility, collectionsite, orsupervisingphysicianbecomingunableto
serve,Dr. Skoblarshall immediatelycommenceurinescreeningattheBoard-
approved drugtestingfacility and collectionsitechosen by theBoard, until such
time, ifany,thattheBoardapprovesasubsequentalternatedrugtestingfacility,
collectionsite, orsupervisingphysician, ifrequested by Dr. Skoblar.
d. TheBoard expressly reservesthe rightto disapproveanyentityorfacility proposed
to serveas Dr. Skoblar'sdesignatedalternatedrugtestingfacility and/orcollection
site, oranypersonproposedto serveashis supervisingphysician,orto withdraw
approval ofany entity, facility orperson previouslyapproved to so servein the
eventthattheSecretaryand SupervisingMemberoftheBoarddeterminethatany
suchentity, facility orpersonhasdemonstrated a lackofcooperation in providing
informationto theBoard orforanyotherreason.
11. All screeningreportsrequired underthisConsentAgreementfrom theBoard-approved
drugtestingfacility and/orcollectionsite, orfrom thealternatedrugtestingfacility
and/orcollectionsiteorsupervisingphysician,mustbereceived in theBoard'soffices

no laterthantheduedate for Dr. Skoblar'squarterlydeclaration. Itis Dr. Skoblar's
responsibilityto ensurethatreportsare timelysubmitted.
12. TheBoardretainstherightto require, and Dr. Skoblaragreesto submit, blood, urine,
breath, salivaand/orhairspecimensfor screeningfor drugsandalcohol, for analysisof
therapeutic levelsofmedicationsthatmay be prescribed for Dr. Skoblar,orfor any other
purpose,atDr. Skoblar'sexpenseupontheBoard'srequestand withoutpriornotice. Dr.
Skoblar'srefusalto submitaspecimenupon requestoftheBoardshallresult in a
minimumof oneyearofactual licensesuspension. Further, thecollectionofsuch
specimensshall bewitnessed by a representativeof the Board,oranotherperson
acceptabletotheSecretaryorSupervisingMemberof theBoard.
RehabilitationProgram
13. WithinthirtydaysoftheeffectivedateofthisConsentAgreement,Dr. Skoblarshall
undertakeand maintainparticipation in an alcohol anddrugrehabilitationprogram, such
asA.A.,N.A., C.A., orCaduceus, no lessthanthreetimesperweek. Substitutionofany
otherspecificprogrammustreceivepriorBoardapproval.
Dr. Skoblarshallsubmitacceptabledocumentaryevidenceofcontinuingcompliance
withthisprogram, includingsubmissiontotheBoard ofmeetingattendancelogs, which
must be received in theBoard'sofficesno laterthantheduedatefor Dr. Skoblar's
quarterlydeclarations.
14. Immediatelyupon completionofanyrequired treatmentfor chemicaldependency,Dr.
Skoblarshall enter intoan aftercarecontractwithaBoard-approvedtreatmentprovider
and shall maintaincontinuedcompliancewiththetermsofsaidaftercarecontract,
provided that, wherethetermsof theaftercarecontractconflictwiththetermsofthis
ConsentAgreement,thetermsofthisConsentAgreementshallcontrol.
CONDITIONS FOR REINSTATEMENT
15. TheBoard shall notconsiderreinstatementorrestoration ofDr. Skoblar'scertificateto
practicemedicineand surgeryuntil all ofthefollowing conditionsare met:
a. Dr. Skoblarshall submitan applicationforreinstatementorrestoration, as
appropriate, accompanied by appropriatefees, ifany.
b. Dr. Skoblarshalldemonstratetothesatisfactionof theBoardthathecan resume
practice in compliancewithacceptableand prevailingstandardsofcareunderthe
provisionsof hiscertificate. Suchdemonstrationshall include but shall not be
limited to the following:
i. Certificationfrom atreatmentproviderapprovedunderSection4731.25 of
theRevisedCodethatDr. Skoblarhassuccessfullycompletedany required

