Access Consent Form for ICT Resources

This Consent Form must be signed prior to students being granted access to the internet and other
information and communication technology resources at St. Catherines School.
Parents/guardians are encouraged to review and discuss the contents of the Acceptable Use of
Information & Communications Technology Resources Student Agreement (see following pages) with
the student and answer any questions that they may have.
By signing this Consent Form, both parents/guardians and students are agreeing to the terms of access as
set out in the Acceptable Use of Information & Communications Technology Resources Student
Agreement and acknowledge that they will be responsible in the event of any breach and that appropriate
disciplinary steps may result.
Any queries in relation to this material should be directed to the School Principal, Mr Paul Leeson.
User Acceptance
I, the student named below, hereby agree to comply with all requirements as set out in the
Acceptable Use of Information & Communications Technology Resources Student Agreement and all
other relevant laws and restrictions in my access to the various information and communication
technology resources through the BCE and school network.

Parent/Guardian Consent
As the parent or legal guardian of the student named above, I grant permission for the student
named above to access the various information and communication technology resources (including
email and the internet).
I understand that access is granted to students subject to the restrictions contained in the
Acceptable Use of Information & Communications Technology Resources Student Agreement and
that if breached, appropriate consequences may follow.
I acknowledge that some material available on the internet may be objectionable and that in
addition to the Acceptable Use of Information & Communications Technology Resources Student
Agreement, I have discussed appropriate restrictions for the student when accessing or sharing
information or material over the internet.

Respecting Myself and Others

1. I will keep in mind that my technology use a!ects others.
2. I will treat others in the same way that I wish to be treated.
3. I will respect others rights to feel safe.
4. I will not hold accounts or participate in any social media sites intended for people 13 years and
older (such as Facebook and MySpace) without express written advice from my teachers to my
parents/guardians that such an account is required for learning and teaching purposes.
5. I will respect the conditions (terms, rules, regulations etc) of all software, apps and ICTs I use.

whenever necessary.
26. I will be aware of my social and ethical responsibilities in relation to digital information security,
including:

protecting the rights, identity, privacy and emotional safety of online audiences

avoiding and prevent bullying

ensuring the security of myself by not giving out personal information including my name,
age, address and phone number.

ensuring the security of others when posting photos of friends, peers or family. I will ensure
images are appropriate for publishing, that I have permission (from all involved) and that I
dont identify names and places.

27. I will regularly reect on and evaluate my learning. I will try to identify the personal
characteristics that contribute to or limit my success.

technology. I will remember that I need to balance periods of technology use with other activities.
Finally...
I realise that use of ICTs in school is a privilege and carries great responsibility. I will always follow
the guidance provided by my teachers and parents when using ICTs. I will follow the procedures put
into place to protect my work and information and respect others information. If I am unsure or
need clarication I will always ask for it.
Consequences
I realise that if I do not follow appropriate digital citizenship practices the school may inform my
parents/caregivers. In serious cases, the school may take disciplinary action against me. My family
may be charged for repair costs.
If illegal material or activities are involved or e-crime is suspected, it may be necessary for the
school to inform the police and hold securely personal items for potential examination by police.
Such actions may occur even if the incident occurs o!-site and/or out of school hours.
