GENERAL GUIDELINES
1. Only candidates for high school graduation of the current school year are qualified to take the UST Entrance Test (USTET) 2. The following applicants must secure clearance from the UST Registrar's Office before submitting their Application and Grades form: a. Passers of the Philippine Educational Placement Test (PEPT) and the Non-Formal Education Accreditation and Equivalency (NFEA&E) b. Students who are graduates of previous school years c. Candidates for graduation from foreign schools d. Foreigners graduating from local schools (Please bring original academic records, certification for passing PEPT/NFEA&E and alien certificate of registration papers or passport, when 3. 1. Applicants may apply on site or online (ofad.ust.edu.ph). 2. Submit the following together with the accomplished Application and Grades Form: a. Clear photocopy of NSO birth certificate b. Two (2) 2" x 2" recent identical pictures with white background and with name and signature at the back c. Non-refundable application fee 3. Submitted documents become property of UST - Office for Admissions (OFAD). PEPT and NFEA&E passers, students who are graduates of previous school years, candidates for graduation from foreign schools foreigners graduating from local schools must secure clearance from the UST Registrar's Office (Mondays to Fridays only). 5. Enrollment in any program or vocational course prior to the release of the USTET result shall lead to the nullification of the examination taken. The fees paid for the USTET shall be forfeited in favor of the University. 6. Any misrepresentation of information written on this form, or any violation of the instructions given above shall be grounds for forfeiture of right to enroll, or debarment in the succeeding semester if discovered in the current semester.