inpatienttreatment, includingat leasttwenty-eightdaysofinpatientor
residential treatmentfor chemicalabuse/dependence, as setforth in Rules
4731-16-02and 4731-16-08,OhioAdministrativeCode,completed
consecutively.
ii. Evidenceof continuingfull compliancewith, orsuccessfulcompletionof, a
post-dischargeaftercarecontractwithatreatmentproviderapprovedunder
Section4731.25 oftheRevised Code. Suchevidenceshall include, butnot
be limited to, acopyofthesigned aftercarecontract. Theaftercarecontract
mustcomplywith rule4731-16-10ofthe AdministrativeCode.
III. Evidenceofcontinuingfull compliancewith thisConsentAgreement.
iv. Twowrittenreports indicatingthatDr. Skoblar'sabilityto practicehasbeen
assessed and thathehas beenfound capableofpracticingaccording to
acceptableand prevailingstandardsofcare. Thereports shall be made by
physiciansknowledgeable in theareaofaddictionologyandwhoareeither
affiliated withacurrentBoard-approvedtreatmentproviderorotherwisehave
beenapproved in advance by theBoard to providean assessmentofDr.
Skoblar. Further,thetwo aforementioned physiciansshall not beaffiliated
withthesametreatmentproviderormedicalgroup practice. Priorto the
assessments, Dr. Skoblarshall providetheevaluatorswith copiesofpatient
recordsfrom anyevaluationsand/ortreatmentthathe hasreceived, and a
copyofthisConsentAgreement. Thereportsfrom theevaluatorsshall
includeanyrecommendationsfortreatment, monitoring,orsupervisionof
Dr. Skoblar,andanyconditions, restrictions, or limitationsthatshould be
imposed onDr. Skoblar'spractice. Thereportsshall alsodescribethebasis
for theevaluator'sdeterminations.
All reportsrequired pursuantto thisparagraphshall be basedupon
examinationsoccurringwithinthethreemonths immediatelyprecedingany
applicationfor reinstatement. Further,atthediscretionof theSecretaryand
SupervisingMemberoftheBoard,theBoardmayrequestan updated
assessmentand report if theSecretaryand SupervisingMemberdetermine
thatsuchupdated assessmentand report is warranted for anyreason.
v. In theeventthattheBoard initiatesfuture formal proceedingsagainstDr.
Skoblar, including butnot limited to issuanceofaNoticeof Opportunityfor
Hearing, Dr. Skoblarshall be ineligiblefor reinstatementuntil such
proceedingsarefully resolved byratification by theBoardof a subsequent
ConsentAgreementor issuance by theBoardof afinal BoardOrder.
c. Dr. Skoblarshall enterinto awrittenconsentagreementincludingprobationary
terms, conditionsand limitationsas determined by theBoardwithin 180daysof
thedateuponwhichall theabove-specified conditionsfor reinstatementor

restoration havebeencompleted or, iftheBoardand Dr. Skoblarareunableto
agreeonthetermsofawrittenConsentAgreement,thenDr. Skoblarfurtheragrees
to abide by anyterms, conditionsand limitations imposed byBoardOrderaftera
hearingconducted pursuantto Chapter 119.oftheOhioRevised Code. TheBoard
shall providenoticeto Dr. Skoblarthatsaid hearinghasbeenscheduled,advising
Dr. Skoblarofhishearing rights, and statingthedate,time, and locationofthe
hearingatwhich theBoard will presentitsevidence, afterwhichtheBoardwill
makeadeterminationofthematterbyBoardOrder.
Further, upon reinstatementofDr. Skoblar'scertificateto practicemedicineand
surgery in thisstate,theBoardshallrequire continued monitoringwhichshall
include, butnot be limitedto, compliancewiththewritten consentagreement
entered into beforereinstatementorwithconditions imposed by BoardOrderafter
a hearingconductedpursuantto Chapter 119. oftheRevised Code. Moreover,
uponterminationoftheconsentagreementorBoard Order,Dr. Skoblarshall
submitto theBoard for at leasttwoyearsannualprogressreportsmade under
penaltyofBoard disciplinaryactionorcriminalprosecutionstatingwhetherDr.
Skoblarhas maintained sobriety.
16. IntheeventthatDr. Skoblarhasnotbeenengaged in theactivepracticeofmedicineand
surgeryfor aperiod in excessoftwoyearspriorto applicationfor reinstatement, the
Boardmayexercise its discretion underSection4731.222,OhioRevised Code,to
requireadditionalevidenceofDr. Skoblar'sfitnessto resumepractice.
REQUIREDREPORTINGBYLICENSEE
17. WithinthirtydaysoftheeffectivedateofthisConsentAgreement,Dr. Skoblarshall
provideacopyofthisConsentAgreementtoall employersorentitieswith which he is
undercontracttoprovidehealthcareservices(includingbutnot limited tothird party
payors)oris receivingtraining; and theChiefofStaffateachhospital wherehe has
privilegesorappointments.Further,Dr. Skoblarshall promptlyprovideacopyofthis
ConsentAgreementto all employersorentitieswith which hecontractstoprovidehealth
careservices,orappliesfororreceivestraining, and theChiefofStaffat each hospital
whereheappliesfororobtainsprivilegesorappointments. IntheeventthatDr. Skoblar
providesany healthcareservicesorhealthcaredirectionormedicaloversightto any
emergency medicalservicesorganizationoremergencymedicalservicesprovider,
within thirtydaysof theeffectivedateofthisConsentAgreementDr. Skoblarshall
provideacopyofthisConsentAgreementto the OhioDepartmentofPublicSafety,
DivisionofEmergencyMedicalServices. Further,Dr. Skoblarshall providetheBoard
withoneofthefollowing documentsasproofofeachrequired notificationwithinthirty
daysofthedateofeachsuch notification: (1)thereturnreceiptofcertifiedmail within
thirtydaysofreceivingthatreturn receipt, (2)an acknowledgementofdelivery bearing
theoriginal inksignatureofthepersonto whomacopyoftheConsentAgreementwas
hand delivered, (3) theoriginal facsimile-generated reportconfirmingsuccessful
transmissionofacopyoftheConsentAgreementtothepersonorentityto whomacopy

of theConsentAgreementwasfaxed, or(4) an originalcomputer-generated printoutof
electronicmail communicationdocumentingtheemailtransmissionof acopyofthe
ConsentAgreementto thepersonorentitytowhomacopyoftheConsentAgreement
wasemailed.
18. Within thirtydaysof theeffectivedateof thisConsentAgreement,Dr. Skoblarshall
provideacopyofthisConsentAgreementto the properIicensingauthorityof anystate
orjurisdiction in whichhecurrentlyholdsany professional license, aswellasany
federal agencyorentity, including butnotlimited totheDrugEnforcementAgency,
throughwhichhecurrentlyholdsany licenseorcertificate. Dr. Skoblarfurtheragreesto
provideacopyof thisConsentAgreementattimeof application to theproper licensing
authorityof anystate in whichheappliesfor any professional licenseorreinstatementof
any professional license. Further, Dr. Skoblarshall providethe Boardwithoneofthe
followingdocumentsas proofofeachrequired notificationwithinthirtydays ofthedate
of eachsuch notification: (1)thereturn receiptof certifiedmailwithinthirty days of
receivingthatreturn receipt, (2) an acknowledgementof delivery bearingtheoriginal ink
signatureofthe person towhomacopyof the ConsentAgreementwas hand delivered,
(3)theoriginal facsimile-generated reportconfirmingsuccessfultransmissionof acopy
oftheConsentAgreementto the personorentitytowhomacopyoftheConsent
Agreementwas faxed, or(4)anoriginalcomputer-generatedprintoutof electronicmail
communicationdocumentingthe emailtransmissionof acopyoftheConsentAgreement
to thepersonorentityto whomacopyoftheConsentAgreementwasemailed.
19. Dr. Skoblarshall promptlyprovideacopyofthisConsentAgreementtoall personsand
entitiesthatprovideDr. Skoblarchemicaldependencytreatmentormonitoring. Further,
Dr. Skoblarshall providetheBoard with oneofthefollowing documentsas proofof
eachrequired notificationwithinthirty daysof thedateof eachsuch notification: (1) the
returnreceiptof certified mail withinthirtydays of receivingthatreturnreceipt, (2)an
acknowledgementof delivery bearingtheoriginal inksignatureoftheperson to whom a
copyof theConsentAgreementwas hand delivered,(3)theoriginal facsimile-generated
reportconfirmingsuccessfultransmission of acopyof theConsentAgreementtothe
personorentitytowhomacopyoftheConsentAgreementwas faxed, or(4) an original
computer-generatedprintoutofelectronicmail communicationdocumentingtheemail
transmissionofacopyoftheConsentAgreementtothepersonorentity to whom acopy
oftheConsentAgreementwas emailed.
20. Dr. Skoblarshall notifytheBoard in writingof anychangeofprincipal practiceaddress
orresidenceaddresswithinthirty daysofsuchchange.
DURATION/MODIFICATION OF TERMS
Theabove-describedterms, conditionsand limitationsmay be amended orterminated in writingat
anytime upontheagreementofbothparties. IntheeventthattheBoard initiatesfuture formal
proceedingsagainstDr. Skoblar, including butnot limited to issuanceof aNoticeof Opportunity
for Hearing,thisConsentAgreementshallcontinue in full force and effectuntil suchtimethatit is

superseded by ratification by the Board of a subsequent Consent Agreement or issuance by the
Board of a final Board Order.
In the event that any term, limitation, or condition contained in this Consent Agreement is
determined to be invalid by a court of competent jurisdiction, Dr. Skoblar and the Board agree that
all other terms, limitations, and conditions contained in this Consent Agreement shall be unaffected.
FAILURE TO COMPLY
If, in the discretion of the Secretary and Supervising Member of the Board, Dr. Skoblar appears to
have violated or breached any term or condition of this Consent Agreement, the Board reserves the
right to institute formal disciplinary proceedings for any and all possible violations or breaches,
including but not limited to, alleged violations of the laws of Ohio occurring before the effective
date of this Consent Agreement.
ACKNOWLEDGMENTSILIABILITY RELEASE
Dr. Skoblar acknowledges that he has had an opportunity to ask questions concerning the terms of
this Consent Agreement and that all questions asked have been answered in a satisfactory manner.
Any action initiated by the Board based on alleged violations of this Consent Agreement shall
comply with the Administrative Procedure Act, Chapter 119., Ohio Revised Code.
Dr. Skoblar hereby releases the Board, its members, employees, agents, officers and representatives
jointly and severally from any and all liability arising from the within matter.
This Consent Agreement shall be considered a public record as that term is used in Section 149.43,
Ohio Revised Code. Further, this information may be reported to appropriate organizations, data
banks and governmental bodies. Dr. Skoblar acknowledges that his social security number will be
used if this information is so reported and agrees to provide his social security number to the Board
for such purposes.
EFFECTIVE DATE
It is expressly understood that this Consent Agreement is subject to ratification by the Board prior
to signature by the Secretary and Supervising Member and shall become effective upon the last date
of signature below.
